    Shorttime impact of allopurinol on arterial elasticity in
    patients with hyperuricemia

    Objective     To evaluate the shortterm impact of allopurinol on arterial elasticity in the patients with hyperuricemia. Methods          59 cases of hyperuricemia, treated for 3 months, were randomly divided into allopurinol and control group. Blood pressure, blood glucose, lipids, hepatic and renal function, hsCRP, UA, brachialankle pulse wave velocity(baPWV) and anklebrachial index (ABI ) were observed before and after the treatment, and the side effects of the drug was also observed. Results   All patients accomplished the observation without serious side effects. In allopurinol group, systolic blood pressure (P<0.01), diastolic blood pressure (P<0.01), and hsCRP(P<0.05) were significantly lower than that of the control group. In allopurinol group, systolic blood pressure (P<0.01), diastolic blood pressure P<0.01), hsCRP(P<0.01), UA(P<0.01), and baPWV(P<0.01) decreased significantly after the treatment.  In control group,  hsCRP(P<0.01), UA(P<0.01) decreased significantly after the treatment. Allopurinol and the decrease of hsCRP were independent impact factors for baPWV by multiple stepwise regression analysis. Conclusion     Allopurinol is safe and effective in the treatment of hyperuricemia, which can lower blood pressure and baPWV, improve blood vessel elasticity, and reverse early atherosclerosis. The decline in baPWV is associated with the improvement of inflamation state, which doesn′t depend on the change in UA.

    Objective  To investigate the effect and mechanism of ulinastatin on learning and cognitive function of hippocampus after ischemiareperfusion injury in rats. Methods  The focal ischemia reperfusion model was established by thread embolism of middle cerebra1 artery. The animals were sacrificed at 24h after the reperfusion. Morphological examination of the CA1 region of hippocampus was perfromed with H&E staining. The expression of nuclear factor kappa B p65 protein(NF-κB p65) was measured by Western blot. The levels of tumor necrosis factor α(TNF-α)and interleukin 10 (IL-10) were determined by enzymelinked immunosorbent assay. The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) were determined by biochemical colorimetry. Morris water maze test was done after the administration of ulinastatin for the other  30 Wistar rats. Results  The pyramidal cells of hippocampus of rats were  protected by ulinastatin.  After the administration of ulinastatin, the expression of NF-κB p65 in the hippocampus, serum level of IL-10 and SOD increased, while the serum level of TNF-α, and the level of MDA in brain tissue homogenate decreased decreased. Meanwhile, ulinastatin could decrease the latency of escaping, increase the percentage of swimming distance in the platform quadrant and number of entries in the target area. Conclusions  Ulinastatin can improve the learning and memory ability of focal brain ischemiareperfusion injury model rats. Antiinflammatory and antioxidant effects are attributed to the basic mechanism.

    Objective   To detect expressions of inflammatory factors, ICAM-1,VCAM-1 and TNF-α, in rats after lithium-pilocarpine-induced status epilepticus(SE) and the effect of tacrolimus(FK506) on them. Methods   168 adult male Wistar rats were randomly divided into the SE group, the FK506 group and the control group. The hippocampal neuronal damage after SE was observed by HE staining, and expressions of ICAM-1, VCAM-1 and TNF-α mRNA and proteins were detected using immunohistochemistry and real-time quantitative PCR method, respectively. Results    Compared with the control group, expression of TNF-α mRNA arrived at the peak 5h after SE. Whereas, ICAM-1 and VCAM-1 mRNA expressions reached maximum levels 1d after SE and remained to elevate until 7d after SE. Pre-treatment with FK506 could remarkably decrease mRNA levels of these inflammatory factors. Significant increases in ICAM-1 and VCAM-1 protein expressions were detected at SE 1d, which were remarkably attenuated by FK506. FK506 also obviously alleviated hippocampal neuronal damage. Conclusion   Inflammatory reaction may play an important role in the pathogenesis of epilepsy. FK506 exerts anti-epileptic and neuroprotective effects through inhibition of inflammatory reaction.

    Objective   To investgate the effect and mechanism of norepinephrine(NE) on glioblastoma invasion and migration, and to evaluate the possibility of β-blocker propranolol as an anti-invasion agent. Methods   RT-PCR was performed to detect the expression of β-adrenergic receptors in T98G, U251 and U87 cell lines. Transwell assay was performed to evaluate the effect of NE on glioblastoma invasion. Gelatin zymography was used to assess the effect of NE on MMP-2 activity in glioblastoma. Results   Compared with the control group, NE (0.1μmol/L~10μmol/L) significantly induced glioblastoma cell invasion. The β-receptor antagonist propranolol (1nmol/L) could block the NE induced glioblastoma invasion. The activity of  MMP-2 increased significantly after NE (0.1μmol/L~10μmol/L) stimulation, which could be abrogated by propranolol (1nmol/L). Conclusion   NE can promote MMP-2 secretion in glioblastoma to facilitate its invasion, which can be inhibited by Propranolol.

    Objective   To construct an eukaryotic vector of human sorbin and SH3 domain containing 1 (hSORBS1) gene and identify the expression and localization of hSORBS1 in osteosarcoma MG-63 cells. Methods   Using GST-hSORBS1 as a template, we obtained human SORBS1 coding sequence by polymerase chain reaction (PCR) amplification and cloned it into the eukaryotic vector 3*Flag. The insert was identified by restriction enzyme digestion and DNA sequencing. 3*Flag-hSORBS1 was transfected into osteosarcoma MG-63 cells. The expression of the recombinant plasmid in MG-63 cells was detected by Western blot. The localization of 3*Flag-hSORBS1 in MG-63 cells was observed with laser scanning confocal microscopy. The hSORBS1 fusion proteins were purified by immunoprecipitation assay. Results   hSORBS1 was constructed into the eukaryotic vector 3*Flag successfully. The expression of 3*Flag- hSORBS1 fusion protein was detected by Western blot, which localization was at the periphery of MG-63 cells. Conclusion   The eukaryotic expression plasmid of hSORBS1 is successfully constructed. The expression of fusion proteins is identified. 3*Flag-hSORBS1 fusion protein is localized at the cell periphery.

    Objective   To introduce a recombinant VP1 gene of EV71 into the Bacillus subtilis genome by means of homologous recombination to display VP1 on the spore as a mucosa vaccine for enterovirus 71. Methods   A recombinant VP1 gene fused with the Bacillus subtilis CotB gene was introduced into integration vector pDG1662 to  generate plasmid pDG1662-CotB-VP1. Bacillus subtilis 1A771 was transformed with linearized pDG1662-CotB-VP1 by means of electroporation. Recombinant strains were screened by antibiotic resistance and were identified by PCR, Western-Blot and immunofluorescence microscopy. Result   The VP1 gene was integrated into the Bacillus subtilis genome and the VP1 protein was displayed on the spore′s surface. Conclusion   The success  of VP1 protein expression on the spore′s surface is a major achievement toward constructing a mucosa vaccine for enterovirus 71.

     Objective   To evaluate the therapeutic effect of 125Iodine seeds on the rabbit VX2 model and to examine changes in tissues of hepatoma and normal liver.  Methods   Twelve VX2 model animals made from  New Zealand rabbits were divided into two groups,a therapy group and a control group. 125Iodine seeds and dummy seeds were implanted into the tumors. CT-enhanced scanning and hepatic function examination were performed before the implantation and at 7, 14, 28 and 60 d after implantation. Pathological study of tumor and normal liver was done to evaluate the damage caused by 125I seeds.  Results   The tumors  in the therapy group were smaller than in the control group at 7, 14, 28, and 60 d after implantation. In the therapy group, the ALT value was decreased  after 14 d, and the AST value was decreased  after 7 and 14 d after implantation, as compared to those in the control group. Radiation injury was not detected with CT  in the therapy group over the period of 60 d. Conclusion   Brachytherapy with 125I seeds can significantly inhibit tumor growth while retaining  liver function; hence it is a safe and effective technique.

    Objective   To investigate induction of apoptosis and generation of reactive oxygen species (ROS) of human gastric cancer SGC-7901 cells in vitro by 5,7-dihydrox-8-nitrochrysin(NOC). Methods   SGC-7901 cells were cultured in vitro and the percentage of Sub-G1 cell population was detected by PI stained flow cytometry (FCM). Cell histone-associated DNA fragments were examined by ELISA assay. The intracellular accumulation of ROS was determined by FCM using the fluorescent probes H2DCFDA. Results   NOC significantly increased the percentage of Sub-G1 cell population and histone-associated DNA fragments in SGC-7901 cells in a concentration-dependent manner (P<0.01). NOC also markedly promoted accumulation of ROS(P<0.01). The pre-incubation of N-acetylcysteine, an antioxidant, effectively antagonized the induction of apoptosis of SGC-7901 cells by NOC. Conclusion   NOC induced apoptosis of SGC-7901 cell line is associated with induction of ROS generation.

    Inhibitory effect of Radix Actinidiae extractive on transplantation

    Objective   To study the inhibitory effect and apoptosis-inducing action of acetidin extractive of Radix Actinidiae on transplantation tumor of lung cancer A549 cells in nude mice, and to explore their mechanisms. Methods   A model of transplantation tumor bearing lung cancer A549 cells was established in nude mice, and the effect of acetidin extractive of Radix Actinidiae in vivo was detected. The control group and treatment groups (highdose group and low-dose group)were set for comparison.According to the transplantation tumor volume, the transplantation tumor growth curve was drawn. According to the final tumor weight, the inhibitory ratio was calculated. Terminal deoxynucleotidyl transferase mediated UTP nick end labeling (TUNEL) was used to determine the apoptosis index of transplantation tumor. Expressions of Survivin protein and mRNA in transplantation tumor were detected by immunohistochemistry (SP) and RT-PCR, respectively. Results The growth of transplantation tumor in treatment groups was slow after taking drug. The final tumor volume and tumor weight in treatment groups were smaller than those in the control group. Tumor weight inhibitory rates in the high-dose group and low-dose group were 69.00% and 45.09%, respectively. The apoptosis index of transplantation tumor in treatment groups was higher than that in the control group. Expression of Survivin mRNA and protein in treatment groups were lower than those in the control group, and they was more signifi-cantly decreased in the high-dose group. Differences were significant in the above indexes between each two groups(P<0.01). Conclusion   The acetidin extractive of Radix Actinidiae has an inhibitory effect on growth of transplantation tumor of lung cancer A549 cells in nude mice, inhibiting proliferation activity of transplantation tumor, degrading expressions of Survivin protein and mRNA, and inducing apoptosis of cancer cells.

    Effects of bortezomib in combination with epirubicin on the
    proliferation and apoptosis of breast cancer cells

    Objective   To study effects of bortezomib, a ubiquitin-proteasome pathway inhibitor with cytotoxic activity in multiple myeloma in vitro and vivo, alone and in combination with epirubicin on biological characteristics of breast cancer cells. Methods   Michigan Cancer Foundation-7(MCF-7) human breast cancer cell lines were cultured with bortezmib (in concentrations of 0.01, 0.10, 1.00, 5.00 and 10.00μg/mL) alone or plus epitubicin (in concentrations of 0.05, 0.25, 0.50, 1.00 and 5.00μg/mL). Effects of bortezomib and epirubicin on the proliferation and viability of breast cancer cells were studied by the MTT assay. MCF-7 cells were cultured with 0.10μg/mL of bortezomib+0.50μg/mL of epirubicin for 24, 48 and 72hours. The cells stained with propidium iodide were detected for cell cycle analysis. Results   Bortezomib inhibited cell proliferation to some extent, without showing a dose-or time-dependent manner, and it stopped the cell cycle at the G2/M period. Epirubicin significantly inhibited  proliferation of MCF-7 human breast cancer cells in a dose- and time-dependent manner, arresting the cell cycle at the S period. Combination of bortezomib and epirubicin could inhibit proliferation of MCF-7 cells at lower concentrations and shorter time, indicating their synergistic effect. Conclusion   Bortezomib and epirubicin have a synergistic effect over the inhibition of MCF-7 human breast cancer cells. The research shows that the dose of epirubicin can be decreased and its sensitivity can be increased if it is applied with bortezomib, thereby enhancing chemotherapeutic effects and reducing adverse effects of the drugs.

    The mRNA-stabilizing factor Human-Antigen R regulates renal cyclinD1

    Objective   To investigate the role of mRNA stabilizing factor Human-Antigen R(HuR) in angiotensinⅡ(AngII)-stimulated cyclinD1 over-expression in cultured human mesangial cells. Methods   Human mesangial cells cultured in vitro were divided into 0A group,3A group, 6A group, 9A group and 24A group according to the time gradient of angiotensin II management(0h,3h,6h,9h,and 24h). Cell cycle was detected by flow cytometry and subcellular localization of  HuR protein was determined by immunochemistry. Total cyclinD1 protein ,cytoplasmic and total HuR protein expression were detected by Western blot and total cyclinD1  mRNA level was determined by RT-PCR. Using RNA interference technology to down regulate HuR protein level, we surveyed protective effect of angiotensinⅡ on total cyclinD1 protein expression. Results   Compared with 0A group, there was a marked increase in mesangial cells proliferation in 24A group(P<0.05). HuR protein shifted from nucleus to cytoplasm, especially in 3A group. There was no significant variation of total HuR protein expression but an increase in cytoplasmic HuR protein especially in 3A group(P<0.05). An increase in total cyclinD1 protein expression was observed in 24A group(P<0.05), but no change in RNA level. Compared with transfection before, there was a marked decrease in cytoplasmic HuR protein expression in 3A group(P<0.05) and total cyclinD1 protein expression in 24A group(P<0.05), but no change in cyclinD1 RNA level. Conclusion   ①AngⅡ induces proliferation of human mesangial cells.②HuR protein shifts from nucleus to cytoplasm induced by AngⅡ.③HuR protein is involved in the expression of cyclinD1 protein with AngⅡ stimulation.

    Expression of TSP-1,TGF-βand PPAR-γin glioma and
    relationships with microvascular density

    Objective   To explore the expressions of thrombospondin-1(TSP-1), transforming growth factor-β(TGF-β), peroxisome proliferator activated receptor-γ(PPAR-γ) in glioma tissues and its correlations with tumor angiogenesis. Methods   Expressions of TSP-1, TGF-β and PPAR-γ and microvascular density(MVD) in 62 glioma cases of low and high grades were examined with SABC immunohistochemistry. Results   The  expression rates of TSP-1, TGF-β and PPAR-γ in low-grade gliomas were 78.1%, 43.7% and 93.7% respectively; the expression rates in high-grade gliomas were 46.7%, 93.3% and 40.0% respectively, the expression of  TSP-1 and PPAR-γ both had negative correlations with TGF-β(P<0.05). It was found that the MVD of low-grade gliomas(26±5.2) was significantly lower than that of high-grade gliomas( 41±7.3)(P<0.05). TSP-1 and PPAR-γ had  negative correlations with MVD(P<0.05), TGF-β had a positive correlation with MVD (P<0.05). Conclusion   The expression levels of TSP-1, TGF-β and PPAR-γ are correlationed with glioma MVD and and these factors may be involved in glioma angiogenesis.

    Relationship between the cognitive function level and cerebral

    Objective   To investigate the relationship between the cognitive function level and cerebral hemodynamics in patients with leukoaraiosis. Methods   Ninety patients with LA were subjected to  cerebral MRI, neuropsychological and TCD examinations. On the basis of  neuropsychological scoring these patients were divided into three groups: without cognitive dysfunction, mild cognitive impairment and dementia. Pulsatility index (PI) and mean blood flow velocity (Vm) of middle cerebral artery (MCA) were studied using variance analysis and correlation analysis. Results    PI Differences PI amongthe three groups were significant (P<0.05),and there was a positive correlation between PI and cognitive function level (r=0.688,P<0.01). By contrast, Vm differences among the three groups were not significant (P>0.05). Conclusion   The level of cognitive function in LA patients and their PI are related, and the greater the PI, the more severe the cognitive impairment. On the other hand, the level of cognitive function in LA patientswas not correlated with the Vm values.

    Objective   To investigate expression of Toll-like receptors 2 and 4(TLR2、TLR4) and nuclear factor-κB(NF-κB) in colonic mucosa of ulcerative colitis. Methods   Immunohistochemistry was performed to examine the expression of TLR2、TLR4 and NF-κB in colonic mucosa of UC (n=70). Normal mucosa of colon polyps was included as a control (n=44). Results   The expression of all three proteins in colonic mucosa of UC was higher than that in control (P<0.01). Cell numbers with positive expression were parallel to the endoscopic classification grades in UC (P values<0.05). Expression of the three proteins in active UC was significantly higher than in unactive UC (P values<0.01). Further, the expression of NF-κB was positively correlated with TLR2 and TLR4(r=0.823, P<0.01;r=0.752, P<0.01, respectively). Conclusion   The elevated expression of TLR2、TLR4 and NF-κB in UC may play a role in UC pathogenesis and, hence, may be of clinical significance.

    Dosimetric evaluation of three techniques in postoperative
    radiotherapy  for  gastric  carcinoma

    Objective   To evaluate threedimension  conformal radiation therapy (3D-CRT), intensity-modulated radiation therapy (IMRT) and simplified intensity-modulated   radiation therapy (sIMRT) in post-operative radiotherapy for gastric cancer. Methods   A total of 10 patients with gastric body cancer were chosen for this study. All were of the T3 or T4 type, with or without  lymph  node metastasis, and surgery had been performed. All patients were  subjected to 3D-CRT, IMRT and sIMRT   treatment regimes. The dose prescriptions were 45.00Gy to 95% of PTV and 42.75Gy to 99% of PTV. DVHs and 3D dose distributions of the three regimes were compared. Results   IMRT, sIMRT and 3D-CRT had  comparable PTV coverages with the prescribed doses, but IMRT and sIMRT  showed better dose conformity than 3D-CRT. For a normal liver, V23, V30 and mean dose  for IMRT and sIMRT  were similar and  lower than those for 3D-CRT, although these doses were  still within the clinical tolerance range for 3D-CRT. V15 and  V23 for sIMRT showed the lowest doses for kidneys, as contrasted to  3D-CRT being the highest. IMRT, sIMRT and 3D-CRT  had comparable maximum doses to the spinal cord. Conclusions   IMRT, sIMRT and 3D-CRT have adequate and  accurate PTV coverages for the prescribed doses in postoperative radiotherapy for gastric carcinoma,and can protect  neighboring organs.  Overall, sIMRT is the best among the three  techniques.

    Influence of acid-inhibitory drugs on diagnosis efficiency for gastric cancer

    Objective   To analyze the influence of proton-pump inhibitors (PPIs) or histamine 2-receptor antagonists(H2RAs) on diagnosis efficiency based on serum pepsinogens (PGs, PGI & PGII) and gastrin-17( G-17). Methods   The serum PGs and G-17 levels were measured in 190 patients (102 in the gastric cancer group, 30 in precancerous lesion group, and 58 in control group) by chemiluminescence microparticle immunoassay and ELISA, respectively. Values of PGR (PGI/PGII) and GPR (G-17/PGI) were calculated. Results   The median levels of PGs and G-17 were significantly (P<0.05) increased among patients who used PPIs but  not H2RAs, compared to non-users. PGR and GPR values were similar between PPI users and non-users. Use of H2RAs did not cause significant changes in these values either. Gastric cancer patients had significantly lower PGR and higher GPR than those in the other two groups(P<0.05). ROC curve analysis showed that AUC for PGR and GPR were 0.836 and 0.720, respectively. The best cutoff values of PGR and GPR for detecting gastric cancer calculated by ROC curve were 5.9 and 90, respectively, with moderate diagnostic sensitivity (76.5% and 60.8%, respectively) and specificity (70.5%). Conclusion   PPIs, but not H2RAs, markedly increased the fasting levels of serum pepsinogens and G-17, independent of gastric mucosa changes.Meanwhile, PGR and GPR values could remain stable with the assumption of acid-inhibitory drugs. PGR and GPR can be used for gastric cancer diagnosis in populations with interference from acid-inhibitory drugs.

    Objective   To examine the expression of vitamin D receptor (VDR) in gastric cancer and normal mucosa adjacent to cancer. Methods   VDR expression in tumor tissue and normal mucosa was detected by immunohistochemistry for 75 cases of gastric carcinoma. Results   VDR expression in gastric tumor was significantly lower than that in normal mucosa (P<0.05). By contrary, VDR expression did not show significant correlation with gender, age, histological classification, TNM stage, or infiltration depth. Yet, a significant correlation was found between VDR expression and carcinoma differentiation status: well differentiated carcinoma had the highest level of VDR expression (P<0.05) among all stages. Conclusions   VDR expression can be considered as a marker of gastric cancer differentiation.

    Objective   To investigate the expression and clinical significance of Mel-18 in esophageal squamous cell carcinoma(ESCC). Methods   Esophageal squamous cell carcinoma and corresponding normal tissues of 53 patients were obtained from Department of Chest Surgery, Qianfoshan Hospital of Shandong University. The reverse transcription polymerase chain reaction (RT-PCR) was used to detect the expression of Mel-18 in the cancer and normal tissue. Further, the association between Mel-18 expression and the clinical parameters of the tumors, including gender, tumor location, size, histological differentiation, invasive depth, clinical stage, and lymph node metastases, were analyzed. Results   There was significantly lowered expression of Mel-18 in the carcinoma tissues than that in the corresponding normal tissues(P<0.01). No correlation between the Mel-18 expression and any clinicopathological parameters, such as gender, tumor location, size, histological differentiation and invasive depth, were identified (P>0.05). The expression levels of Mel-18 in TNM stageⅠ, Ⅱ, Ⅲ and Ⅳ reduced consequently (P<0.05). The difference of expression between the groups with and without lymph node metastases was significant (P<0.05). Conclusion   Mel-18 might play a role to suppress carcinogenesis and progression of ESCC. It is possible that Mel-18 could be used as a bio-marker for diagnosis and prognosis .

    Objective   To investigate values of interleukin-6(IL-6)and soluble E-selectin(sE-selectin) in evaluating severity and outcome of pneumonia. Methods   37 patients with severe community-acquired pneumonia (CAP), 35 patients with non-severe CAP, and 36 healthy subjects were selected as groups A, B and C. Levels of IL-6 and sE-selectin were detected by ELISA, and high sensitivity C-reactive protein (hsCRP), percentage of neutrophils(PMN%), and PaO2/FiO2 were also determined. Results   Levels of IL-6 and sE-selectin were significantly higher in group A than those in groups B and C(P<0.001), and also higher in patients with  serere CAP complicated by multiple organ dysfunction(MODS) than in patients without MODS(P<0.001). Levels of IL-6, sE-selectin, hsCRP and PMN% were all greatly decreased in patients with CAP after treatment (P<0.001).The serum level of IL-6 had positive correlations with sE-selectin, hsCRP and PMN% in patients with pneumonia (r=0.406, 0.752, 0.678, respectively; P=0.006, 0.008, 0.009, respectively),while it had a negative correlation with PaO2/FiO2(r=-0.612, P=0.007). sE-selecttin also had positive correlation with hsCRP and PMN% (r=0.832, 0.791, respectively; P=0.009, 0.007, respectively),while it had negative correlation with PaO2/FiO2(r=-0.689,P=0.009).Conclusion   Detection of serum IL-6 and sEselectin has good clinical values in accessing severity and outcome of pneumonia.

    Objective   To investigate the relationship between the leptin receptor gene Pro1019Pro polymorphism and obstructive sleep apnea hypopnea syndrome(OSAHS). Methods   The genotypes of leptin receptor gene Pro1019Pro were detected by polymerase chain reactionrestriction fragment length polymorphisms(PCRRFLP) in 360 unrelated subjeces in Northern Han population(including 180 OSAHS subjects and 180 healthy controls). The difference were analyzed. Results   There was no significant difference in Genotypes and allele frequencies betwwen the OSAHS and  control group(P>0.05). The differences in neck circumference, abdominal circumference, and polysomnography (PSG) were not significant among the three genotypes(P>0.05). Conclusion   It is unlikely that the leptin receptor gene Pro1019Pro polymorphism may contribute to the pathogenesis of OSAHS.

     Objective   To explore the diagnosis and treatment of primary adrenal lymphoma(PAL). Methods   Two cases of PAL(males, at a mean age of 80 years) from June 2001 to January 2011 in our hospital were retrospectively analyzed. The disease, without special clinical manifestation, was found by physical examination. Tumors were located on the left adrenal gland, and averaged 5.7(5.5-6) cm in diameter. The two patients were treated with operation, one with laparoscopic surgery, and the other with routine open surgery. Results   The tumors were pathologically diagnosed as diffuse large B cell lymphoma of the left adrenal gland. After operation, they were treated with R-CHOP (rituximab+ cyclophosphamide+ doxorubicin+ vincristine+ prednisone) regime, and followed up for 6 and 20 months,respectively. Conclusions   PAL is an extremely rare malignant tumor, and usually occurs in old men without special clinical manifestation. It could be definitely diagnosed dependent on pathological examination. PAL, not sensitive to treatment, has a poor prognosis.

    Objective   To investigate changes of serum visfatin and tumor necrosis factor-α(TNF-α)in patients with polycystic ovary syndrome(PCOS) and their significances in pathogenesis of PCOS. Methods   50 cases of PCOS patients were collected as the experimental group and 48 subjects as the control group. According to the body mass index(BMI), which was calculated by measuring height and body weight, they were subdivided into obese (BMI≥25kg/m2) and non-obese (BMI<25kg/m2) groups, that was 23 cases in the obese experimental group,27 cases in the non-obese experimental group, 20 cases in the obese control group, and 27 cases in the non-obese control group. Fasting serum visfatin was detected by enzyme linked immunosorbent assay(ELISA), TNF-α by radioimmunoassay (RIA), fasting plasma glucose(FPG) by glucose oxidation enzyme method, and fasting insulin(FIN) by electrochemiluminescence immunoassay method. The homeostasis model assessment-insulin resistance(HOMA-IR) was analyzed. Results   Visfatin and TNF-α levels in the experimental group were higher than those in the control group(P<0.05). The level of visfatin was higher in both obese and non-obese experimental groups than in the non-obese control group, and it was higher in the obese experimental group than in the obese control group. The level of TNF-α was higher in obese and nonobese patients than in the corresponding control groups, and it was higher in the non-obese experimental group than in the obese control group (P<0.05). Visfatin and TNF-α levels were both positively correlated with BMI and HOMA-IR (P<0.05). Conclusion   Increases of visfatin and TNF-α levels in PCOS patients are closely related with HOMA-IR. Visfatin and TNF-α play important roles in occurrence and development of obesity and IR in PCOS patients.

    Objective   To investigate tissue expression and serum level of perm-associated antigen 9(SPAG9) in serous ovarian tumors, and explore its diagnostic significance. Methods   Expression of the SPAG9 protein was retrospectively detected in paraffin sections from 24 benign epithelial ovarian tumors, 27 borderline serous epithelial ovarian tumors, and 32 serous ovarian carcinomas by immunohistochemistry. Enzyme-linked immunosorbent assay (ELISA) was used to detect the level of serum SPAG9 in patients with epithelial ovarian tumors, and the correlation between serum levels of SPAG9 and CA125 was analyzed. Clinical values of SPAG9 and CA125 were evaluated by the receive operating characteristic ( ROC) curve. Results   ①Expression of SPAG9 in serous ovarian carcinomas was obviously higher than that in benign and borderline serous tumors. And it was associated with the grade of tissue differentiation(P<0.05). ② The serum SPAG9 level in serous ovarian carcinomas was statistically higher than that in benign epithelial ovarian tumors. In addition, statistical analysis showed a positive correlation  between serum levels of SPAG9 and CA125(P<0.01). The ROC curve revealed that SPAG9 had a significant value in the diagnosis of serous ovarian carcinomas, while it was not preferable to CA125.  Conclusion   SPAG9, highly expressed in serous ovarian carcinomas, is positively associated with the grade of malignancy. It may serve as a new biological marker  for early diagnosis and post-operative treatment of epithelial ovarian tumors.

    Objective   To investigate expression and significance of forkhead box protein-3 (FOXP3) in normal skins, pityriasis rosea, lichen planus, large-plaque parapsoriasis, and mycosis fungoides. Methods   The immunohistochemical EnVision method was applied to detect expression of FOXP3 in 21 cases of pityriasis rosea,19 cases of lichen planus,17 cases of large-plaque parapsoriasis, 22 cases of mycosis fungoides, and 10 normal skin controls. Results   Rates of FOXP3-positive lymphocytes in lesions of pityriasis rosea and lichen planus had no significant difference compared with normal skins (P>0.05). Rates of FOXP3-positive lymphocytes in lesions of large-plaque parapsoriasis and mycosis fungoides were higher than those in pityriasis rosea, lichen planus, and normal skins(P<0.05), while no significant difference was found between large-plaque parapsoriasis and mycosis fungoides (P>0.05). Conclusions   FOXP3-positive lymphocytes show a gradual increase tendency in  inflammatory skin diseases, tumor tendency inflammatory skin diseases and cutaneous lymphoma. They may play an important role in immune escape of tumors and promote tumors′ occurrence and development.

    Objective   To isolate and identify enterovirus 71(EV71) in the endotracheal aspirate specimen from a patient with hand-foot-mouth disease(HFMD). Methods   The endotracheal aspirate specimen was inoculated in MA104 cells. Total RNA was extracted after the cytopathic effect (CPE) was observed, and the national standard primer was used to detect EV71 by RT-PCR. The VP1 complete sequence was obtained by PCR and then sequenced. The evolutionary tree was structured by MEGA4. Results    Cells showed CPE 3 days after inoculation. The virus was identified as EV71 by RT-PCR. After the VP1 complete sequence was analyzed, the virus was confirmed as C4 serotype, and the evolutionary tree was drew by MEGA4. Conclusions   For the first time, EV71 is isolated from lower respiratory tract secretion from HFMD patients, providing a basis for the study of pathogenesis of lung tissue injury in HFMD patients.

    Objective   Different serum protein levels in pre- and post-operative T1 glottic laryngeal carcinoma patients were analyzed using proteomics methods to determine the tumor biomarker proteins. Methods   Comparative proteomic analysis of human sera between pre-and post-operative T1 glottic laryngeal carcinoma patients was performed using two-dimensional (2D) gel electrophoresis, in combination with matrix-assisted laser desorption ionization time of flight mass spectrometry and database searching, to determine alternative proteins in T1 glottic laryngeal carcinoma patients. Western blot analysis, as well as the measurement of malondialdehyde (MDA) and superoxide dismutase (SOD) levels, was also performed. Results   Image analysis of 2D gels indicated that 16 protein spots corresponding to 12 proteins exhibited significant different expression levels between the two groups of T1 glottic laryngeal carcinoma patients. Six protein expression levels were upregulated. Western blot analysis confirmed the results of proteome analysis. MDA levels in the pre- treatment group were higher than those in the control and post-treatment groups. By contrast, SOD levels in the pretreatment group were lower than those in the control and post-treatment groups. Conclusion   Proteomics results combined with bioinformatics analysis indicate the important roles of oxidant stress and immune response as targets in the monitoring, as well as treatment, of T1 glottic laryngeal carcinoma. The differentially expressed proteins may also reflect the pathological progress of T1 glottic laryngeal carcinoma and should be studied further.

    Objective   To analyze the feasibility and application of mini-thoracotomy in the treatment of heart diseases. Methods   A total of 21 patients with heart disease received mini-thoracotomy approach surgery, including 6 cases of mitral valve replacement, 3 cases of mitral valvuloplasty, 2 cases of the repair of atrial septal defect, 5 cases of the repair of ventricular septal defect, 2 cases of left atrial mucus tumor enucleation, 2 cases of patent ductus arteriosis ligation, and 1 case of minimally invasive coronary artery bypass grafting. Results   The surgical outcomes were successful without  obvious complications. The cardiopulmonary bypass time, the aortic cross clamping time, the duration of postoperative mechanical ventilation and the postoperative hospital stay were (40.3±12.5)min,  (26.4±11.3)min,  (5.1±2.2)hrs, and  (7.1±3.6) days, respectively. Conclusions   Comparing to the conventional median sternotomy approach, mini-thoracotomy  is minimally invasive and can prevent the patients from postoperative sternal deformation. All the patients are satisfied with the quick recovery and excellent cosmetic effect.

    Objective   To investigate the prognostic values of matrix metalloproteinase 9 (MMP-9) in predicting unstable angina pectoris (UAP) after the PCI(Percutaneious coronary intervention) treatment. Methods   A total of 136 patients with UAP were treated with PCI. Blood samples were collected pre- and postoperatively. Enzyme-linked immunosorbent assay was used to detect MMP-9. We recorded important adverse cardiac events (MACE) in 6-month follow-up. Results   MMP-9 concentration dropped after the operation (preoperative 15.9 ± 2.5 ng / mL vs postoperative 10.9±2.3ng / mL, t=17.165, P=0.000). The average MMP-9 decrease rate was 31.4%. Within six-month follow-up, 19 patients had MACE (14.0%). Postoperative MMP-9 concentration in MACE patients was higher than those without MACE (13.9 ± 2.5ng / mL vs 10.5 ± 2.4ng / mL, t=11.441, P=0.000). The area under the ROC curve of MMP-9 decrease rate to predict MACE was 0.723, 95% CI of 0.624-0.754. If the cutoff was set at 15%, the specificity was 78.5%, while the sensitivity was 82.1%. Prognostic analysis showed that incidence of MACE in the group with MMP-9 decreased rate not above 15% was higher than those on the contrary (χ2=2.579, P=0.038). Adjusted by Cox multivariate, OR for the occurrence of MACE was 1.454 (≤15% vs>15%). Conclusion   Matrix metalloproteinase-9 can predict the prognosis for unstable angina pectoris after percutaneous coronary intervention. When the decreasing rate of MMP-9 is below 15%, more attention should be paid for the occurrence of major adverse cardiac events.

    Objective   To invesgate the independent factors influencing the long-term mortality of elderly patients(age more than 70 years old) undergoing coronary artery bypass grajb surgery. Methods   Preoperative,intraoperative and postoperative datas of 345 elderly patients(age more than 70 years old), who underwent coronary artery bypass graft surgery from December of 2005 to May of 2011, were reviewed. They were divided into 2 groups according to the followup results(i.e. dead or not).   45 patients died during the follow-up. Comparing the  datas of the 2 groups,the factors, of which P value was less than 0.05, were included into the Cox multivariate analysis to extract the independent factors influencing the long-term mortality. Results   Among all 345 patientst, there were 74 on-pump coronary artery bypass graft, 219 off-pump coronary artery bypass graft, and 52 cardiopulmonary bypass assistant beating heart coronary artery bypass graft. 15 patients died in the hospital after the surgery(4.35%). For the other 345 patietns, the follow-up period was 72 months(1-72 months, median 68 months, mean 62.557±1.282 months). The accumulated mortality during the follow-up was 13.04% (45/345).5 independent factors influencing the long-term mortality, including intubation time (RR: 1.002;95%CI 1.001-1.003), postoperative renal dysfunction (RR 20.59; 95%CI 6.131-69.154), preoperative CVD (RR 4.605;95%CI 1.796-11.806), regularly intaking statins (RR 0.334; 95%CI 0.158-0.706), and good blood pressure control (RR 0.135; 95%CI 0.047-0.385). Conclusion   The inbubation time, postoperative renal dysfunction, preoperative CVD, regularly intaking statins and good blood pressure control are the independent factors influencing the long-term mortality of elderly patients(age more than 70 years old) undergoing coronary artery bypass graft surgehy.

    Objective   To evaluate the nutritional risk incidence and nutritional support application rate in  patients hospitalized for common internal medicinal?diseases applying the nutrition risk screening (NRS2002) . Methods   Patients hospitalized in Shengjing Hospital Affiliated to China Medical University for seven diseases were screened for nutritional risk, and the incidence of malnutrition and nutritional support status were evaluated. Results   The nutritional risk and the overall incidence of malnutrition?in the seven diseases were 37.8% and 19.1%, respectively. The nutritional risk and the incidence of malnutrition in patients with liver cirrhosis were highest, 61.9% and 47.4%, respectively. The nutritional support rate in all the patients was 6.7%, and that in patients with NRS ≥ 3 points was 14.5%, and the ratio of parenteral nutrition(PN)/enteral nutrition(EN) was 3.6∶1. Conclusion   At present, there are a considerable number of medicinal patients with nutritional risk or malnutrition, while the nutritional support rate is low. The current situation should be actively improved for reducing complications and improving quality of patients′ life. 2

    Stress responses and related factors of patients with coronary
    heart disease after coronary intervention

    Objective   To study the stress responses and related factors of patients with coronary heart disease after coronary intervention. Methods   For 60 patients 3 days post-coronary intervention, a survey was performed using Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), Symptom Checklist-90 (SCL-90), D Personality Questionnaire and Trait Coping Style Questionnaire (TCSQ). Skin resistance was also measured for all patitents. A group of 30 healthy individuals was used as a control. Results   Both total scores and unit scores of SAS, SDS and SCL-90 of the patient group were significantly higher than thoes of the control  group(P<0.05, P<0.01).  The patient group scored significantly higher for both type D personality  and negative affectivity/social inhibition than the control group(P<0.05;P<0.01). The patients had more negative coping style(P<0.05).  In addition, the patient group exhibited lower skin resistance than the control(P<0.05), indicating  an elevated sympathetic responsein the former. It was shown that depression, anxiety and level of mental health had no correlation  with age, gender and education, while a significant correlation was detected   with personality and coping style(P<0.001).  Conclusion   Coronary intervention in patients with coronary heart disease triggered  strong responses  to stresses including anxiety, depression, lower mental health and . These responses were related with personality and negative coping style.

    Objective   To explore the coping styles of patients with major depression. Methods   A total of 303 patients with major depression were diagnosed with SCID-I-P(Study Group). In another group (Control), 303 health individuals were included, who were matched one-to-one to the Study Group, by gender, age and village. All participants were in the database of an epidemiological survey for mental disorders in persons of ≥18 years in Shandong province. The project included Simplified Coping Style Questionnaire (SCSQ), Global Assessment Function (GAF) and Common Data Questionnaire. Results   Active coping scores on SCSQ in the Study Group were lower than those in the Control Group (P<0.01). Active SCSQ scores of farmers, fishermen, older people or those with low education were lower than those of others in both groups (P<0.05). Active SCSQ scores of patients with multiple suicide attempts or with serious social dysfunction were low (P<0.05), but these patients′ passive SCSQ scores were high (P<0.05). Conclusion   Patients with major depression had poorer active coping ability than health people. Age, education, times of attempted suicide and social dysfunction are likely to influence the active coping ability of patients with major depression. The fervency of intension to suicide may be a factor to influence passive coping ability.

    Objective   To construct a predictive model of epileptic seizures based on unbalanced data. Methods   The study included 736 epileptic patients treated in Linyi People′s Hospital from September 2008 to January 2011. Epidemiological investigation on risks factors for seizures was made. As the frequency of seizures in epileptic patients was non-balanced data, the data were made a balance based on the Smote Algorithm. Then the random forest was applied to construct a model to make discriminant prediction on the frequency of seizures. Results   Using the random forest to analyze the data, the correctly classified accuracy was 82.53%, incorrectly classified accuracy was 17.47%, area under the receiver operating characteristic(ROC) curve was 94.2%, and out of bag error(OOB) was 13.3%. Conclusion   The random forest is capable of rapidly discriminating the frequency of seizures after processing unbalanced data, which can provide a scientific basis for the forecast of seizures.

    Objective   To determine the change of blood pressure and evaluate the effect of community comprehensive lifestyle modification on blood pressure control in mid-western rural areas of Shandong Province. Methods   A non-communicable disease control project was launched in mid-western rural areas of Shandong Province from 2007 to 2010. The baseline survey was performed using multi-stage random sampling method in 8 counties of mid-western rural areas in 2007, the participants aged 25 and above were recruited to undergo questionnaire survey, physical examination and blood pressure measurement. Health education focusing on a balance diet, physical activity promotion and tobacco & alcohol control was performed among the intervention population. The terminal survey using the same questionnaire with the baseline was performed in 2010, and the intervention group and control group were selected from intervention and non-intervention areas, respectively. The effect was evaluated compared the difference between the two surveys. Results   Mean systolic blood pressure (SBP) and diastolic blood pressure(DBP) in the baseline population in 2007 were 134.46 and 83.50mmHg, respectively, and those in 2010 were 132.47 and 80.80 mmHg in the intervention group, 133.54 and 81.12mmHg in the control group. Significant decreases in SBP and DBP were found between the  baseline in 2007 and the intervention group in 2010(P<0.05). Insignificant decreases were found between the intervention and  control population in 2010(P>0.05), and there were more obvious decreases in males than in females, and in elderly population than in younger population. Conclusions   Community comprehensive lifestyle modification exerts a significant effect on blood pressure control, especially on males and elderly population. Intervention measures should be strengthened in middle-aged rural population.

    Objective   To put forward a large-scale longitudinal monitoring health check-up data analysis strategy. Methods   The strategy was introduced in data preprocessing, Markov Chain Monte Carlo imputation and generalized estimating equation. Association between white blood cells and metabolic syndrome was taken as an example to illustrate the application of the strategy. Results   After data preprocessing, large missing data was complemented. Using generalized estimating equation in longitudinal analysis, problems caused by ignoring interclass correlation were overcome and the time factor got controlled. Conclusion   Longitudinal monitoring large-scale health check-up data analysis strategy can make full use of information from health check-up records and greatly improve the quality of statistical analysis.

    Objective   To investigate the relationship between GPX1 gene Pro200Leu polymorphism and coronary artery disease (CAD).  Methods   A hospital-based case-control study was conducted on 375 premature CAD, 519 late-onset CAD patients and 334 controls with polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP)  to detect the polymorphism. Logistic regression models were performed to estimate the odds ratios (ORs) with 95% confidence intervals (CIs).  Results   The odds ratio of CT+TT versus CC genotype of Pro200Leu polymorphisms with CAD between early-onset CAD group and controls was statistically significant in multiple logistic model (OR: 1.96, 95%CI: 1.17-3.31). For the comparison between late-onset CAD group and controls, the odds ratio became nonsignificant after adjusting the potential confounders (OR: 1.75, 95%CI: 0.76-4.02).  Conclusion   T allele of GPX1 gene Pro200Leu polymorphism may be a risk factor in early-onset CAD.

    Objective   To investigate traditional Chinese medicine(TCM) prescription, Chinese herbal medicine property and therapeutic effects of TCM as a whole, construct the Bayes network, and clarify the relationship and internal action mechanism among them.  Methods   Based on historical books, such as Shen Nong's Herbal Classic, Famous doctor Don′t Record and New Materia Medica, 60 kinds of TCM were selected to build the Bayes network, and it was used to perform Bayes concluding.  Results   The Bayes network was stable and reliable to evaluate direct and indirect relationship among TCM prescription, properties and therapeutic effects using the Bayes formula.  Conclusion   The Bayes network can be used to investigate the causal and latent relationships among prescription, properties and effects of TCM, providing some theories for understanding TCM.
