    Changes of true insulin secretion in newly diagnosed T2DM  patients after treatment with xiaoke pills

    Objective    To observe characteristic changes of true insulin secretion and ratios of true insulin to immunoreactive insulin(TI/IRI) in patients newly diagnosed as having type 2 diabetes mellitus(T2DM) before and after treatment with xiaoke pills or repaglinide.  Methods    90 patients with newly diagnosed T2DM were randomly divided into two groups: 44 patients in the xiaoke pills group, and 46 patients in the repaglinide group. After collecting fasting, 30min and 120min venous blood before and after treatment, blood glucose was measured with the glucose oxidase method, true insulin with ELISA and immunoreactive insulin with radioimmunoassay.  TI/IRI was calculated at various times. Results     120min TI/IRI in xiaoke pills group had an obvious increase after treatment compared with that before treatment[ (0.60±0.13) vs (0.53±0.14), P<0.05], while 30 min TI/IRI had a minor increase(P>0.05).  30min TI/IRI in the repaginate group had an obvious increase compared with that before treatment[(0.63±0.08) vs (0.44±0.17), P<0.05].  Conclusions    The quality and quantity of true insulin secretion in 120 minutes have obvious improvement after treatment with xiaoke pills. The quality and quantity of true insulin secretion in 30 minutes have an obvious difference compared with treatment with repaglinide.

    Objective    To investigate changes of CD3+, CD4+ and CD8+ T cells and natural killer(NK) cells in neonates born to gestational diabetes mellitus(GDM) mothers and their relationship with blood glucose. Methods33 full-term neonates born to GDM mothers were enrolled as the experimental group and 30 healthy full-term neonates were enrolled as the control group. Numbers of CD3+, CD4+ and CD8+ T cells and activity of NK cells were measured by flow cytometry within 24h after birth. Blood glucose was detected by a trace glucose meter.  Results    Compared with the control group, numbers of CD3+ and CD4+ T cells, activity of NK cells and the ratio of CD4+/CD8+ were lower in the experimental group (P<0.01). The level of blood glucose in the experimental group was lower (P<0.05). The incidence of hypoglycemia in the experimental group was 21.21%, which was higher than that in the control group (P=0.023).  Numbers of CD3+ and CD4+ T cells and activity of NK cells were positively correlated with the level of blood glucose(P<0.05). Conclusion    GDM could lower numbers of CD3+ and CD4+ T cells, activity of NK cells and the ratio of CD4+/CD8+, and causes damage to the neonatal immune function. Attention should be paid to perinatal health care and prevention of neonatal infection.

    Effect of Qindan capsule on vascular adventitial remodeling and the TGF-β1/Smad signaling pathway

    Objective    To observe the effects of Qindan Capsule (QD) on  the TGF-β1/Smad Signaling Pathway and its effect on blood pressure to investigate its mechanism to improve the remodeling of vascular adventitia. Methods    Spontaneously hypertensive rats(SHR) were divided into the model group(SHR group) , the QD high dosage group(SHR+QDH group), the QD low dosage group(SHR+QDL group), and the losartan group(SHR+Los group). At the same time, 16 Wistar-Kyoto(WKY) rats of the same age were the normal control(WKY group)  and the QD group(WKY +QDH group). All rats were administered with corresponding medicine or normal saline and their blood pressures were measured. The level of TGF-β1 and Smad7 in the thoracic aorta were detected by immune-histochemical staining. Ⅰ, III collagen were stained with picric-sirius red and observed under a polarizing microscope. The data were quantified by ImagePro Plus.  Results    Compared with the SHR group, the blood pressure of the SHR+QDH group, the SHR+QDL group and the SHR+Los group were reduced (P<0.05), with decreased expression of TGFβ1 (P<0.05), increased  Smad7 (P<0.05), and  decreased  expression of Ⅰ, Ⅲ collagen (P<0.05). The SHR+QDL group was better than the SHR+QDL group (P<0.05).  Conclusion    QD can not only reduce blood pressure in SHR, but also can interfere with the TGF-β1/Smad Signaling Pathway by inhibiting the expression of Ⅰ, Ⅲ collagen in the arterial adventitia. Therefore, it can improve and even reverse the remodeling of vascular adventitia.

    Effects of the estrogen receptor on stress-induced premature senescence of  vascular smooth muscle cells in vitro

    Objective    To explore the effects and mechanism of the estrogen receptor on stressinduced premature senescence of vascular smooth muscle cells (VSMCS). Methods     The VSMCS, cultured from female SD rats and followed by 23 vitro passages , were induced into senescence by exposure to 150μmol/L H2O2 in the presence or absence of different concentrations (10-10mol/L-10-8mol/L) of PPT/DPN. Expressions of senescence associated marker (DcR2) were detected by flow cytometry. Expressions or activities of the protooncogene Ras were detected by pull-down assay and Western blotting analysis.   Results     Flow cytometry analysis showed that in physiological concentrations, the estrogen receptor agonist significantly inhibited H2O2promoted highlevel expression of DcR2 of VSMCS in a dosedependent manner(P﹤0.05). Pulldown assay and Western blotting analysis revealed that administration of (10-10mol/L-10-8mol/L) of PPT/DPN obviously reduced the H2O2-induced activity of Ras(P﹤0.05).  Conclusion     The estrogen receptor exerts inhibitive effect on stress-induced senescence of VSMCS by suppressing activity of Ras. ERa and ERb may have the same effects on stress-induced premature senescence of vascular smooth muscle cells in vitro.

    Determination of the concentration of SIgA in BALF in rats with chronic  obstructive pulmonary disease and diabetes mellitus

    Objective     To determine the concentration and differences of SIgA in bronchoalveolar lavage fluid (BALF) in the rat model of chronic obstructive pulmonary disease(COPD), diabetes mellitus (DM)and chronic obstructive pulmonary disease plus diabetes mellitus(COPD+DM).  Methods    48 rats were divided into four groups: the normal group, the COPD group, the DM group, and the COPD+DM group. There were 12 rats in each group. Bred in the experimental environment for one week, the DM  model was established. One week later, the COPD model was established by cigarette inhalation and intratracheal LPS exposure. After the rats were sacrificed, pathological changes of the right lung were observed, and the left lung was lavaged to determine the concentration of SIgA in BALF by ELISA.  Results     ① Pathological changes of lung tissue: in the normal group, the structure of alveolus was normal and  no inflammatory cell infiltration was found. In the COPD group,  the bronchial epithelium was ablated, and the interstitialpulmonary and airway walls were infiltrated with inflammatory cells, and the ruptured and enlarged alveolus and bullaes were identified. In the DM group, there was inflammatory cell infiltration under the basement membrane and the lodging cilias of bronchial epithelium. In the COPD+DM group, the bronchial epithelium was  severely ablated, and the interstitial-pulmonary and airway walls were infiltrated with abundant inflammatory cells.The structure of the alveolus were destroyed and some formed into bullaes. ② Concentration of SIgA in BALF: Compared with the normal group, the concentration of SIgA in the COPD group decreased (P<0.01), but increased in the DM group and the COPD+DM group (P<0.01). Compared with the COPD group, the concentration of SIgA in the DM group and the COPD+DM group significantly increased (P<0.01). There was no difference in the concentration of SIgA between the DM group and the COPD+DM group (P>0.05).  Conclusion    SIgA plays a defensive role in pulmonary mucosal immunity. Compared with the normal group, the significant decrease of SIgA in the COPD group indicates that the pulmonary mucosal immunity declines in the COPD group. The increased concentration of SIgA in the DM and COPD+DM group may be attributed to the immune response of advanced glycosylation end products (AGEs) and the increased compensatory immunity to pathogenic microorganisms.

    Effect of the interlukin-17 on proliferation and cell cycle of the hepatic stellate cells

    Objective    To investigate the effect of interlukin-17(IL-17) on proliferation and the cell cycle of hepatic stellate cells(HSC). Methods    HSC were treated with IL-17(200, 400, 600, 800, or 1000μg/L)for 24, 48 and 72 hours. HSC in the blank control group were treated without IL-17. The effects of IL-17 on proliferation and the cell cycle were detected by MTT assay and flow cytometry, respectively. Results    The quantity of the cells was associated with the dose of IL-17 and the incubation time. IL-17(200μg/L) had no significant stimulation effect on HSC(P>0.05), while IL-17(800μg/L) for 72h had the most significant stimulation effect (P<0.01). IL-17(1000μg/L) could inhibit the proliferation of HSC(P<0.01). IL-17 could change the distribution of cells at different phases of the cell cycle, promoting the G1 phase cells entering into the S phase. IL-17(1000μg/L) could block cells at the G1 phase(P<0.01).  Conclusions    IL-17 could stimulate the proliferation of HSC, which is associated with the dose of IL-17 and the incubation time, and it could promote G0/G1 phase cells entering into the S phase.

    Effects of γ-aminobutyric acid and its antagonist on γδT cells

    Objective    To research effects of GABA and GABAA receptor antagonist (picrotoxin, PTX) on proliferation, surface receptor expression and cytotoxicity of human peripheral blood γδT cells, and possible mechanism of action. Methods    After γδT cells were treated with different concentrations of GABA and PTX, the proliferation, surface receptor expression and cytotoxicity of γδT cells were detected by MTT, flow cytometry and the lactate dehydrogenase method, respectively. Results    GABA could decrease proliferation of γδT cells(P<0.01) in a dose-dependent manner. Expression of NKG2D on γδT cells was down-regulated by GABA, while TCR-γδ was up-regulated. GABA could inhibit γδT cells from killing the human pancreatic cancer cell line SW1990. PTX had an antagonistic role in the actions of GABA on the proliferation and cytotoxicity of γδT cells, but for surface receptor expression, PTX had a slightly synergistic effect. Conclusion     GABA may affect γδT cells via multiple pathways, besides its a receptor. The cytotoxicity of γδT cells is mainly mediated by NKG2D, which may need expressions of both γδTCR and NKG2D.

    Protective effect of tricyclic antidepressant nortryptyline of cortical  neurons on rats with OGD-induced injury and its mechanism

    Objective    To investigate the protective effect and mechanism of nortryptyline on oxygen-glucose deprivation (OGD)-induced primary cortical neuronal death.  Methods    In vitro cultured primary cortical neurons were induced by OGD with nortryptyline in different concentrations(1.0,2.5,5.0,10.0μmol/L), or without nortryptyline pretreatment. Then the neuronal activity and survival rate were determined by MTT, immunocytochemical staining and trypan blue staining. Also, release amounts of cytochrome c, Smac/Diablo and apoptosis-inducing factor (AIF) were determined by Western blot. Results     Nortryptyline significantly improved the growth condition and survival rate of primary cortical neurons, and inhibited cytochrome c, Smac/Diablo and AIF release after OGD injury.  Conclusion    Nortryptyline plays a neuroprotective role in cortical neuronal death induced by OGD, via the inhibition of cytochrome c, Smac/Diablo and AIF release.

    Protective effects of ginsenoside Rb1 and Re on  SK-N-SH cells injured by Aβ25-35

    Objective    To investigate protective effects of ginsenoside Rb1 and Re against injury of SK-N-SH cells induced by Aβ25-35 and the possible mechanism. Methods        Aβ25-35 was added to the medium for cell culture to make a cell model of Alzheimer disease, and ginsenoside Rb1 or Re were used to treat the injured cells. Cell survival rates were determined by the MTT assay, intracellular ROS levels were determined by fluorescence probes, and expressions of phosphorylated tau and active glycogen synthase kinase 3β(GSK-3β) were determined by Western blot. Results     The survival ratio of SK-N-SH cells were significantly decreased after exposure to Aβ25-35, and treatment with ginsenoside Rb1 or Re significantly increased the survival ratio and decreased the cellular ROS level. Ginsenoside Rb1 and Re decreased expressions of phosphorylated tau and active GSK-3β, respectively. Conclusion     Ginsenoside Rb1 and Re may exert a neuroprotective effect on SHNSH neural cells induced by neurotoxic Aβ25-35.

    Protective effect of intrathecal administration of VEGF on nerve fibers and  neurons after spinal cord injury in rats

    Objective    To investigate the effect of intrathecal administration of vascular endothelial growth factor (VEGF) on repair of spinal cord injury(SCI), and explore neuroprotection of VEGF on neurons and nerve fibers. Methods    45 animals were randomly divided into three groups. VEGF was intrathecally administrated at a dose of 25ng twice a day for a week in the VEGF group after SCI. The Basso, Beattie and Bresnahan locomotor rating scale (BBB) was observed each week. 8 weeks later, histological changes were examined by hematoxyhn and eosin (HE), immunohistochemical staining and an electron microscope. Results     The BBB scale in the VEGF group was higher than that in the model group, especially at the last 4 weeks. Histology revealed that the morphology of fibers and neurons at the ventral of the spinal cord in the VEGF group was better than those in the model group. More vessels were observed in animals administrated with VEGF compared with the model group(P<0.001). The number of caspsase-3-positive cells in the VEGF group was smaller(P<0.001). Slightly damaged nerve fibers and neurons were found in the VEGF group, with fewer mitochondria with disrupted cristae and secondary lysosomes were fewer compared with the model group. Conclusions     Administration of VEGF improves part of the functions of nerve fibers and provides neuroprotection for damaged neurons and fibers.

    Expression of recombinant human β-defensin 2 in human bladder  epithelial cells and its antimicrobial activity

    Objective    To examine expression of humanβ-defensin 2 (hBD2) in human bladder epithelial cells (T24 cells), and to observe the antimicrobial activity of recombinant hBD2 in vitro.  Methods     By using the cationic Liposome-TransFastTM Transfection Reagent, the plasmid vector carrying the hBD2 gene (pCAGGhBD2) was transferred into T24 cells cultured in serum-free medium. Expression of hBD2 mRNA in T24 cells was determined by reverse transcriptionpolymerase chain reaction (RT-PCR). HBD2 peptide expression was detected by Western blot and concentration of recombinant hBD2 in supernatants was measured by ELISA. Antimicrobial activity of recombinant hBD2 was assessed by the colony-forming unit (CFU) assay. Results    hBD2 could be effectively expressed in T24 cells, and the concentration of recombinant hBD2 in supernatants was (36.5±3.2)ng/106 cells. Recombinant hBD2 in supernatants showed antimicrobial activity. Conclusion    The recombinant eukaryotic expressing vector carrying the hBD-2 gene could be effectively expressed in human bladder epithelial cells, and recombinant hBD2 in supernatants has an obvious antimicrobial effect on some bacterial lines.

    Effects of ketamine on cerebrovascular pathology and NADPH oxidase activation in cynomolgus monkeys

    Objective    To explore the effect of long-term administration of ketamine on cerebrovascular(communicating branches of the cerebral arterial circle) pathology and nicotinamide adenine dinucleotide phosphate(NADPH) oxidase activation in cynomolgus monkeys. Methods    Twelve male monkeys were randomly divided into the experimental group  and the control group, six in each group. Monkeys in the experimental group were injected with ketamine at 1mg/ kg, while the control group received normal saline. 6 months later, the monkeys were sacrificed. Cerebrovascular pathology was observed by HE staining. Reactive oxygen species (ROS) level was expressed as the decrement of H2O2 with the biochemical method.  NADPH oxidase activation was observed by immunohistochemistry and Western blot. Results    The experimental group showed a significant increase in NADPH oxidase activation and ROS level compared with the control group. Thrombus and inflammatory cells infiltration were observed in the cerebrovascular tissue of the experimental group. Conclusion    Long-term administration of ketamine up-regulate NADPH oxidase level  leading to cerebrovascular pathological changes.

    Effect of reduced expression of PARP-1 by RNA interference on cisplatin-resistance and proliferation in the ovarian cancer cell line

    Objective     To investigate the role of the PARP-1 gene in the mechanism of chemoresistance in ovarian cancer, and to study the reverse effect of silencing PARP-1 by RNA interference on cisplatin(CDDP)-resistance of C13* cells.  Methods     Expressions of PARP-1 mRNA and protein in C13* and OV2008 cells were detected by real-time fluorogenic quantitative PCR (RFQ-PCR) and Western blot, respectively. RFQ-PCR and Western blot were also used to validate whether the PARP-1 gene was silenced. The influence of siRNA on cellular sensitivity to CDDP and cell proliferation activity was detected by MTT assay. Results    The PARP-1 mRNA and protein levels in C13* cells were (2.12±0.97) and (1.89±0.23) fold of those in OV2008 cells (P<0.05). PARP1siRNA significantly down-regulated PARP-1 mRNA and protein levels (P<0.05). mRNA and protein expressions of PARP-1 were decreased by (71.23±5.41)% and (73.84±3.78)%, respectively(P<0.05). The inhibitory concentration 50%(IC50) of CDDP was decreased by (89.77±10.06)% (P<0.05), and proliferation activity of C13* cells was inhibited (P<0.05).  Conclusion      Expression of the PARP-1 protein in ovarian cancer cell lines is related to cellular sensitivity to CDDP. PARP1siRNA treatment can significantly down-regulate PARP-1 expression in transcriptional and translational levels, and effectively reverses resistance of C13* cells to CDDP.

    Effect of CPB2 gene silencing on TAFI expression and   metastasis in breast cancer cells

    Objective     To investigate effects of carboxypeptidase B2 (CPB2) gene silencing on expression of thrombin activated fibrinolysis inhibitor(TAFI) and growth and metastasis of the breast cancer cell line MDA-MB-231.  Methods    Cells were divided into five groups: the siRNA-1 group, the siRNA-2 group, the siRNA-3 group, the negative control group and the blank control group. Based on the sequence of CPB2, three siRNAs were designed and chemically synthesized, and then transfected into the breast cancer cell line MDA-MB-231 with lipofectamine 2000.  TAFI mRNA and protein levels were determined by reverse transcription-polymerase chain reaction (RT-PCR) and Western blot.  Cell invasive and metastasis abilities and growth inhibition were determined by Transwell assay and MTT assay.  ResultssiRNAs targeting CPB2 significantly down -regulated mRNA and protein levels of TAFI (P<0.05), and the inhibition ratio ranged from 55.4% to 42.6%.  The growth of the breast cancer cell line MDA-MB-231 was inhibited in siRNA groups (P<0.05), and invasive and metastasis abilities were also significantly inhibited compared with the negative control group (P<0.05).  Conclusion    CPB2-siRNA can significantly down-regulate expression of TAFI in transcriptional and translational levels, and inhibits cell growth, invasion and metastasis ability of the breast cancer cell line MDA-MB-231.

    Effects of huohuazengshi medicinalgranules on MCP-1 and TGF-β2 in  experimental proliferative vitreoretinopathy

    Objective     To study the curative and preventive effects and the mechanism of huohuazengshi(HHZS) medicinalgranules on proliferative vitreoretinopathy(PVR) through establishing an experimental animal model of PVR. Methods     45 rabbits (45 eyes) were randomly divided into the model group, the joletion tablet group and the HHZS medicinalgranules group. Experimental PVR models were induced by injecting macrophages into the vitreous. Contents of MCP-1 and TGF-β2 in the vitreous humour were determined by ELISA. Then the rabbits were executed at days 3, 7, 14, 21 and 28. Results     Compared with the model group and the joletion tablet group, the content of MCP-1 in the HHZS medicinalgranules group was significantly decreased at day 7(P<0.01), while the content of TGF-β2 was increased at day 3 and reached the peak at day 14(P<0.01).  Conclusion     HHZS medicinalgranules could inhibit the formation and development of PVR by decreasing the content of MCP-1 and increasing the content of TGF-β2 in the vitreous at the inflammatory phase and early proliferative phase.

    Objective    To screen reliable molecular markers for epidermal stem cells(ESCs) in mice by using the double immunofluorescence labeling technique and labelretaining cells(LRCs). Methods     The epidermis was labeled for 7 days with 5-bromodeoxyuridine(BrdU) in postnatal day 5(P5) mice, followed by a trace period of up to 12 weeks. The tissue of the mice was frozen and sliced in a frozen microtome after 90 days. Slices were stained with polyclonal antibodies of anti-β1 integrin, P63 and CD34 and monoclonal antibodies of anti-Brdu, and then observed under a fluoroscope. Results    After 90 days, few LRCs were found in a region of the outer root sheath adjacent to the insertion of the arrector pili muscle, known as the bulge region. The percentage of double immunofluorescence cytochemically stained β1 integrin-and BrdU-positive cells was 81% in all β1-positive cells, and 94% in all BdrU-positive cells. The percentage of P63-and BrdU-positive cells was 65% in all P63-positive cells, and 86% in all BdrU-positive cells. The percentage of CD34- and BrdU-positive cells was 30% in all CD34-positive cells, and 62% in all BdrU-positive cells. Conclusion    Among these molecular markers, β1 integrin, with highest accordance with BrdU, is the best molecular marker for ESCs, and P63 is the next best.

    Objective    To study the effect of SKI-II on arresting gastric cancer SGC7901 cell cycle, and explore the mechanisms. Methods    SGC7901 cells were treated with SKIII alone or in combination with cisplatin,  and the cell cycle was analyzed by flow cytometry.  Expressions of Sphk1 and P27 were detected by immunocytochemistry and Western-blot. Results     After being treated with SKI-II alone or in combination with cisplatin for 48h,  the ratio of cells arresting in the G0/G1 phase was increased, compared with the control group(P<0.05); the effect of combined treatment group was more significant(P<0.05). Expression of P27 in SGC7901 cells was increased, while expression of Sphk1 was decreased, compared with the control group(P<0.05). Pearson correlation analysis showed that expressions of Sphk1 and P27 were negatively related. Conclusion    SKI-II could arrest gastric cancer SGC7901 cell cycle and inhibit cell growth through increased expression of P27, which was regulated by inhibiting expression of Sphk1.

    Objective     To compare multidrug-resistant characteristics of two human colorectal carcinoma cell lines selected/induced by continuous stepwise exposure and pulse drug exposure.Methods     The human colorectal carcinoma cell line LOVO was exposed to continuous stepwise-increased concentration of 5-fluorouracil (5-Fu) and constant high concentration of 5Fu,respectively,and the corresponding new sublines were designated as L2 and L3.Drug-sensitivity was measured by MTT. The cell cycle and apoptosis of cells were determined by flow cytometry.Expression of multidrug resistance 1 (MDR1) mRNA was detected by realtime PCR, and the P-gp expression by Western blot. Results    After 8 months′ stepwise drug exposure, the subline L2 exhibited more resistant features, with a resistance index of 26.53 against 5-Fu.It could normally grow and propagate in the medium with 2.5μg/mL of 5Fu.After several exposures with a high concentration of drug(2.5μg/mL of 5-Fu) for 6 months, the subline L3 presented a resistance index of 13.78 and could normally grow and propagate in the medium with 2.5μg/mL of 5-Fu. MTT assay showed that the two sublines were resistant not only to 5-Fu but also to other chemotherapy agents, manifesting multidrug-resistant characteristics.Compared with L2, L3 was more sensitive to most drugs, the number of cells at the G1 phase was lower,the apoptosis rate greatly increased, and expressions of MDR1 mRNA and P-gp were obviously reduced.Conclusion    After exposure with the two different methods, human multidrug-resistant colorectal carcinoma cell lines were successfully established, and the new sublines L2 and L3 manifest different multidrug-resistant characteristics. The mechanism of multidrug resistance may be related to the inhibition of MDR1 mRNA and P-gp overexpressions.

    Suppression of c-myc expression by lentiviral vector-mediated- small interfering RNA in Jiyoye cells

    Objective    To explore the effect of lentiviral vector-mediated siRNA on c-myc gene expression in Jiyoye cells by using the RNAi technique in vitro. Methods     Three interference sequences c-myc-1, c-myc-2 and c-myc-3 and the negative control c-myc-neg that targeted human c-myc mRNA were designed and synthesized. After annealing, all the fragments were cloned into the pLVX vector,which were transfected into human leukemia Jiyoye cells by lentivirus and were cultured for 72 hours. The cells were divided into five groups: the blank control group(untransfected), the c-myc-neg group, the c-myc-1 group, the c-myc-2 group and the c-myc-3 group. After 72 hours, the transfection rate in each group was determined by flow cytometry. Expressions of the c-myc mRNA and c-Myc protein were detected by Real-time PCR and Western blot. Results    PLVX-c-myc-neg, PLVX-c-myc-1, PLVX-c-myc-2 and PLVX-c-myc-3 were constructed. C-myc mRNA and protein expression levels in the three groups respectively transfected with c-myc-1, c-myc-2 and c-myc-3 were significantly down-regulated -compared with the negative control group transfected with c-myc neg(P<0.05). The c-myc-3 group decreased most significantly compared with the c-myc-1 group and the c-myc-2 group (P<0.05). There was no significant difference between the untransfected group and the negative control group (P>0.05). Conclusion     The successfully constructed shRNA expression vector for the cmyc gene suppresses expression of cmyc in Jiyoye cells, which might provide an experimental basis for further study of the role of cmyc gene silencing in targeting treatment of leukemia and lymphoma.

    Relationship of expressions of PDGFR-β and podoplanin with cancer-associated fibroblasts and  lymphangiogenesis in NSCLC

    Objective    To evaluate the relationship of expressions of plateletderived growth factor receptor(PDGFRβ) and podoplanin with cancerassociated fibroblasts(CAFs) and lymphangiogenesis in nonsmall lung cancer(NSCLC). Methods     Applying the immunohistochemical Elivision method, CAFs were labeled by αsmooth muscle actin(αSMA), and lymphatic vessels and expressions of PDGFR-β and podoplanin were detected in 80 cases of NSCLC and 20 adjacent normal tissues. The lymphatic microvessel density (LMVD) was counted by D2-40 as the specific marker of lymphatic endothelium. Relationships of expressions of PDGFR-β and podoplanin with CAFs, LMVD and clinicopathological features of NSCLC were then analyzed. Results    Expressions of PDGFR-β and podoplanin in the interstitium of NSCLC were both higher than those in adjacent normal tissues [PDGFR-β:  46.35%(39/80) vs 20%(4/20);podoplanin: 55.00%(44/80) vs 10%(2/20), both P<0.05]. Expressions of PDGF-β and podoplanin were closely related to CAFs. Expression of PDGFR-β in the interstitium of NSCLC was significantly correlated with that of podoplanin(r=0.380, P<0.05). LMVD in NSCLC was significantly higher than that in adjacent normal tissues (U=93.00, P<0.05). LMVD was significantly higher in NSCLC with lymph node metastasis and PDGFR-β-or podoplanin-positive expression than that  with no lymph node metastasis and negative expression[lymph node metastasis:(26.06+16.41) vs (17.26+12.97), t=2.582,P<0.05;PDGFR-β:(27.63+15.46) vs (17.47+14.68), t=3.01,P<0.05; podoplanin:(27.70+17.68) vs (15.96+9.84), t=3.74,P<0.05]. Expression of PDGFR-β in the interstitium of NSCLC was correlated with differentiation (P<0.05). Expression of podoplanin in the interstitium of NSCLC was correlated with differentiation and lymph node metastasis(both P<0.05). ConclusionIn     NSCLC, expressions of PDGFR-β and podoplanin are closely related to CAFs, and can promote lymphangiogenesis.

    Objective    To study the role of receiver operating characteristic(ROC) analysis in comprehensive diagnosis to differentiate malignant from benign solitary pulmonary nodules (SPNs) with multi-slice spiral computed tomography(MSCT).  Methods    177 SPNs were examined with MSCT and pathologically confirmed by surgery and/or needle biopsy. All MSCT images were assessed and graded by three senior radiologists on the basis of malignant likelihood of morphological and density characteristics. According to the maximum diameter (D), ROC curves were constructed and optimal thresholds were determined for diagnosis of malignancy in SPNS of D≤3.0cm, 2.0cm<D≤3.0cm and D≤2.0cm. ROC analysis was used to evaluate the efficiency of MSCT in differentiating malignant from benign SPNs. The area under the ROC curve (Az) was used as an index for diagnostic efficiency with MSCT. Az values were statistically analyzed by Z-test. Results    Az values of ROC curves were 0.879±0.030 in SPNs of D≤3.0cm, 0.899±0.039 in SPNs of 2.0cm<D≤3.0cm and 0.867±0.044 in SPNs of D≤2.0cm. The differences were statistically significant according to the Ztest (P<0.05). The three optimal thresholds for diagnosis of malignancy were 3.0, 3.5 and 2.5. Conclusion     Application of ROC analysis is of obvious value in the differential diagnosis of SPNs with comprehensive imaging characteristics obtained by MSCT. It is favorable to combine the diagnosis of SPNs by MSCT with the malignant probability and to improve the treatment of SPNs in clinical practice.

    Objective     To investigate the etiology, clinical features, diagnosis and treatment of cochlear fistula due to noncholesteatomatous chronic otitis media. Method    A retrospective investigation of a case of cochlear fistula due to noncholesteatomatous chronic otitis media and review of two cases (3 ears) previously reported in literatures were conducted. Results     There was no obvious decrease of bone conduction in these cases with cochlear fistula due to noncholesteatomatous chronic otitis media. No vertigo was complained by these cases and the results of fistula test were all negative. The endosteums in fistula were found to be intact during surgery. The bone conductions were slightly decreased after surgery. Conclusions     Mechanical pressure might not be essential to bone resorption, complete images of HRCT should be used to investigate the bone lesion in chronic otitis media to avoid missed diagnosis of cochlear fistula, complete removal of the lesion above the endosteum in cochlear fistula is necessary, and cochlear function is quite possible to be preserved by careful operation.

    Value of the dual-source CT virtual endoscopy in the assessment of thoracic aortic dissection

    Objective    To explore the clinical value of the dual-source CT virtual endoscopy (CTVE) by the ECG gating technique in the assessment of thoracic aortic dissection (AD). Methods    Data from 50 patients diagnosed as having thoracic aortic dissection aneurysms using dualsource CT were retrospectively reviewed. CTVE images were reconstructed at a workstation. Imaging features including the scope and configuration of the AD tear and intimal flap were reviewed according to CTVE. Evaluated results of CTVE images were divided into the clear (n=40)and unclear (n=10)tear groups in order to identify the association between concentration of contrast media and display of the tear. The difference was analyzed in the CT value of the true lumen near the tear between the two groups. Results     There were 22 patients with Stanford type A AD and 28 with type B among a total of 50 patients,  and CTVE revealed 40 cases in the clear tear group (80%), and 10 in the unclear group(20%) including 4 without display and 6 with poor display of the tear. No difference was shown in the mean CT value of the true lumen near the tear between the two groups (P>0.05). However, the marked artifact and noise were identified in the area of the tear in the lumen. 16 fusiform or oval tears and 24 irregular ones in CTVE images were shown in the clear tear group. In CTVE images, the torn intima could be clearly shown in 45 cases (90%), spiral ones in 35 and the others in 10. Conclusion    Dual-source CTVE an display the three-dimensional structure of the tear and intima of thoracic AD. Poor display of the tear is not associated with the concentration of contrast media.

    Influence of local mild hypothermia therapy on serum contents ofneuron-specific enolase and  S-100 protein of patients with acute cerebral infarction

    Objective    To observe clinical efficacy of local mild hypothermia therapy and its influence on serum contents of neuron-specific enolase (NSE) and S-100 protein in patients with acute cerebral infarction. Methods     66 patients were randomly divided into the treatment group (33 cases) and the control group (33 cases). Drugs were used in both groups, and local mild hypothermia therapy only was applied to the treatment group. Nerve function deficiency was scored by the National Institutes of Health Stroke Scale(NIHSS) before and after 7d, 14d and 30d of treatment. Serum contents of NSE and S-100protein were observed 1d, 3d, 7d and 14d after stroke. Results    Nerve function deficiency scores after 14d and 30d of treatment were lower in the treatment group than those in the control group (P<0.01). Serum contents of NSE 3d, 7d and 14d after stroke(P<0.01) and S-100 protein 7d and 14d (P<0.05) after stroke were also significantly lower. Conclusion    Local mild hypothermia therapy can protect neural cells and improve the nerve function in patients with acute cerebral infarction.

    Objective    To study expressions of p53 and Nup88 and their correlation in patients with colorectal carcinoma(CRC). Methods     Expressions of p53 and Nup88 mRNA were determined by reverse transcriptive-polymerase chain reaction (RTPCR) in HCT-116p53+/+ and HCT-116p53-/- cell lines.  Protein expressions of p53 and Nup88 were detected in 48 CRC samples and 13 normal colon samples by immunohistochemistry. Results    In the HCT-116p53-/- cell line, expression of p53 mRNA was more significantly decreased than that in HCT-116p53+/+, while there was no significant difference in expression of Nup88 mRNA between the two cell lines. Positive expressions of p53 (68.75%) and Nup88 (72.92%) proteins in CRC were notably higher than those in the normal tissues (P<0.01). However, there was no correlation between p53 and Nup88 expressions (r=0.173, P>0.05). Conclusion     Over-expressions of p53 and Nup88 play a role in the development of CRC, while there is no significant correlation between them.

    Values of Golgi protein 73 and Alpha-fetoprotein-L3 in the  diagnosis of primary hepatic carcinoma

    Objective     To explore significances of Golgi protein 73(GP73) and Alpha-fetoprotein variant(AFP-L3) in the diagnosis of hepatitis B virus-related primary hepatic carcinoma(PHC).  Methods      Enzymelinked immunosorbent assay(ELISA) was used to detect the serum level of GP73 in 56 cases of PHC, 33 cases of liver cirrhosis, 23 cases of chronic hepatitis B and 20 healthy controls. AFP-L3 was isolated by using affinity microcentrifugalcolumn; AFP and AFP-L3 were detected with chemiluminescent immunoassay and then the proportion of AFP-L3 was calculated. Results     Serum GP73 levels in the PHC, liver cirrhosis, chronic hepatitis and healthy groups were 247.05(228.35~287.72), 176.83(159.51~183.98), 152.58(135.19~162.85) and 43.00(37.96~52.64)ng/mL, respectively, and AFP-L3% was 14.52(12.33~15.28)%, 5.68(4.32~7.74)%, 4.31(3.57~6.45)%  and 3.29(2.60~4.25)%,  respectively. GP73 and AFPL3% levels were significantly higher in patients with PHC compared with those in other groups(P<0.01). Areas under the ROC curves of GP73 and AFPL3% were 0.825 and 0.828, respectively, and optimal cut-off values were 187.60ng/mL and 9.98% when diagnosing PHC, with sensitivities of 69.6% and 64.3%and specificities of 86.8% and 96.1%. Combined detection of the two parameters could improve the sensitivity up to 82.1% and maintain a high specificity of 85.5%. Conclusion    Both serum GP73 and AFP-L3% could be used as biomarkers in the diagnosis of PHC, and combined detection of them could improve diagnostic accuracy, with a higher sensitivity.

    Objective     To investigate expressions ,  significance and interaction of hTERT and P53 in colorectal carcinomas. Method     Expressions of hTERTand P53 were detected in 52 cases of tumor tissues and tumor-adjacent norma1 tissues by the immunohistochemical method of SABC. Results     The positive rate of hTERT in colorectal cancer was 80.8%(40/52) and in cancer adjacent normal tissues was 0%; The positive rate of P53 in colorectal cancer was 73.1%(38/52) and in cancer adjacent normal tissues was 0%. Over-expression of hTERT and P53 had no relation with gender,  age and tumor depth(P>0.05). but was significantly related to tumor differentiation,  lymph node metastasis,  and Dukes staging(P<0.05). hTERT and P53 expression was positively correlated(r=0.538, P<0.05). Conclusion     hTERT and P53 participate in common pathways in tumor development,progression and metastasis,and may play an important role in colorectal carcinomas.

    Objective    To evaluate the feature of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) at 3.0T for rectal diseases and to determine it′s value. Methods     DCE-MRI was performed on 51 patients with rectal diseases at 3.0T using 3D F-SPGR sequence. The contrast material was injected at the rate of 3mL/s with a dose of 0.1mmol/kg. DCE-MRI with 15-20 phases was consecutively performed once the injection of contrast media started. The timeintensity curve (TIC) was obtained and signal intensity of peak enhancement (SIpeak) and time to peak enhancement (Tpeak) of different rectal diseases and various degree of differentiation and TNM staging of rectal carcinoma were measured and compared. Results     Four types were classified for TICs according to the time to enhancement peak and washing out of the contrast material. All of adenomas presented as type A, while local recurrence of rectal carcinoma and abscesses presented as B, C or D type. The TICs of rectal carcinoma were various and changed gradually from type A to type D with the worsening of differentiation. There were significant differences among the Tpeak in different diseases. The order of the  Tpeak from early phase to late phase was adenoma, cancer, abscess and recurrence. The Tpeak became longer with the worsening of the differentiation of rectal carcinoma and depth of rectal carcinoma invasion. The Tpeak of rectal carcinoma without serosa invasion and lymph nodes metastasis was earlier than rectal carcinoma with such invasion and metastasis(P<0.05). Conclusion      TIC is helpful in the diagnosis of rectal diseases. The Tpeak is a reliable index for the diagnosis, differential diagnosis and prognosis prediction of rectal carcinoma.

    Objective     To investigate expressions of hepatoma-derived growth factor(HDGF) and micro-vascular density(MVD) and their correlations with clinicopathological characteristics and prognosis in patients with extrahepatic cholangio-carcinoma. Methods    Expressions of HDGF and MVD were analyzed in 65 patients after surgery for extra-hepatic cholangio-carcinoma by immunohistochemistry using the avidin-biotin peroxidase complex method,  and then their correlations with clinicopathological characteristics and survival time of patients were analyzed.  ResultsImmunohistochemical results showed positive expression rates of HDGF and MVD were 46.2% and 58.5%, respectively. Spearman rank correlation analysis demonstrated HDGF and MVD expressions had a positive correlation(r=0.279,P=0.024).   Survival analysis showed that the HDGF-positive group had a significantly poorer survival rate than the HDGF-negative group did (23.3% vs 62.9%, P<0.001). Multivariate analysis demonstrated that HDGF expression is an independent prognostic factor.  Conclusions    HDGF expression significantly correlates with MVD and is an independent prognostic factor for human extra-hepatic cholangio-carcinoma. HDGF plays an important role in promoting angiogenesis during the development of extrahepatic cholangio-carcinoma.

    Objective     To study mutation patterns of telbivudine(LdT)-resistant in the HBV P gene and clinical features in patients with chronic hepatitis B(CHB). Methods     15 patients with hepatitis B e antigen (HBeAg)-positive CHB that had a virological breakthrough and LdT-resistent mutation confirmed by the pyro-sequencing method were retrospectively analyzed. Results     5 mutation patterns at 4 sites of LdT-resistant were detected, rtM204I in 9 cases, rtM204I+rtL180M in 3 cases, and rtM204I+rtV173L, rtM204I+rtV173L+rtL180M and rtM204I+rtV173L+rtA181V in each 1 case. Serum HBV-DNA levels of all the 15 patients were below 104 copies/mL after 24 weeks of LdT treatment, 60% (9/15) of which were below 103copies/mL. Resistant mutations were detected from 40 to 111 weeks and all patients failed to take on HBeAg seroconversion. Conclusions     RtM204I is the main mutation pattern of LdT-resistance. Resistant mutation should still be of concern for patients who fail to take on HBeAg sero-conversion even if serum HBV-DNA is below 103 copies/mL at week 24.

    Objective     To study changes and influencing factors of the tear film after phacoemulsification combined with intraocular lens implantation. Methods     Sixty patients (70 eyes) with age-related cataract who underwent phacoemulsification combined with intraocular lens implantation were selected, and the chief complaint of dry eye syndrome, Schirmer I test (SIT), tear breakup time(BUT), corneal fluorescein staining(FSC)and corneal sensitivity values were analyzed before and 1 day,1 week,2 weeks and 1 month after operation. Results     Compared with the data before operation, the complaint, SIT, BUT and FSC scores had statistically significant differences 1 day,1 week and 2 weeks after operation(P<0.05), while there was no statistically significant difference 1 month after operation(P>0.05) . Corneal sensitivity returned to the preoperative level at 1 month postoperatively. Conclusion     Phacoemulsification combined with intraocular lens implantation significantly affects the tear film function in the  short term, and some patients may have dry-eye syndrome after surgery. Tear film stability has a significantly positive relationship with corneal sensitivity.

    Objective     To study expression of lysyl oxidase(LOX) in human breast cancer and explore its relationship with clinical prognostic features(patient′s age, tumor size, lymph node metastasis, clinical stage, ER, PR and HER-2 state) and breast cancer invasion / metastasis. Methods    mRNA expressions of LOX and HIF-1α in tissues were detected in 56 cases of human breast cancer, 56 cases of normal breast and 20 cases of breast benign lesion by reverse transcriptase polymerase chain reaction (RTPCR), and protein expressions of LOX and HIF-1α by S-P immunohistochemistry. Results     Expression rates of LOX mRNA in breast cancer,normal breast tissues and breast benign lesion tissues were 57.14%, 32.14% and 25.00%, respectively, and statistical differences were significant among them (P<0.01). Expression rates of the LOX protein in breast cancer, normal breast tissues and breast benign lesion tissues were 48.21%, 26.78% and 20.00%, respectively, and statistical differences were significant among them (P<0.05). Expressions of LOX mRNA and protein were associated with the clinical stage and lymph node metastasis (P<0.05), and were significantly higher in stage Ⅲ and Ⅳ breast cancer than those in stageⅠ and Ⅱ(P<0.01), while they had no correlation with age and tumor size(P>0.05). The LOX protein had significant positive correlation with HIF-1α protein expression in breast cancer(r=0.368). Conclusion     LOX protein and mRNA expressions are higher in breastcancer compared with normal breast tissues and breast benign lesion tissues, and LOX is probably relevant  to occurrence of breast cancer. Higher expression of LOX can increase the malignant degree of human breast cancer cells and facilitates metastasis. LOX and HIF-1α have a synergistic effect in breast cancer invasion and metastasis.

    Objective     To investigate expressions of cysteinyl aspartate specific protease-3(Caspase-3), osteopontin(OPN) and Ezrin in invasive ductal carcinoma(IDC) of breasts and their relationship with clinicopathological characteristics. Methods     Expressions of Caspase-3, OPN and Ezrin were detected in 71 IDC tissues and 33 normal mammary specimens with the immunohistochemical streptavidin-perosidase (SP) method, and their relationship with clinical pathological characteristics was analyzed. Results     Positive rates of Caspase-3, OPN and Ezrin in IDC tissues(43.66%, 74.65% and 64.79%, respectively) were significantly different from those in normal mammary tissues(75.76%, 33.33% and 39.39%, respectively) (P<0.05). Also, expressions of Caspase-3, OPN and Ezrin in IDC tissues had a significant relationship with lymph node metastasis and recurrence of the tumor (P<0.05), while they had no significant relationship with expressions of ER, PR and Her2. Expression of Caspase-3 was negatively related to those of OPN and Ezrin in IDC (r=-0.258, -0.313), while expression of OPN was positively related to that of Ezrin (r=0.29).  Conclusion    Abnormal expressions of Caspase-3, OPN and Ezrin play important roles in the occurrence and development of IDC of breasts.

    Subjective burden of family caregivers for hospitalized cancer patients and related factors

    Objective    To explore the state of subjective burden among family caregivers for cancer patients and its associations with the caregiver′s family relationship and other sensitive factors. Methods     71 caregivers for hospitalized cancer patients were assessed by the general sociological survey, Caregiver Reaction Assessment(CRA), Index of Family Relations (IFR)and Family Assessment Device (FAD), and 45 caregivers for general medicine as the control group were studied by CRA. Results    The burden in health (P<0.05), scheduling (P<0.01) and finances (P<0.001) of caregivers for cancer was higher than that in the control group. Higher quality of family relationship was related to lower burden in caregiver′s health (P<0.01), scheduling (P<0.05), finances (P<0.01) and family abandonment (P<0.05), while it was related to higher self-esteem (P<0.05). Affect intervention was related to lower burden in caregiver′s health (P<0.05) and family abandonment (P<0.05), while it was related to higher self-esteem. The age, marriage state, job and care time of caregivers were related to burden outcomes. Conclusions     Subjective burden of caregivers for cancer is higher than that of caregivers for general medicine. The burden is related to the caregiver′s family relationship quality, age, marriage state, job, care time and family income.

    Objective     To investigate expressions of trefoil factor 1 (TFF1) ,trefoil factor 2 (TFF2) and cyclooxygenase-2(COX-2) in the normal gastric mucosa, carcinoma-adjacent tissues and gastric carcinoma, and to explore their roles in pathogenesis of gastric carcinoma. Methods    Expressions of TFF1,TFF2 and COX-2 were determined by immunohistochemistry in 130 gastric specimens, including 30 specimens of normal gastric mucosa, 50 specimens of carcinoma-adjacent tissues and 50 specimens of gastric carcinomas. Results    In the normal gastric mucosa, carcinoma-adjacent tissues and gastric carcinoma, TFF1 and TFF2 expressions showed a decreasing tendency(P<0.01)), while the COX-2 expression showed an increasing tendency(P<0.01). Expressions of TFF1 and TFF2 were negatively related to that of COX2(P<0.01). Conclusion    The increase in expression of COX-2 inhibits expressions of TFF1and TFF2, and unbalance between proliferation and apoptosis of cells results in carcinogenesis. 

    Functional assessment of the foot with toe amputation by gait analysis

    Objective     To study the function of the foot with different types of toe injuries by the method of gait analysis. Methods    Forty post-operative patients  were followed-up for 2 to 8 years. They were divided into four groups: the partial distal hallux amputation group, the whole distal hallux amputation group, the second toe amputation group and the second caput ossis metatarsalis amputation group.  Plantar vito-dynamics were analyzed by the Footscan system and the normal foot of each patient was taken as the control. Results    Local pressure changes were observed in the partial distal hallux amputation group, the gravity center shifted back and the forefoot push-off-phase was shortened in the whole distal hallux amputation group, the plantar pressure shifted laterally in the second toe amputation group and the propodium′s pressure increased in the second caput ossis metatarsalis amputation group. Conclusion     Any kind of toe amputation may cause different influences to foot kinematics. Amputation of the distal hallux and second caput ossis metatarsalis may have significant impact on plantar vito-dynamics. It is worth trying to preserve the distal hallux and caput ossis metatarsalis as much as possible.

    Evaluation of effects of five screening methods for  asymptomatic type 2 diabetes

    Objective    To evaluate effects of five screening methods for asymptomatic type 2 diabetes, i.e. fasting capillary blood glucose (FCBG), fasting plasma glucose (FPG),  2hour plasma glucose (2HPG), glycosylated haemoglobin (HbA1c) and Diabetes Risk Score (DRS) by establishing receiver operating characteristic (ROC) curve models.  Methods    Data from a cross-sectional survey of 4070 subjects who underwent the oral glucose tolerance tests (OGTT) for type 2 diabetes screening in the Qingdao Diabetes Prevention Program in 2006 were analyzed. All the subjects had no history of diabetes and were required to test FCBG, FPG, 2HPG, HbA1c and finished the DRS designed for the program at the same time. The sensitivity, specificity, and the area under the ROC curve (AUROC) of the five methods for detecting asymptomatic diabetes were calculated to evaluate their effects.  Results    The sensitivity, specificity and  AUROC of the five methods were as follows:① FCBG: 64.08%, 69.81% and 0.712; ② FPG:60.10%,100.00% and 0.896; ③ 2HPG: 67.12%,100.00% and 0.898; ④ HbA1c: 40.21%, 81.96% and 0.673; and  ⑤ DRS: 61.50%, 63.72 and 0.679.  FPG and 2HPG had significant differences in AUROC from the other three screening methods (P<0.001).  The difference of AUROC between FPG and 2HPG and the differences of AUROC between FCBG, HbA1c and DRS were not significant (P>0.05).  Conclusion    FCBG and DRS may be simple and effective methods in primary detection for symptomless diabetics. HbA1c is not suitable for screening T2DM at present in the community until conditions permit. If the diagnosis thresholds of FPG and 2HPG are properly lowered, their diagnostic sensitivity will be enhanced and the rate of missed diagnosis reduced.
