    Lung cancer is a kind of malignant tumor with high morbidity and mortality in China. In recent years, with the improvement of medical level and the development of science and technology, interventional therapy plays an important role in lung cancer trerapy. More and more studies have shown that implantation of 125I radioactive seeds for treatment of patients with advanced lung cancer has achieved good results. But for the lung cancer at early stage, the effect of implantation of 125I radioactive seeds needs to be further studied.

    Benign central airway stenosis is a central airway stenosis caused by benign lesions, and it is difficult to deal with. The main reason is that patients and their families are with high expectations on the treatment effect and can not accept the serious complications caused by the interventional therapy. Therefore, the treatment of benign central airway stenosis is a difficult field of interventional bronchoscopy. In this paper, we present the causes, types, diagnostic methods, treatment methods and principles of interventional treatment of benign central airway stenosis in detail, hoping it is helpful for the diagnosis and treatment of benign central airway stenosis.

    Bronchoscopy is widely used in clinical work and plays a more and more important role in urgent tracheal intubation and airway management in patients undergoing mechanical ventilation. In particular, it has a unique role in artificial airway establishment, confirmation and adjustment of tracheal intubation, and etiological diagnosis of lower respiratory tract infection. However, problems such as standardized operation, prevention and control of infection related to bronchoscopy should be stressed and improved.

    With the rapid development of rigid bronchoscopy and interventional pulmonology in China, Chinese pulmonologists who are engaged in the practice of interventional pulmonology now have the conditions of tracheal T tube placement by rigid bronchoscope technique for the treatment of proximal tracheal stenosis. The tracheal T tube placement by rigid bronchoscope technique has a certain minimally invasive advantage compared to that by traditional surgery technique. However, all the clinical problems usually can not be solved by a single technique due to the complex situations of laryngotracheal stenosis. Better clinical results and low complications for laryngotracheal stenosis are related to the understanding of the difference between two techniques, developing multidisciplinary treatment approaches and strictly controlling of the indications of tracheal T tube placement.

    Transbronchial needle aspiration(TBNA)is a common technique, being widely used in the diagnosis and stage of lung cancer for many years. Though it is appealed by clinicians, however, there are still some misunderstanding aspects in practice which affects its utilization. In order to deepen the understanding of TBNA, we briefly illustrate several common misunderstanding aspects about the application of this technique.

    Objective To evaluate the diagnostic value and safety of endobronchial ultrasound transbronchial lung biopsy with guide-sheath(EBUS-GS-TBLB)in peripheral pulmonary lesions(PPL), and to investigate the related factors of improving EBUS-GS-TBLB diagnostic rate. Methods A total of 67 patients with suspected PPL who underwent transbronchial lung biopsy in the Department of Respiratory Medicine of the Second Hospital of Shandong University between January 2015 and March 2016 were prospectively evaluated. The patients were divided into the conventional group(n=38)and the endobronchial ultrasound group with guide sheath group(EBUS-GS group, n=29). The diagnostic rate and the related factors were analyzed. Results There was no significant difference in gender, age and lesion size of the two groups(P>0.05). The diagnostic rate of the EBUS-GS group was 75.9%(22/29), which was higher than that of the conventional group, 47.4%(18/38),(χ2=5.55, P=0.018). In EBUS-GS group, the diagnostic rate of the lesion diameter >2 cm was 94.1%(16/17), while the lesion diameter ≤2 cm was 50.0%(6/12). The difference was statistically significant(P=0.011). The diagnostic rate of EBUS probe within the lesions was 89.4%(17/19), while probe outside the lesions was 50.0%(5/10). The difference was statistically significant(P=0.030). Conclusion The procedure of EBUS-GS-TBLB is minimally invasive, has higher diagnostic rate and fewer complications, so it is a safe and effective method to diagnose PPL. The diagnostic rate of EBUS-GS-TBLB is related to both the lesion size and ultrasonic probe position.

    Objective To investigate the effect of miR- 498 on A549 cells epithelial-to-mesenchymal transition(EMT)and its correlated mechanism. Methods After miR- 498 was transfected into A549 cells, the cells were divided into control group, empty vector group and miR- 498 group; Transwell assay was employed to test the migration ability of A549 cells, Western blotting was used to investigate the expressions of E-cadherin, fibronectin, vimentin and FOXM1 in A549 cells. Luciferase assay was used to confirmed whether FOXM1-3'-UTR was the target gene of miR- 498. Then, the cells were divided into miR- 498 group, miR- 498 + FOXM1 group, and miR- 498 + empty plasmid group, and the above biological indicators of A549 cells were detected again. Results Compared with the empty vector group, the expression of E-cadherin was increased(P=0.001), the expression of fibronectin, vimentin and FOXM1were decreased(all P<0.01), and the migration ablitiy of A549 cells was decreased in the miR- 498 group(P=0.001); the Luciferase activity of the FOXM1-3'-UTR plasmid was significantly suppressed by miR- 498(P=0.001); over-expression of FOXM1 could reverse the effect of miR- 498 on A549. Conclusion miR- 498 inhibits A549 cells EMT by down-regulating FOXM1 expression.

    Objective To investigate the effects of corticotropin releasing factor(CRF)on the intestinal goblet cell secretory productions. Methods LS174T cells were treated with different concentrations of CRF to determine the optimum concentration. After that, LS174T cells were treated with 10-10mol/L CRF. Real-time qPCR, Western blotting, immunofluorescent staining and ELISA technique were used to examine the expression levels of intestinal goblet cell secretory productions. Results Real-time qPCR results showed that CRF decreased the mRNA expressions of mucin2(MUC2), fucosyltransferase-2(FUT2)and trefoil factor-3(TFF3)in LS174T cells at 6 h(P<0.05), compared with the PBS control group. Immunofluorescence and ELISA results showed that CRF reduced the protein expression of MUC2(P<0.05)at 6 h. Western blotting results showed that FUT2 and TFF3 protein levels were decreased(P<0.05)in LS174T cells treated with CRF for 24 h. Conclusion CRF could damage the intestinal mucosal barrier by altering the expression of the LS174T cell secretory productions.
    Preparation and characteristics of novel nanoscaled ultrasound contrast agents

    Objective To prepare lipid nanoscaled ultrasound contrast agents(UCAs), and to explore their physiochemical properties. Methods Orthogonal test was used to screen out the optimal condition for preparation aiming at three mainly factors. Amalgamator was applied to fabricate the lipid nanoscaled UCAs. The morphology, diameter, polydispersity index(P.I), zeta potential and stability of nanoscaled UCAs were assessed. The enhancement effect of nanoscaled UCAs in vitro was also detected. Results The results of orthogonal test indicated that the oscillation time had a significant effect on the diameter of nanoscaled UCAs(P<0.05). The optimal condition of preparation was as follows: the amount of DPPA was 0.5 mL, oscillation time was 90 s, and centrifugation time was 3 min. The mean diameter and P.I of the nanoscaled UCAs fabricated under the optimal condition was(313.20±6.90)nm and 0.15, respectively. The Zeta potential was -12.40 mV. There was no significant difference in the enhancement effect between the nanoscaled UCAs and SonoVue in vitro(t=2.02, P>0.05). The basic physical characteristics of the nanoscaled UCAs remained unchanged after being stored in 4 ℃ refrigerator for 20 hours. Conclusion Nanoscaled UCAs with smaller diameter and good stability were successfully fabricated. All these characteristics indicate a promising future in targeted molecular imaging and drug or gene delivery.
    Clinical value of PNI and HGB in evaluating the prognosis of the middle to late stage non-small cell lung cancer patients

    Objective To explore the clinical value of prognosis of nutrition index(PNI)and hemoglobin(HGB)in evaluating the prognosis of the middle to late stage non-small cell lung cancer patients. Methods A total of 120 cases of the middle to late stage non-small cell lung cancer patients were selected during August 2013 and August 2015. The clinical information and data of all patients were collected when they were first confirmed. All patients were followed-up by the date of death or August 2016 through electronic medical record system, outpatient and telephone, etc. The patients were divided into high and low PNI groups, and high and low HGB groups on the basis of the medians of PNI(46.52)and HGB(115 g/L). The impact of PNI and HGB on overall survival(OS)of patients was analyzed through single factor and multiple factors methods. The survival curves of two groups were produced by Kaplan-Meier method. The Log-rank test was exerted. The related influencing factors of PNI and HGB were analyzed through single factor method. Results The single factor analysis showed that the patients with age≥65 years, smoking index≥400, PNI<46.52, HGB<115 g/L when they were first confirmed had significantly lower OS than those with age<65 years, 山 东 大 学 学 报 (医 学 版)55卷4期 -孙启晶,等.PNI及HGB评估中晚期非小细胞肺癌患者预后的临床价值 \=-smoking index<400, PNI≥46.52, HGB≥115 g/L(P<0.05). PNI were closely related to the age, clinical stages, pleural effusion and smoking index(P<0.05). HGB were closely related to the age and weight loss rate(P<0.05). The multiple factors analysis showed that only PNI and HGB were statistically significant on OS of the patients[Exp(B)=2.180, P<0.001; Exp(B)=2.032, P=0.001]. The survival curves and Log-rank test showed that low PNI and HGB groups had lower OS than high PNI and HGB groups(P<0.001). Conclusion PNI and HGB can be used as the independent risk factors for the middle to late stage non-small cell lung cancer patients.
    Establishment and preliminary evaluation of a model for predicting the malignant probability of solitary pulmonary nodule with tumor markers

    Objective To establish a model for predicting the malignant probability of solitary pulmonary nodule(SPN)with the tumor markers and to evaluate its clinical value. Methods A retrospective cohort study in Yantai Yuhuangding Hospital of Shandong Province included 250 patients with definite pathological diagnosis of SPN from Jan. 2010 to Oct. 2015. Clinical data included age, gender, quantity of smoking history, symptoms, site, maximum diameter, the levels of CEA, CYFRA21-1 and NSE. By means of Logistic regression, 9 factors were analyzed to establish the model. Receiver operating characteristic curve(ROC)was performed and the area under the curve(AUC)was calculated. Our model was compared with the Mayo model to evaluate its clinical value. Results CEA level(P=0.002, OR=5.921, 95%CI=1.968-17.819), CYFRA21-1 level(P=0.046, OR=2.500, 95%CI=1.018-6.142), symptom 山 东 大 学 学 报 (医 学 版)55卷4期 -李笑莹,等.肿瘤标志物预测孤立性肺结节恶性概率模型的建立与初步评价 \=-(P=0.010, OR=2.384, 95%CI=1.234-4.607)and maximum diameter(P=0.001, OR=2.331, 95%CI=1.441-3.773)were associated with malignant SPN. Our prediction model: P=ex/(1+ex), X=-1.991+0.869×symptom+0.846×maximum diameter+1.779×CEA+0.916×CYFRA21-1. The goodness-of-fit of the model was fairly good. When the optimal cut-off point was 0.636, the sensitivity was 63.5% and specificity was 71.2%. The difference of AUC between our model and the Mayo clinic model was not statistically significant(P>0.05). Conclusion CEA, CYFRA21-1, symptom and maximum diameter are independent risk factors of malignant SPN. Our research shows that the Logistic regression model has high accuracy and clinical value.
    Role of hypoxia-inducible factor-1α in the regulation of programmed death ligand 1 expression in lung adenocarcinoma

    Objective To analyze the association of hypoxia-inducible factor-1α(HIF-1α)and programmed death ligand 1(PD-L1)expression and clinicopathologic features in lung adenocarcinoma, and to investigate the potential role of HIF-1α in the regulation of PD-L1 expression. Methods The expressions of HIF-1α and PD-L1 of 99 cases with lung adenocarcinoma were examined by immunohistochemical method. The lung adenocarcinoma cells were treated with hypoxia mimic CoCl2 and divided into normal group and CoCl2 group. The mRNA and protein expressions of HIF-1α and PD-L1 were detected with qRT-PCR and Western blotting, respectively. The lung adenocarcinoma cells were transfected with siRNA-HIF-1α and divided into negative control group and siR-HIF-1α group. The transfection efficiency of siRNAs and the expression of PD-L1 were detected by qRT-PCR and Western blotting, respectively. The expression of HIF-1α was correlated with age and clinical stage(P<0.05). The expression of PD-L1 was correlated with clinical stage(P<0.05). The expressions of HIF-1α and PD-L1 were positive correlated(P<0.01). Compared with the normal group, the protein expression of HIF-1α significantly increased(t=-2.394, P=0.029), and the mRNA and protein expressions of PD-L1 increased subsequently in the CoCl2 group(t=-3.146, P=0.006; t=-3.308, P=0.004). Compared with the negative control group, the mRNA and protein expressions of PD-L1 decreased dramatically in the siR-HIF-1α group(t=2.746, P=0.014; t=2.339, P=0.033). Conclusion The HIF-1α and PD-L1 proteins are highly co-expressed, and HIF-1α can up-regulate the expression of PD-L1 in lung adenocarcinoma.
    Tigecycline in treating ventilator-associated pneumonia in critically ill patients

    Objective To evaluate the clinical efficacy and safety of tigecycline in critically ill patients with ventilator-associated pneumonia(VAP). Methods A prospective study was conducted based on critically ill patients with VAP in Intensive Care Unit of Qilu Hospital of Shandong University during Jan 1, 2011 and May 31, 2013. All patients accepted intravenous infusion of tigecycline with a first dose of 100 mg and then 50 mg every 12 h. The course was directed by medical advice. The clinical and bacterial efficacy and safety were evaluated at the end of the therapy. Results A total of 37 patients were included at the beginning, and 33 patients were evaluable at the endpoint. The total clinical success rates was 60.6%. Thirty-six multidrug-resistance bacteria strains were detected in specimens obtained from all patients before the start of tigecycline treatment, and all these strains were sensitive to tigecycline. The total clearance rate was 61% while Acinetobacter baumannii predominated a 60% clearance rate. Adverse reactions were well tolerated with predominantly digestive symptoms, and all responded to symptomatic treatment afterwards. Six patients died within 28 days. Conclusion Tigecycline proved to be effective and safe for critically ill patients with VAP.
    Expression and role of Helios gene in regulatory T cells of pediatric precursor B-cell acute lymphoblastic leukemia

    Objective To detect the proportion of human CD4+CD25+FoxP3+ regulatory T cells(Treg)in patients with pre-B acute lymphoblastic leukemia(ALL)and to explore the role of transcriptional factor Helios in pre-B ALL Treg cells. Methods A total of 10 primary ALL cases(ALL group)and 6 cases of osteoarthropathy free from tumors, hematological diseases and immunological diseases(control group)were involved. Peripheral blood samples were collected from all cases. The proportion of Treg cells was tested with flow cytometry, and the expressions of Helios, FoxP3 and related transcription inhibitors in CD4+CD25+ Treg cells of pre-B ALL samples were tested with real-time quantitative polymerase chain reaction(qRT-PCR). The lentiviral vectors expressing Helios mRNA and Helios-siRNA were constructed to regulate Helios expression in vitro to assay the role of Helios. Results The percentage of Treg cells in the ALL group and control group was(6.16±0.79)% and(2.62±0.34)%, respectively, with significant statistical difference(P=0.005). The expressions of Helios, FoxP3 and in TGF-β1 mRNA were upregulated in Treg cells.     Objective To evaluate the in vitro activities of sitafloxacin, cefoperazone-sulbactam(1∶1)and polymyxin E used alone or in combination against extensively drug-resistant Acinetobacter baumannii (XDR-A. baumannii). Methods A total of 22 strains of XDR-A. baumannii were isolated from clinical patients. The minimum inhibitory concentration(MIC)values of sitafloxacin, cefoperazone-sulbactam(1∶1), and polymyxin E used alone against XDR-A. baumannii were determined by broth microdilution method, and the MIC of drugs used in combination were determined by the checkerboard method. According to the MICs results, the values of fractional inhibitory concentration(FIC)and FIC index(FICI)were calculated. Results The MIC of sitafloxacin, cefoperazone-sulbactam(1∶1), and polymyxin E used alone was 0.25-16, 4.0/4.0-128/128, and 0.5-8.0 μg/mL, respectively. When two drugs were used in combination, all of the MICs were decreased. Conclusion Most combinations of the three drugs were synergy, partial synergy or additive when compared to the drugs used alone.

    Objective To investigate the expressions of adiponectin, visfatin and resistin in systemic lupus erythematosus(SLE), and to explore their role in SLE, lupus nephritis, and SLE complicated with bone necrosis. Methods A total of 58 SLE patients including 12 SLE complicated with bone necrosis and 46 SLE without bone necrosis, 23 lupus nephritis(LN)cases, 35 SLE cases without LN, were recruited. Another 43 healthy volunteers were selected as healthy controls. The serum levels of adiponectin, visfatin and resistin were detected with ELISA to analyze their relationship with renal injury and bone necrosis. The correlation between clinical and laboratory parameters was analyzed. Results Serum levels of adiponectin, visfatin and resistin were higher in SLE patients than in healthy controls(P<0.001). Adiponectin and resistin levels were significantly higher in LN cases than in SLE without LN cases(P<0.001, P=0.033). Visfatin was clearly associated with erythrocyte sedimentation rate(ESR)(rs=0.281, P=0.046). Body mass index(BMI)and ESR were the significant factors of visfatin(P=0.010, P=0.020), and BMI was the significant factor influencing resistin(P=0.046). Conclusion The high expressions of serum adiponectin, visfatin and resistin may be associated with SLE, and increased adiponectin and resistin levels correlate with renal damage, but the underlying mechanisms need further studies.
    Sturge-Weber syndrome in patients with intractable epilepsy: a clinicopathologic study of 7 cases

    Objective To investigate the clinicopathologic features of the brain tissues from the patients with refractory epilepsy related Sturge-Weber syndrome(SWS). Methods The clinical and pathologic data were reviewed in 7 SWS patients who underwent epilepsy operation. Seven patients including 4 males and 3 females had been diagnosed SWS. The age of patients for surgery was 2-20 years old. The age of patients when seizure onset was 1-12 months. Six cases accepted multilobar resettion, and 1 case accepted functional hemispherectomy. Results The pathological diagnoses were meningioangiomatosis, remarkable calcification in the brains gray matter and small blood vessels, focal cortical dysplasia(FCD)Ⅲc. Seizure outcome after the epilepsy operation revealed 3 patients had an Engel grade I, 2 patients had an Engel grade Ⅱ, 1 patient had an Engel grade Ⅲ, and 1 patient had an Engel grade Ⅳ. Conclusion SWS patients had earlier onset, longer course and characteristic pathological change. Children with typical symptom should accept operation as soon as possible. Atypical patients also had good prognosis after operation.
    Work status of cancer survivors from 5 counties in Shandong Province and the influence factors

    Objective To investigate the work status among the survivors who have completed their main treatments for over one year, and to explore the factors which are associated with their work changes. Methods We randomly selected 1003 cancer survivors who were diagnosed with breast cancer, lung cancer, intestine cancer and gastric cancer between 2011 and 2013 from the local cancer registry files of five counties in Shandong Province. Results According to the selection criterion, 664 survivors were included. Of all the survivors, 24.7% of the survivors did not change their work time; 34.3% had to reduce their work time; and 41.0% stopped working. Multivariate Logistic regression results showed that, those above 65 year-old(P=0.022), with lower education levels(P<0.05), in advanced stage(P=0.016), and having an annual family income of above 50 000 RMB(P=0.005)were more likely to stop working. Conclusion About 40% of cancer survivors had to stop their work. The main influence factors included age, cancer stage, family income, and education level.

    Objective To investigate the sex characteristics and syphilis infection among amphetamine-type stimulants(ATS)users in Yantai City. Methods ATS users were recruited with convenient sampling method in Yantai City. They were interviewed to collect related information using questionnaires, and blood samples were collected to detect syphilis antibodies and HIV antibodies. Results Among the 411 ATS users, the prevalence rate of syphilis was 18.0%(74/411). After taking drugs, the main sex partner was regular partner, accounting for 51.6%(212/411), casual and commercial partners were 35.8%(147/411)and 12.7%(52/411)respectively. In the last year, 11.4%(47/411)of new drug users had sex with gays. In the latest month, the rate of condom use in the last sex was 56.0%(108/193). Univariate analysis showed that age, marital status, employment situation, monthly income, age of first drug use were influencing factors of syphilis infections. Multiple logistic regression model demonstrated that over 35 years of age(OR=2.943, 95%CI: 1.145-7.564, P<0.05), cohabitating(OR=4.172, 95%CI: 1.228-14.171, 山 东 大 学 学 报 (医 学 版)55卷4期 -董安琪,等.烟台市新型毒品吸食者性行为特征与梅毒感染情况 \=-P<0.05), self-employment(OR=2.845, 95%CI: 1.361-5.948, P<0.05), unemployment(OR=2.605, 95%CI: 1.134-5.98, P<0.05), monthly income of 5,000 RMB or more (OR=2.511, 95%CI: 1.081-5.834, P<0.05)increased the chances of syphilis infections. Conclusion The situation of sexual partners is complicated among ATS users, and the rate of condom use is low. The high syphilis prevalence and common homosexual behaviors among ATS users underscore the urgency to develop effective intervention measures.
