Evaluation of the effects of high blood pressure on white matter lesion in young adults using diffusion tensor imaging

Abstract: Objective To explore subtle white matter lesions in young hypertensive adults using magnetic resonance diffusion tensor imaging(DTI). Methods Routine brain MRI and DTI sequences were performed in participants with hypertension(n=45, hypertension group)and controls with normal blood pressure (n=45, control group). Fractional anisotropy(FA)values of centrum ovale, cerebral peduncle, corona radiata, genu of corpus callosum, splenium of corpus callosum, corpus callosum body, anterior limb of internal capsule, posterior limb of internal capsule, bilateral frontal lobe, parietal lobe, temporal lobe and occipital lobe were measured. Results Compared with the control group, the hypertension group had significantly decreased FA values in the cerebral peduncle and frontal lobe(P<0.001). FA values in the cerebral peduncle and frontal lobe were negatively correlated with systolic blood pressure(r=-0.625, r=-0.665, P<0.001; r=-0.417, r=-0.495, P<0.05). FA values of the other regions of the white matter were not correlated with either systolic pressure or diastolic pressure(P>0.05). Conclusion The microstructural white matter in young adults with hypertension may be selectively impaired, indicating the need for early and optimum control of blood pressure.
