    As a powerful selective β3 receptor agonist, mirabegron is an effective and well-tolerated new drug for the treatment of overactive bladder(OAB). Clinical studies have confirmed that mirabegron can significantly reduce the number of urinary incontinence and micturition frequency, increase the average voiding volume, and decrease the urgency 山 东 大 学 学 报 (医 学 版)56卷3期 -种铁,等. 米拉贝隆治疗膀胱过度活动症的临床应用 \=-feeling as well as the number of nocturia. Combined with anticholinergic drugs, mirabegron can improve curative effect without increasing the incidence of adverse drug reactions. Mirabegron provides a new treatment method for OAB with a good application prospect.

    Intravesical botulinumtoxin(BTX)injection has been proved to be a safe and effective treatment for intrac-table overactive bladder syndrome(OAB)that is refractory to antimuscarinics. This review is going to highlight the major progresses of this technique to provide a detailed picture for its clinical applications. The biological properties of BTX and its working mechanisms will be introduced first, and then, the properties of BTX products from different companies, indications and contraindications of this technique, the right injected dosage and appropriate sites of injection, effects and adverse effects will be stated and discussed. Meanwhile, the effects of BTX injection in specific groups of patients like children, diabetes mellitus(DM)patients and elderly people will be explored.

    Overactive bladder(OAB)is a symptom complex, which includes urinary urgency, frequency, and nocturia with or without incontinence. Peripheral nerve stimulation is one of the effective ways to cure patients who have failed to respond to conservative treatments as the third line therapy. This paper reviews the history, mechanism, clinical efficacy and related researches of peripheral nerve stimulation in the treatment of OAB.

    Sacral neuromodulation(SNM)is one of minimally invasive techniques for regulating downstream target organs, such as bladder, urethral sphincter and pelvic floor muscle. Pulsatile stimulating is carried out on sacral nerve through a manually implanted “pacemaker” in SNM. For patients, SNM technology has the advantages of safety and efficiency, little damage, good long-term tolerance, long effective duration, and has broad application prospects in the supplement and adjuvant therapy of refractory bladder dysfunction. This article introduces the principle and advantages of SNM, which focuses on the application in bladder dysfunction treatment, and analyzes the direction and the broad prospect of SNM.

    Objective To explore the inhibitory effect of capsaicin on the invasion and metastasis of human bladder cancer 253J and RT112 cells. Methods Human bladder cancer 253J and RT112 cells were treated with 0, 100, 200 μmol/L capsaicin for 0 h, 12 h and 24 h. The viability, migration and invasion of cells were measured with MTT assay, wound healing assay, and Transwell assay, respectively. The expressions of epithelial-mesenchymal transition(EMT)markers(E-cadherin, vimentin, and N-cadherin)and nuclear erythroid-related factor 2(Nrf2)proteins were analyzed with Western blotting. Results Compared with 0 μmol/L, in the 100 and 200 μmol/L capsaicin groups, the inhibition rate of cell viability increased(P<0.01), the index of invasion decreased(P<0.01), and migration decreased(253J: P<0.01; RT112: P=0.026 or P=0.014). The expressions of E-cadherin and C-nrf2 protein increased, while the expressions of vimentin and N-nrf2 protein decreased in a dose- and time-dependent manner. Conclusion Capsaicin can inhibit the invasion and metastasis of human bladder cancer cells possibly due to mediation of Nrf2 activity.

    Objective To explore the effect of androgen ablation and androgen receptor competitor biclutamide on autophagy and apoptosis in bladder cancer UM-UC-3 cells. Methods DHT and bicalutamide were respectively used to activate or inhibit androgen receptor signaling in UM-UC-3 cells. In addition, rapamycin and chloroquine were respectively used to induce or inhibit autophagy in UM-UC-3 cells. Hoechst staining, flow cytometry and Western blotting were used to observe the apoptotis. Autophagy flux was detected and quantified under transmission electronic microscope and confocal fluoresence microscope. Results Elevated autophagy and apoptosis levels were detected in UM-UC-3 cells after androgen ablation and androgen receptor competitor bicalutamide treatment. In addition, apoptosis induced by bicalutamide could be rescued by chloroquine and enhanced by rapamycin. Conclusion Androgen ablation and androgen receptor competitor bicalutamide promote apoptosis in UM-UC-3 cells due to autophagy induction.

    Objective To explore the anti-tumor effect of valproic acid(VPA)and metformin on human prostate cancer cells. Methods PC-3 cells were treated with VPA and metformin andmetformin+VPA, and were devided into control group, VPA group, Met group and VPA+MET group. The cell viability and invasion properties were detectedwith CCK-8 and Transwell assay, respectively. Related proteins including pAkt, pSmad3, aH3 and aH4 were detected with Western blotting. Results Both VPA and metformin could inhibit tumor cell migration and viability. Metformin+VPA had much stronger anticancer effect than metformin and VPA alone. Compared with control group and MET group, the VPA+MET group showed induced levels of Akt, pmTOR and pSmad3, but elevated levels of aH3 and aH4. Conclusion The metformin+VPA combination has advantage over metformin mono-treatment in vitro. The regulation of transforming growth factor-β(TGF-β)and mammalian target of rapamycin(mTOR)pathways and the responsiveness of aH3 and aH4 might be involved in this process.
    Berberines effects on the proliferation, apoptosis and DNA damage and repair of human renal cell carcinoma cells

    Objective To determine the effects of berberine(BBR)on the growth and DNA damage of human renal cell carcinoma(RCC)cells. Methods The effects of BBR on the proliferation of A498 and 786-O RCC cells at concentrations of 0-240 μmol/L were tested with CCK-8 testing kit. Based on the results of CCK-8 test, RCC cells were divided into control group(0 μmol/L), 30 μmol/L group and 60 μmol/L group. The cell apoptosis after BBR treatment was evaluated with Annexin V-FITC/PI Apoptosis Detection Kit. The expressions of proteins involved in the apoptotic pathway and DNA damage repair pathways, including pro-caspased3, cleaved-caspased3, γH2A.X and KU70, were assessed with Western blotting. Results The proliferation of RCC cells was significantly suppressed by BBR in a time- and dose-dependent manner(A498: P<0.001; 786-O: P=0.002). In comparison with the control group, the 30 and 60 μmol/L groups showed increased expressions of cleaved-caspase3(A498: P=0.018, P<0.001; 786-O: P=0.038, P<0.001), and γH2A.X(A498: P<0.001, P<0.001; 786-O: P<0.001, P<0.001), but decreased expression of KU70(A498: P=0.002, P<0.001; 786-O: P<0.001, P<0.001). Compared with the 30 μmol/L group, the 60 μmol/L group showed increased expressions of cleaved-caspase3(A498: P=0.020; 786-O: P=0.010), and γH2A.X(A498: P=0.002; 786-O: P<0.001), but decreased expression of KU70(A498: P<0.001; 786-O: P=0.005). Conclusion BBR can effectively suppress RCC cell proliferation, induce RCC cell apoptosis and DNA damage, and inhibit DNA damage and repair.

    Objective To explore the clinical and radiology characteristics and laboratory test results of medullary sponge kidney(MSK). Methods A total of 8 MSK patients, in whom 2 were two years old or so, 3 ranged from twenty to forty years old, and 3 ranged from forty to sixty-five years old, hospitalized in Qilu Hospital from Jan. 2012 to Dec. 2016, were enrolled. The clinical manifestations, laboratory test results, and radiology characteristics, including X-ray, intravenous pyelography(IVP)and computed tomograph(CT), were analyzed. Results Multiple cystic dilatation could be seen in collecting duct of children, but the radiology signs of the kidney stone were not as obvious as the adult. The patients ranged from 20 to 40 years old had less complications and thicker cortex renis; their pelvis were lightly dropsical. The patients ranged from 40 to 65 years old had thinner cortex renis and more serious hydronephrosis, companied with the increased creatinine level. The staghorn stone surface was smooth along the mucosa while the opposite side was rough. The gas density shadow could be seen in pelvis when it was infected with bacteria. CT and X-ray images showed high density shadows with circle and oval signs ranged in carunculae papillaris and inter medulla regions. IVP images showed the ecstatic collecting tubes and papillary ducts with the line and brush shadow. Stone grows along the mucosal surface with the parallel, sand-like, and eggshell calcification signs. Stone can overgrow into the pelvis and calyx, and integrate into clusters, which leads to the local hydrops. Laser lithotripsy is needed to improve the kidney function when the hydrops situation is serious, but the stone in papillary ducts is difficult to remove and easily recurrent after the surgery.
    Detrusor contractility in patients with benign prostate hyperplasia combined with acute urinary retention

    Objective To elucidate the urodynamic and clinical characteristics of benign prostate hyperplasia(BPH)patients complicated by acute urinary retention(AUR), specially focusing on their detrusor contraction function. Methods The urodynamic parameters and clinical characteristics of 264 BPH patients from Aug. 2014 to Mar. 2017 were analyzed retrospectively. The patients were divided into AUR group(n=99)and non-AUR group(n=165). Urodynamic study was performed 7-10 days after the urinary catherization for AUR patients. Age, prostate-specific antige(PSA)level, prostate volume(PV), and urodynamic parameters in the two groups were compared. Results Age and PSA level of AUR patients were significantly higher than those of non-AUR patients(P<0.05). Maximum flow rate, voided volume, bladder volume at first desire and strong desire, and bladder compliance were decreased in AUR patients than those in non-AUR patients(all P<0.05). The percentage of patients with Schaefer obstruction grade IV or higher was greater in AUR group than that in non-AUR group(P<0.05). In both groups, bladder contraction index(BCI)and maximum Watts factor(Wmax)were increased with the rising of obstruction grade; however, there was no significantly statistical difference for these two indexes. Multivariate Logistic regression analysis showed that obstruction 山 东 大 学 学 报 (医 学 版)56卷3期 -王文振,等. 良性前列腺增生并发急性尿潴留患者的逼尿肌收缩功能 \=-grades III, IV and VI were risk factors for predicting the development of AUR. Conclusion Obstruction severity rather than the impairment of detrusor contractility is closely related with the occurrence of AUR in BPH patients.
    Comparative study on biological characteristics of mesenchymal stem cells from human umbilical cord and adipose

    Objective To study the biological characteristics of mesenchymal stem cells(MSCs)derived from human umbilical cord and adipose in vitro. Methods Umbilical cord-derived mesenchymal stem cells(UC-MSCs)and adipose-derived mesenchymal stem cells(AD-MSCs)were isolated. Cell growth curve was detected using CCK-8 method, and cell surface antigens were measured using flow cytometry. The adipogenic and osteogenic differentiation assays were analyzed by Oil Red O, Alizarin Red S and Von Kossa staining. The immunomodulatory properties of UC-MSCs and AD-MSCs on lymphocytes were evaluated through the co-culture assay with PHA activated adult peripheral blood monocuclear cells(PBMCs). Results Cell growth curve showed that UC-MSCs proliferated more rapidly than AD-MSCs. Flow cytometry analysis showed that UC-MSCs and AD-MSCs were positive for CD13、CD44、CD73 and CD90, and 山 东 大 学 学 报 (医 学 版)56卷3期 -黄平,等.脐带和脂肪源性间充质干细胞生物学特性比较 \=-negative for CD34 and CD45. AD-MSCs had the stronger adipogenetic ability than UC-MSCs, but there were no difference on osteogenetic ability between these two cells. UC-MSCs has the stronger suppression effects on PBMC proliferation than AD-MSCs. Conclusion UC-MSCs are the more attractive cell population for usage in immune cellular therapy.
    Association of ApoE gene polymorphism and risk of atrial fibrillation among Han people in Shandong Province

    Objective To investigate the association of the polymorphism of codon 112 and codon 158 in the ApoE gene and risk of atrial fibrillation(AF)in Han people of Shandong Province. Methods A total of 92 diagnosed AF cases were included in the AF group and 93 hospitalized patients over the same period were selected as control group. The general clinical information and whole blood of all patients were collected, and then the genomic DNA was extracted. The polymorphisms of 112 locus and 158 locus in ApoE gene were detected with microarray. The count data were compared with chi square test and the Fisher exact test was used when the count was less than 5. The measurement data were compared with t test. The AF-related factors were analyzed with binary Logistic regression analysis. Results There were significant differences in the genotype frequency and allele frequency at codon 112 between the AF group and control group(P<0.05), while there were no significant difference at codon 158(P>0.05). There was statistical difference in the phenotype frequency of ApoE between the AF group and control group(P<0.05). People who carried ApoE4 had a higher risk of AF. Conclusion ApoE gene polymorphism is associated with the risk of AF among Han people in Shandong Province. ApoE4 is a sensitive phenotype and people who carry ApoE4 are more likely to develop AF than those who do not.
    Association between basophils percentage and chronic kidney disease: a retrospective cohort study

    Objective To explore the effect of basophils percentage on the incidence of chronic kidney disease(CKD). Methods A retrospective cohort was conducted using the data from Shandong Multi-center Longitudinal Cohort for Health Management. All subjects ≥20 years old who were free of CKD at baseline and accepted at least two annually physical examinations were selected. The participants were divided into four groups(denoted by Q1, Q2, Q3, Q4)according to quartiles of basophils percentage at baseline. Cox regression models were used to identify the association between basophils percentage and CKD. Results The cohort consisted of 17 173 subjects, including 10 614 males and 6 559 females. There were 737 CKD cases occurring during the 42 204.04 person-years following up, resulting in an incidence of 17.46/1 000 person-year. The multivariate Cox regression model with adjusting age and gender showed that HRs(95%CI)of basophils percentage to CKD for the groups of Q2, Q3 and Q4(with Q1 as reference group)were 0.990(0.776-1.263), 1.235(1.011-1.509)and 1.352(1.099-1.663)respectively. Furthermore, after all other related variables such as body mass index, hypertension, diabetes, blood uric acid, serum creatinine, blood urea nitrogen, total cholesterol, low density lipoprotein cholesterol and triglyceride were adjusted, the HRs(95%CI)of Q2, Q3 and Q4 山 东 大 学 学 报 (医 学 版)56卷3期 -周苗,等.嗜碱性粒细胞百分比与慢性肾脏病关系的回顾性队列研究 \=-were 0.966(0.740-1.262), 1.225(0.985-1.525)and 1.355(1.077-1.705), respectively. Conclusion Increasing basophils percentage is an independent risk factor of CKD.
