    Laparoendoscopic single-site surgery not only has the advantages of multi-site laparoscopic surgery, but also produces better abdominal appearance. In such procedures, a mini surgical instrument is placed around the navel, which 山 东 大 学 学 报 (医 学 版)57卷12期 -胡三元.经脐单孔腹腔镜手术的再认识 \=-exposes clear surgical fieldand facilitates operation without leaving obvious scars. Therefore, it is a very good choice. In clinical selection among multi-site, single-site, reduced site or mixed laparoscopic surgery, we should take into account a variety of factors and design to individually tailore surgical plan for patients.

    Laparoendoscopic single-site surgery is currently one of the hottest topics in gynecological surgery. From trans-umbilical to transvaginal single-site surgery, and from artificial to Da Vinci robotic single-site platform, the combination of robot and single-site surgery set up an arena full of possibilities. However, we think that robots are just tools, single-site is just a surgical method, while artificial intelligence is the future direction of laparoendoscopic single-site surgery.

    Thanks to its advantages of minimal trauma, aesthetic effect and fast recovery, single-site laparoscopic surgery has been applied to operations on gynecological benign and malignant tumors. This paper reviews the development, advantages and limitations of single-site laparoscopy in gynecologic oncology, summarizes the indications and key surgical techniques, and forecasts the prospect of single-site laparoscopy and robot-assisted single-site laparoscopy.

    Vulval cancer is one of the malignancies of female genital tract which is characterized by lymphatic metastasis. Inguinal lymphadenectomy is an important treatment for vulval cancer. Traditional open inguinal lymph node dissection may cause serious decline of patients’ quality of life as it is traumatic and causes many complications. In recent years, laparoendoscopic single-site surgery(LESS)is widely used in gynecology. This paper reviews the anatomy, transformation and application of LESS in inguinal lymphadenectomy for vulva cancer.

    A minimally invasive surgery,laparoendoscopicsingle-site surgery(LESS),has gained popularity as it is “scarless” through a single incision. However, the steep learning curve, in-line vision and surgical indications limit its wide acceptance among surgeons.This paper discusses the factors that restrict the development of LESS, such as the concept of doctors, bottleneck of surgical operation, learning curve, surgical instruments, surgical indications, and potential risks of surgery.

    Laparoscopic surgery is preferred for many gynecologic procedures. Compared with open surgery, minimally invasive surgery reduces morbidity, shortens hospital stay and improves recovery. Recently, laparoendoscopic single-site surgery(LESS)has drawn the attention of gynecologists because it can complete laparoscopic operations via small skin incision and thus achieve the goal of “few scars” or “no scar”. This paper reviews the advantages, complications, key surgical points and strategies of LESS.

    Objective To explore the role of cylindromatosis(CYLD)in vascular neointima formation and vascular smooth muscle cells phenotype transformation. Methods Cyld KO(Cyld-/-)mice were used to establish vascular injury models of carotid artery ligation. The neointima formation was observed with H&E and Van Gieson stainings. The cell apoptosis was examined with TUNEL staining, and cell proliferation and inflammation were detected with Ki67, cluster of differentiation 3(CD3)and monocyte chemoattractant protein-1(MCP-1)immunohistiochemistry, respectively. The mRNA expressions of tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β), interleukin-6(IL-6)and interleukin-10(IL-10)were determined with qRT-PCR. The CYLD expression was examined with immunofluorescence. The growth rate of VSMCs was observed with growth curve and 3H-thymidine intake assay. The effect of THP1 cells adhesion on VSMCs after TNF-ɑ stimulation was tested with monocyte adhesion assay. The expressions of alpha smooth muscle actin(α-SMA)and smooth muscle 22 alpha(SM22α)were measured with Western blotting. Results After carotid artery ligation for 4 weeks, the neointima formation and cell proliferation were more obvious in Cyld-/- mice than in wild type(WT)mice(P<0.01), while there was no significant difference in cell apoptosis. The expressions of CD3, MCP-1, TNF-ɑ, IL-1β and especially IL-6 were significantly higher in Cyld-/- mice than in WT mice(P<0.01), while the expression of IL-10 was significantly lower(P<0.01). CYLD had a low expression rate in cultured VSMCs. The proliferation rate of VSMCs was much higher in Cyld-/- mice than in WT mice. The adhesion effect of THP-1 was more obvious in Cyld-/- mice than in WT mice under TNF-ɑ stimulation(P<0.05). The expressions of α-SMA and SM22α were lower in Cyld-/- mice than in WT mice. Conclusion In vascular injury, CYLD is involved in vascular neointima formation by affecting vascular smooth muscle cells phenotype transformation and therefore plays an essential role in vascular remodeling.
    Biological effects of ubenimex on ovarian cancer A2780 cell

    Objective To investigate the effect of ubenimex on cell proliferation, cell cycle, apoptosis, autophagy, migration and invasion in ovarian cancer A2780 cell and its possible mechanism. Methods CCK-8 assay was used to analyze the effect of ubenimex on proliferation of A2780 cell. Transwell assay was performed to detect the effect of ubenimex on migration and invasion of A2780 cell. Flow cytometry was used to analyze the cell apoptosis and cell cycle. Western blotting was performed to investigate the protein expression levels of APN, AKT, p-AKT, LC3-B, and p62. Results (1)Ubenimex inhibited A2780 cell proliferation(Fconcentration=812.56,P<0.001;Ftime=8.58,P=0.010;Fconcentration×time=4.00,P=0.027), migration(t=56.9, P<0.001)and invasion(t=11.8, P<0.001), increased the cell proportion in G2/M phase (t=7.9, P=0.016)and decreased the cell proportion in G1 phase (t=14.8, P=0.005)which led to G2/M cell cycle arrest. (2)Ubenimex induced A2780 cell apoptosis(P=0.002), and cell apoptosis was increased when ubenimex was combined with AKT inhibitor(P=0.007). (3)Ubenimex reduced the APN expression, but increased p-AKT, LC3B and p62 expressions. Conclusion Ubenimex may be a treatment or adjuvant therapy for ovarian cancer, and its therapeutic effect can be enhanced in combination with AKT inhibitor.
    Evaluation of artifact of CBCT image of oral full crown prosthesis based on pixel gray value difference

    Objective To establish a method for the two-dimensional tomographic image’s artifact of oral full crown prosthesis in cone beam computed tomography(CBCT)based on pixel gray difference. Methods A cobalt-chromium porcelain crown was prepared for the full crown preparation of the mandibular first molar, and then was fixed on a photocurable resin fixing plate. The full crown was scanned with CBCT, and then the DICOM data were input into MIMICS10.0 software. The obvious layer of artifacts was selected in the two-dimensional tomogram, and the normal gray value of the crown(G0), correct crown edge gray value(GB)and visible crown edge gray value(GP)were measured with Photoshop CS6 software, thereby quantitatively determining the artifact value V and the crown edge sharpness index C according to the gray value difference of each pixel point of the image. Results By simulating the intraoral scanning environment, the CBCT scan data of the full crown restoration was successfully obtained. According to the analysis results, the normal full crown gray value G0 of cobalt chromium full crown restoration was obtained. Conclusion Using the difference of gray value of image pixels, the crown edge of oral prosthesis can be clearly determined, and a method for quantitative evaluation of crown artifacts in oral prosthesis is established. Clinically, physicians can use the evaluation method for two-dimensional tomographic image artifact in CBCT based on image pixel gray value difference to quantitatively evaluate the artifacts of various restorations, and select appropriate restorations for patients.

    Objective To analyze the genetic mutation types of 55 patients with hemophilia A(HA)in Shandong Province, and explore its molecular mechanism. Methods The intron 22 inversion mutation of factor Ⅷ(FⅧ)in all patients was detected with LD-PCR. The 26 exons of FⅧ negative patients were detected with next-generation sequencing and matched in the humangene mutation database. Results Of all 55 patients, 20 had intron 22 inversion mutation, and the other 35 had 28 different types of mutations, of whom 3 had 2 simultaneous mutations, while 6 had no mutation. Altogether 9 new mutation types unreported before were discovered, including c.2034_2062del, c.6052_6058del, c.6046_6047del, c.1009+1G>T, c.2997dupA, c.4001delT, c.1334T>A, c.1493G>A and c.557A>T. Conclusion FⅧ mutation is the direct cause of HA. The most common mutation in severe HA is intron 22 inversion mutation. The most common mutation in coding region mutation is the missense. The 9 new mutations discovered in the study enrich the gene mutation profile and provide reference for the study of HA pathogenesis.
    Clinical features and prognosis of adult secondary hemophagocytic syndrome: an analysis of 32 cases

    Objective To investigate the clinical features and prognostic factors of adult secondary hemophagocytic lymphohistiocytosis(HLH). Methods Clinical data of 32 cases of adult secondary HLH were collected, clinical features, etiology and recurrence were reviewed, and prognostic factors were analyzed with univariate and Cox multivariate analyses. Results Of the 32 patients, 17(53%)were male and 15(47%)were female, median age 57(19-88)years. Fever and elevated serum ferritin were observed in 32 cases, hepatosplenomegaly in 26 cases, and cytopenia in 25 cases. Etiology analyses showed that 14 cases(43.75%)were caused by malignant tumors, 10(31.25%)by infections, and 8(25%)by other factors. Ultimately 24(75%)deaths occurred. The median survival was 27 days and recurrence rate was 11.1%. One-way analysis of variance showed that age, rash, hemoglobin, triglyceride, clotting time and activated partial prothrombin time were related to death(P<0.05). Cox multivariate analysis indicated that age and triglyceride level were independent risk factors for prognosis. Conclusion Secondary HLH in adults has diverse etiology and clinical features. Advanced age and high triglyceride level are independent prognostic factors associated with death.
    Tumor-specific survival advantage in young patients with stage Ⅲ small cell lung cancer

    Objective To estimate the prognostic value of age on small cell lung cancer(SCLC)patients. Methods The Surveillance, Epidemiology, and End Results(SEER)population-based data was queried for SCLC patients from Jan. 2010 to Dec. 2013, and 17 237 cases were enrolled. The cases ≤ 49 years were defined as young group(n=733), 50-64 years were defined as middle-aged group(n=6 332), and the cases ≥ 65 years were defined as elderly group(n=10 172). Clinicopathological characteristics of patients were analyzed. Cancer-specific survival(CSS)data were obtained. The analysis of CSS outcomes and risk factors were analyzed by Kaplan-Meier methods and multivariable Cox regression model. Results Both univariate and multivariate analyses showed that CSS significantly decreased with advanced ages(χ2=342.08, P<0.001). The OS(χ2=203.9, P<0.001)and CSS(χ2=160.5, P<0.001)of the young group were improved. In middle-aged group, hazard ratio(HR)was 1.177(95%CI: 1.068-1.296, P=0.001); in elderly group, HR was 1.643(95%CI: 1.495-1.807, P<0.001). Smaller age guaranteed a significantly improved CSS in patients who were at stages Ⅰ(P=0.015), Ⅲ(P<0.001), Ⅳ(P<0.001)or underwent non-surgical treatment(P<0.001). Conclusion Compared to older patients, the SCLC patients ≤49 years hold advantages in both OS and CSS. The prognostic value of age is more exact in patients at stage Ⅲ or managed non-operatively.
    Correlation between urinary α1-antitrypsin and transferrin levels and hormone therapy effect on childrens primary nephrotic syndrome

    Objective To investigate the value of urinary α1-antitrypsin(AAT)and transferrin(TRF)levels in predicting the clinical efficacy of hormone therapy on children with primary nephrotic syndrome(PNS). Methods A total of 90 PNS children hospitalized in Qilu Childrens Hospital of Shandong University from May 2016 to June 2017 were studied. According to the response of glucocorticoid treatment for 4 weeks, they were divided into hormone-sensitive group (n=60)and steroid-resistant group(n=30). At the same time, 30 healthy children were selected as normal control group. Fresh morning urine(20 mL)was collected and centrifugated at 4 ℃, then supernatant was acquired. The basic information and biochemical index levels of theserum cholesterol, serum creatinine, albumin and 24 h urinary protein measured before and after hormone therapy were collected. Enzyme linked immunosorbent assay(ELISA)was used to determine the concentrations of AAT and TRF levels in urine, and the correction value by urinary creatinine(Cr) 山 东 大 学 学 报 (医 学 版)57卷12期 -潘艳艳,等.尿液α1-抗胰蛋白酶、尿转铁蛋白水平与小儿原发性肾病综合征激素治疗效果的关联分析 \=-was used to predict the response of PNS children to hormone therapy. Results Compared with normal control group, the ratios of urinary AAT/Cr and TRF/Cr in steroid-sensitive group and steroid-resistant group were higher(P<0.05). The urinary AAT/Cr ratio and TRF/Cr ratio were statistically different between steroid-sensitive group and steroid-resistant group(P<0.05). The regression analysis showed that urinary AAT/Cr ratio and TRF/Cr ratio were risk factors for the clinical efficacy of steroid therapy on PNS children(P<0.05). Conclusion The concentrations of urinary AAT and TRF in steroid-resistant children after treatment are higher than those in steroid-sensitive children. If the urinary AAT and TRF levels increase in early stage of PNS, the children may be steroid- resistant.
    Comparison and clinical significance of thrombelastogram in patients with cerebral infarction or cerebral hemorrhage

    Objective To investigate the changes of thromboelastogram(TEG)indicators in patients with cerebral infarction and cerebral hemorrhage, and the correlation with the results of traditional coagulation tests. Methods A total of 73 cases of cerebral infarction and 46 cases of cerebral hemorrhage admitted during March 2018 and March 2019 were selected as the research subjects, and 20 healthy people who received physical examination in our hospital during the same period served as healthy controls. The main indicators of TEG and traditional coagulation function indicators were detected, including coagulation reaction time(R value), coagulation formation time(K value), blood clot formation rate or α angle(Angle), maximum thrombus amplitude(MA), coagulation index(CI), prothrombin time(PT), activated partial prothrombin time(APTT), thrombin time(TT), fibrinogen concentration(FIB), D-Dimer, and platelet counts. Results There were significant differences in the values of K, Angle, MA and CI among the 3 groups(P<0.05)while no significant difference in the R value. The K value of hemorrhage group was higher than that of infarction group, while the platelet count(PLT)was lower(both P<0.05). The APTT in the hemorrhage group was lower than that in the infarction group, while PT and D-Dimer were higher(all P<0.05). There were no significant differences in FIB and TT between the two groups. There were positive correlations between R and APTT, between K, PT and Angle, between MA, CI, FIB and PLT, while there were negative correlation between K and FIB, between PLT, MA and PT, between TT, CI, PT and APTT(all P<0.05). Conclusion Cerebral infarction and cerebral hemorrhage are complicated in the acute onset stage. The traditional coagulation function test is one-sided and untimely. TEG can reflect the true coagulation status more intuitively and accurately, so as to improve the diagnosis and treatment efficiency.
    A clinical analysis of pelvic arterial embolization for postpartum hemorrhage

    Objective To evaluate the clinical advantages, method selection, efficacy and safety of pelvic arterial embolization(PAE)in the treatment of postpartum hemorrhage(PPH). Methods Clinical data of 46 PPH cases who received PAE in our hospital during Jan. 2016 and Jan. 2018 were retrospectively analyzed. The clinical operating characteristics and effectiveness of PAE, short- and long-term complications and re-fertility outcomes were evaluated. Results Among the 46 cases,24 had virginal delivery and 22 received cesarean section.The most frequent cause of PPH was placental problems(63.04%), followed by uterine atony(52.17%), uterine arteriovenous malformation(8.70%), and genital tract trauma(4.35%). Eight cases had combined causes. All 46 patients received one or more interventions before PAE, and PAE was performed when these interventions failed. Altogether 133 arteries received PAE, including bilateral uterine arteries in 27 cases, bilateral internal iliac arteries in 1 case, bilateral uterine arteries and bilateral iliac arteries in15 cases, bilateral inferior epigastric arteries in 1 case,bilateral ovarian arteries in 1 case, and bilateral uterine arteries, bilateral internal iliac arteries, and right ovarian artery in 1 case. Altogether 51 PAE were performed, and the overall technical success rate was 100%(51/51). Of all 46 patients, 3 had no effective relief after the first PAEand had to receive asecond one; 1 received 3 consecutive PAE; 1 received hysterectomy after PAE failed. The clinical success rate was 88.23%(45/51). Fever occurred in 6 patients after PAE. Of the 42 patients who were followed up, 39(92.86%)resumed normal menses, 3(7.14%)reported amenorrhea, and 7(16.67%)had pregnancy again. Conclusion PAE is effective and safe in the treatment of postpartum hemorrhage.

    Objective To evaluate and compare the incidence of menopausal syndrome in pre-, peri- and post-menopausal women in rural areas, and to examine the association with neuroticism and mindfulness. Methods The Menopause Rating Scale(MRS), neuroticism subscale of TIPI-C, Mindful Attention Awareness Scale(MAAS)and the general questionnaire were used to investigate 836 middle-aged women in Shandong rural area. Results The incidence of menopausal syndrome in pre-, peri- and post-menopausal women was 54.1%, 65.2% and 70.9%, respectively. The score of menopausal syndrome in the above status was 6.56±5.72, 8.46±6.58 and 9.28±7.13, respectively. Multivariate analysis showed that age(OR=1.051, 95%CI: 1.004-1.099), chronic diseases(OR=2.705, 95%CI:1.912-3.828), gynecologic diseases(OR=3.007, 95%CI: 1.953-4.631), and neuroticism(OR=1.204, 95%CI: 1.116-1.299)were risk factors of menopausal syndrome, while mindfulness(OR=0.941, 95%CI: 0.926-0.956)was the protective factor. Conclusion The incidence of menopausal syndrome is common in rural middle-aged women, which is the highest in post-menopausal women, followed by peri- and pre-menopausal women. Due attention should be paid to middle-aged women with high neuroticism, and improving the level of mindfulness may be an effective way to alleviate menopausal syndrome.
    Correlation between health literacy,maternal mortality and infant mortality in Shanghai residents from 2008 to 2017

    Objective To explore the relationship between health literacy(HL), maternal mortality rate(MMR)and infant mortality rate(IMR)among Shanghai residents. Methods The HL, MMR and IMR data from Shanghai Municipal Bureau of Statistics and other literatures were used for descriptive statistical analyses to show the changing trends. Correlation and regression results between HL, MMR and IMR were obtained from Spearman rank correlation and generalized linear model analyses. Results The HL level had increased continuously from 6.97% to 25.36% between 2008 and 2017, while MMR and IMR declined. Spearman rank correlation analyses showed that there was a significant negative correlation between HL, healthy lifestyle and behavior(HLB), level of scientific health concept literacy(LSHCL), level of safety and first aid literacy(LSFAL)and MMR(the correlation coefficients were -0.817, -0.683, -0.783, -0.717, P<0.05), and IMR(the correlation coefficients were -0.833, -0.717, -0.883, -0.800, -0.683, P<0.05). Regression results indicated that HL, HLB, LSHCL and LSFAL had affected MMR and IMR(P<0.05), and LBML could affect IMR(P<0.05). Conclusion There is a significant negative correlation between HL, MMR and IMR. HL has an important impact on maternal and infant health. We suggest that government policies, community health promotion programs and HL monitoring programs should pay due attention to HL.
    Factors influencing awareness of hypertension among rural adults in Pingyin County of Jinan City

    Objective To investigate the awareness rate of hypertension and its influencing factors in rural areas of Pingyin County of Jinan City, and to provide reference for the prevention and control of hypertension. Methods During Oct. and Dec. 2017, 2 283 adult rural residents aged 18-90 years were selected by cluster sampling. They then received a questionnaire survey and physical examination. Results Of the 2 283 subjects, 1 489 were hypertensive. The crude prevalence rate of hypertension was 65.22%, and standardized prevalence rate was 45.85%. Among the hypertensive patients, 491 knew that they had hypertension, and the awareness rate was 32.98%. After 1∶1 match was made according to the same gender and age difference of no more than 2 years old, 482 hypertension knowers were successfully matched. Univariate Logistic regression analyses showed that the awareness rates of hypertension among subjects with different sleep quality, drinking history, family history of hypertension, history of diabetes, hyperglycemia or cardiocerebrovascular diseases, body mass index, central obesity and self-rated health status were significantly different(P<0.05). Conclusion The prevalence of hypertension among rural adults in Pingyin County of Jinan City is high, while the awareness rate is low. Hypertension has become an important public health hazard in this area. Effective measures should be taken to improve the comprehensive management of hypertension.
    3D laparoscope-assisted resection of gastric glomus tumor: a case report

    Objective To investigate the clinical manifestations, differential diagnosis and treatment of gastric glomus tumor(GGT). Methods The clinical features, diagnosis, differential diagnosis, pathological features and treatment of one GGT patient were analyzed, and related literature was reviewed. Results The patient underwent 3D laparoscope-assisted local gastric resection in our enhanced recovery after surgery(ERAS)programs. Postoperative HE staining and immunohistochemical pathology confirmed the case was GGT. The patient recovered well after surgery. Conclusion GGT is a rare benign tumor derived from the antrum of stomach. Diagnosis of GGT mainly depends on postoperative histopathology and immunohistochemistry. Surgical resection is safe and has good short-term outcomes.
