    Menopause is a universal event for women. As a result of ovarian dysfunction, it causes changes in physiological, physical, and mental symptoms as well as some long-term chronic diseases. The perimenopausal womens health care is a comprehensive measure. It is very important to take reasonable and effective interventions for perimenopausal women. The past decade has witnessed an amazing progress in the understanding of menopause hormone therapy(MHT)and in the choice of treatment modalities that are available to address the symptom burden of menopause.     Perimenopausal women undergo a decline of the whole bodys function, especially the ovarian aging accompanied with endocrine changes, which will induce a series of physical and mental discomforts even illnesses, and will 山 东 大 学 学 报 (医 学 版)57卷2期 -朱小明,等.围绝经期妇女内分泌变化及相关疾病 \=-affect the life and living quality in a negative way. With more knowledge of the critical changes in perimenopause, it is now realized that perimenopause is not just a simply physiological period, but an important period that need more protection and health cares. Deep understanding of the natural aging and illnesses in perimenopausal women is essential to maintain the natural physiology, guarantee the health and improve the living quality.

    Ovarian dysfunction is a prominent manifestation of female aging. Women will experience menstrual changes until menopause, accompanied by a variety of menopausal-related symptoms. Menopause not only has a persistent adverse effect on cardiovascular, skeletal and cognitive functions, but also leads to various changes in the immune system. Studies have shown that estrogen can regulate humoral and cellular immunity, and the imbalance of estrogen content and function can lead to immune dysfunction. Menopausal hormone therapy can not only improve menopausal-related symptoms, but also improve ovarian hormone deficiency-related immune aging, enhance the immune function of normal aging women, and improve the quality of life of menopausal women.

    As aging causes many diseases, delaying the process of aging can prevent some of the diseases. Ovarian aging,a special type of organ senescence, is considered as the pacemaker of female body aging, which drives the aging of multiple organs and leads to several aging associated disorders. Hence, it is of great significance to investigate the mechanisms of ovarian aging and explore ways to delay the progress. This review attempts to discuss the latest advances in the molecular mechanisms of ovarian aging and the current status of preventive measures, in order to offer new insights into the prevention and treatment of ovarian aging and related diseases.

    The factors that affecting fertility are very complex. Age growth is the most irresistible factor for fertility decline. Other factors, including heredity, environment, psychosocial status and lifestyle, also accelerate the damage of fertility. With the progress of science and technology, many indicators were employed for fertility assessment. It is suggested that men and women at childbearing age, especially elderly couples, should timely accept fertility assessment to eliminate pregnancy-related risks, and correctly choose assisted reproductive technology or egg donation, and understand the upper limit of ART age.

    With the development of antineoplastic treatments, the survival rate of cancer patients has been greatly improved, and fertility preservation has been put on more and more attention from doctors and patients, especially with the sub-discipline of oncofertility. With the improvement of modern living standards, womens reproductive age is postponed and there is a great demand for fertility preservation. Oocytes cryopreservation and embryos cryopreservation are common methods of fertility preservation. Ovarian tissue cryopreservation is no longer a pilot technique in developed countries. This article mainly describes the indications of fertility preservation and the progress of commonly used fertility preservation methods.

    Objective To compare the effectiveness and safety of different doses of menopause hormone therapies(MHT)in the remission of menopausal symptoms. Methods The clinical data of 450 perimenopausal women who received MHT during 2012 and 2017 were retrospectively analyzed. According to the doses of estradiol valerate, the pa- 山 东 大 学 学 报 (医 学 版)57卷2期 -徐文娴,等.围绝经期应用不同剂量绝经激素治疗的临床观察 \=-tients were divided into 3 groups:group A(standard dose, n=62), group B(low dose, n=322)and group C(ultralow dose, n=66). One treatment cycle lasted for 28 days, during which the continuous sequential regimen was adopted. Before and after treatment, modified Kupperman menopausal index(mKMI)were surveyed, reproductive hormones and glucolipid metabolic indexes were detected, and breast ultrasound was performed. Results Before treatment, there were no significant differences in the general characters and clinical indexes among the 3 groups(all P>0.05). Compared to baseline, the mKMI scores and follicle stimulating hormone(FSH)level significantly decreased, while estradiol level significantly increased after 1, 3 and 6 treatment cycles in all 3 groups(all P<0.001). After 6 treatment cycles, the level of high-density lipoprotein cholesterol(HDL-C)markedly increased in groups B and C, and the levels of low-density lipoprotein cholesterol(LDL-C), fasting blood glucose and fasting insulin significantly decreased in groups A and B(all P<0.01). There were no statistical changes in the thickness of mammary glands, size of mammary nodules and dilatation of ducts in the 3 groups before and after treatment(all P>0.05). Breast distention occurred in 7 cases(11.3%)in group A, 14 cases in group B(4.3%)and 1 case in group C(1.5%), the difference in incidence between groups A and C was significant(P<0.05). Conclusion The standard, low and ultra-low doses of MHT can effectively relieve the menopausal symptoms in perimenopausal women. The standard and low dose are superior to the ultra-low dose in improving glucolipid metabolism. The ultra-low dose has advantages in tolerance. The different doses of MHT has no effects on the mammary glands during the observation period.
    Effects of human placenta mesenchymal stem cell transplantation on ovarian function in premature ovarian failure in rats

    Objective To investigate the antioxidant potential of human placenta mesenchymal stem cell(hPMSC)transplantation on the silent information regulator transcript-1(SIRT1)and nuclear factor-erythroid 2-related factor 2(Nrf2)and peroxisome proliferator-activated receptor gamma coactivator1-α(PGC1-α)in the treatment of premature ovarian failure(POF)in rats. Methods A total of 60 SD female rats with normal estrous cycle were randomly divided into 4 groups(n=15): blank control group, POF model group, hPMSC transplantation group, and treatment control group. The POF models were established by intraperitoneal injection of cyclophosphamide. The serum levels of estradiol(E2), follicle-stimulating hormone(FSH)and anti-müllerian hormone(AMH)were determined with ELISA. The fibrosis and histomorphological features of ovarian tissues were observed with Van Gieson(VG)staining and HE staining. The expressions of SIRT1, Nrf2 and PGC1-α were detected with Western blotting and immunohistochemical staining. Results Serum FSH level was significantly higher in the POF model group than in blank control group(P<0.05), while E2 and AMH levels were significantly lower(P<0.01). Compared with the treatment control group and POF model group, the hPMSC transplantation group had decreased FSH level and increased AMH level(P<0.01). HE staining showed that in the POF model group, the numbers of primordial follicles and primary follicles increased, while the number of atretic follicles decreased; in the hPMSC transplantation group, the number of normal follicles increased, while the number of atretic follicles decreased. Immunohistochemical staining results showed that the protein expressions of SIRT1, Nrf2 and PGC1-α were significantly lower in the POF model group than in the blank control group, and significantly higher in the hPMSC transplantation group than in the POF model group. Western blotting results showed the same tendency of changes(P<0.01). Conclusion The hPMSC transplantation may have antioxidant potential and protective effects on ovarian by activating the SIRT1/PGC1-α/Nrf2 signaling pathway.

    Objective To explore the effect of human cord blood mononuclear cells(CB-MNCs)on relieving the gastrocnemius muscle atrophy and promoting the damage repair in the isolated sciatic nerve rats after intramuscular administration. Methods CB-MNCs were isolated from healthy term infants and cultured in vitro. Thirty juvenile Wistar rats weighting about 200 g were randomized into three groups: CB-MNCs intramuscular administration group(Therapy group,n=10), saline intramuscular administration group(NS group,n=10)and healthy group(Normal group,n=10). The animal model of denervated gastrocnemius muscles were formed by cutting both sides of the sciatic nerve of rats 山 东 大 学 学 报 (医 学 版)57卷2期 -刘子琳,等.人脐血单个核细胞对大鼠腓肠肌失神经损伤的治疗作用 \=-which resulted in the nerve disconnection about 1 cm in the first two groups. One million of CB-MNCs were injected into the gastrocnemius muscles of rats in d7, d14, and d21 after despairing the nerves of Therapy group. The rats in Normal and NS groups were only injected saline with equal volume at the same time. Hind limb movements and gangrenous area ratios of rats were observed. The both sides of gastrocnemius muscles of each rat were weighed to measure wet weight ratios and HE staining was taken to determine cross-sectional area of muscle fiber at the end of the forth week. Peripheral blood serum and bilateral gastrocnemius muscles were taken to measure creatine kinase(CK), catalase(CAT), malondialdehyde(MDA), and superoxide dismutase(SOD)levels. The mRNA expressions of Bax, B-cell lymphoma-2(Bcl-2), Caspase-3, vascular endothelial growth factor-α(VEGF-α), tumor necrosis factor-α(TNF-α), interleukin-10(IL-10), α-actin and Dystrophin were tested by qRT-PCR. The expressions of α-actin, Dystrophin and VEGF-α protein were detected by Western blotting. The expression of Dystrophin in cells was also observed by immunohistochemistry. Results The gangrenous area ratio of the Therapy group was lower than that of the NS group(P=0.002)and the wet weight of the gastrocnemius muscle in the Therapy group was higher than that in the NS group(P<0.001). The HE staining results showed that the muscle cross-sectional areas of the Normal group, NS group and Therapy group were respective(12 452.0±202.8),(6 287.0±142.2), and(8 193.0±115.5)μm2 with a statistical difference(P<0.001). Compared with the NS group, the CK and MDA contents of the serum and muscles in the Therapy group were significantly decreased, and the SOD and CAT contents were significantly increased(all P<0.001). In contrast to the NS group, the mRNA levels of Caspase-3, Bax and TNF-α decreased while the mRNA levels of Bcl-2, IL-10, α-actin, Dystrophin and VEGF-α increased in the Therapy group(all P<0.05). Compared with the NS group, the expressions of α-actin, Dystrophin and VEGF-α protein in the Therapy group were increased(all P<0.05). IHC analysis showed that the expression of Dystrophin was increased in both NS and Therapy groups, but the expression of Dystrophin in the Therapy group was significantly higher than that in the NS group(all P<0.05). Conclusion Injecting CB-MNCs into the paralyzed gastrocnemius can reduce the proportion of apoptotic cells, promote the wound healing, prevent muscle atrophy, and improve muscle functions.

    Objective To explore the expression and significance of Forkhead box protein O1(FOXO1)in ovarian granulosa cells of patients suffering from polycystic ovary syndrome(PCOS). Methods A total of 130 patients, including 67 PCOS patients and 63 controls, were enrolled. The expression of FOXO1 in ovarian granulosa cells was examined by real-time quantitative reverse transcription PCR(qRT-PCR), and the clinical significance was analyzed. Results PCOS patients had significantly higher body mass index(BMI), homeostasis model assessment-insulin resistance(HOMA-IR), luteotropic hormone(LH), testosterone(T), anti-Mullerian hormone(AMH), average follicle count(AFC)levels(all P<0.05), but lower FSH level(P<0.05)than controls. Compared with controls, the expression of FOXO1 was much lower in granulosa cells of PCOS patients(P<0.05). Binary Logistic regression analysis revealed that FOXO1, AFC and HOMA-IR were risk factors influencing the occurrence of PCOS(P<0.05). Conclusion The lower expression of FOXO1 in the granulosa cells of PCOS patients may play an important role in the occurrence and development of PCOS.

    Objective To explore the effects of FK506 binding protein 52(FKBP52)on proliferation of human endometrial stromal cells(HESCs), and provide the new basis and target for treatment of thin endometrium. Methods Normal HESCs were isolated and cultured. After infecting HESCs with lentiviruses contain GFP-FKBP52 or transfecting HESCs with FKBP52-targeting small interfering RNA(siRNA)to overexpress or knockdown FKBP52 respectively, the cells proliferation and colony-formation were compared with CCK8 assay and plate clone formation assay. Besides, changes in cell cycle were detected by flowcytometry and expression levels of FKBP52 and cell cycle-related proteins were examined by Western blotting. Results After overexpressing FKBP52, the proliferation activity of HESCs was enhanced(P<0.05), and the clone formation ability of cells was improved(P<0.01). The process of cell cycle was accelerated(P<0.01)and the expression levels of cell cycle dependent kinase 4(CDK4), CyclinD1, cell cycle dependent kinase 2(CDK2)and CyclinE1 were increased(P<0.05). Affected by FKBP52 knockdown in transfected cell, the proliferation capacity of endometrial stromal cells was reduced(P<0.05), and the clone formation capacity was weakened(P<0.05). The cell cycle process was blocked(P<0.05)and expression levels of CDK4, CyclinD1,CDK2 and CyclinE1 were suppressed(P<0.05).     Hospitalization expenses and the influencing factors for scar uterine maternal subsequent delivery in a tertiary hospital of Shandong Province

    Objective To analyze the influencing factors and structural changes of the hospitalization expenses for scar uterine maternal subsequent delivery in a tertiary hospital of Shandong Province. Methods A total of 3 495 cases of hospitalized scar uterine maternal subsequent delivery from January 2014 to December 2016 were collected. Descriptive analysis was conducted on hospitalization expenses and its composition. Univariate analysis and multiple linear regression were applied to analyze its influencing factors. Results The median of hospitalization costs in 2014, 2015 and 2016 were RMB 9 045.7 Yuan, 9 703.6 Yuan and 10 612.4 Yuan, respectively. The medical materials expense accounted for the largest constitution of hospitalization costs, followed by western medication and surgery. The total medicine proportion was 24.98%, and the proportion of medical materials was 25.84%. Gestational age, placenta previa, premature rupture of membranes, gestational diabetes, anemia, delivery mode, antibiotics and hospitalization days were the main influencing factors for hospitalization expenses. Conclusion Comprehensive interventions are needed to control the cost growth by encouraging scar uterine maternal to take vaginal delivery, regulating the use of antibiotics, strengthening prenatal care for elderly pregnancy, so as shortening hospitalization days. 山 东 大 学 学 报 (医 学 版)57卷2期 -邵纯纯,等.山东省某三甲医院瘢痕子宫产妇再次分娩住院费用及其影响因素 \=-

    Objective To explore the role of adenosine monophosphate dependent protein kinase(AMPK)in the pathogenesis of adenomyosis by investigating its expression and localization in the eutopic endometrial tissues in adenomyosis. Methods The localizations of AMPK in eutopic adenomyosis endometrium(20 cases)and normal endometrium(20 cases)were detected with immunohistochemistry. The mRNA and protein expressions of AMPK and M2 pyruvate kinase(PKM2)were determined with Real-time PCR and Western blotting, respectively. Results AMPK was mainly expressed in the glandular epithelial cytoplasm in normal endometrium and eutopic adenomyosis endometrium. Immunohistochemistry showed that AMPK expression was higher in eutopic adenomyosis endometrium than in normal endometrium either in the secretory or proliferative phases(P<0.001), while there were no statistical differences in the expression of AMPK in the proliferative phase and secretory phase of each group(all P>0.05).     Objective To explore the correlation between BMI before pregnancy and postpartum pelvic floor dysfunction in women undergoing vaginal delivery. Methods The pelvic floor muscle function of maternity were tested on 42-day-old after childbirth and questionnaire survey was conducted to collect relevant information. Chi-square test and Logistic regression were used to analyze the association between BMI before pregnancy and pelvic floor muscle dysfunction. The median effect of perineal laceration on the relationship between BMI before pregnancy and pelvic floor muscle dysfunction was analyzed by sequential test and sobel test. Results The incidence rate of pelvic floor muscle dysfunction in the women was 85.28%(1 112/1 304). After controlling of the confounding factors, such as body weight, gestational age, age, maternity education and birth times, the results of Logistic regression analysis were as follows: obesity before pregnancy increased the risk of pelvic floor type 1(OR=2.765, 95% CI:1.441-5.305)and type 2(OR=2.367, 95% CI:1.084-5.170)muscle dysfunction. The vaginal delivery of degree two and above perineal laceration increased the risk of pelvic floor type 1 (OR= 9.159, 95%CI:1.910-43.926)and type 2 (OR= 8.008, 95%CI:1.346-47.640)muscle dysfunction. Mediating effect analysis showed that the perineal laceration had an incomplete mediating effect on pelvic floor muscle dysfunction caused by obesity, and the mediating effect percentage was 25.43%.     Mediating effect of psychological capital on mindfulness and psychological distress among men in infertile couples

    Objective To evaluate the level of psychological distress among men in infertile couples, analyze the relationship among mindfulness, psychological capital and psychological distress, and investigate the mediating effect of psychological capital on mindfulness and psychological distress. Methods A total of 300 men in infertile couples were surveyed with Mindful Attention Awareness Scale(MAAS), Positive Psychological Capital Questionnaire(PPQ)and 10-item Kessler Psychological Distress Scale(K10). The mediating effect of psychological capital was tested with Bootstrap. Results The mean score of K10 was(20.20±6.70), and 73.3% of the subjects had psychological distress. Hierarchical regression analysis revealed that the K10 scores were negatively correlated with the scores of MAAS(β=-0.341, P<0.001)and PPQ(β=-0.326, P<0.001)after significant demographic variables were controlled, and psychological capital had a mediating effect on the correlation between mindfulness and psychological distress. Bootstrap test indicated that the mediating effect was significant. Conclusion The incidence of psychological distress is high among men in infertile couples. Focus on individual mindfulness and psychological capital may be an effective way to improve psychological distress.
    Association of catechol-O-methyl transferase gene G1947A polymorphism with smoking behavior: a Meta-analysis

    Objective To explore the relationship between catechol-O-methyl transferase(COMT)gene G1947A polymorphism and smoking behavior. Methods Relevant literature were searched in PubMed, EMBASE, Web of Science and Cochrane Library from inception up to Nov. 2018. Data were strictly screened according to the inclusion and exclusion criteria. The combined effect values and the 95% confidence intervals (CI) were calculated based on Meta-analysis. The differences in gender and ethnicity were investigated with subgroup analysis. Results A total of 16 researches were included, with 7 565 current smokers, 6 491 ex-smokers, 9 370 never smokers, 14 535 ever smokers and 16 140 nonsmokers involved. Significant effects of smoking initiation with COMT G1947A polymorphism were found in the dominant model(OR=0.92, 95%CI=0.85-0.98, P=0.014), especially in Asians(OR=0.77, 95%CI=0.62-0.93, P=0.025). The associations of COMT G1947A polymorphism with smoking persistence and cessation were not significant in the 3 models. After sensitivity and influence analyses, the pooled results did not change substantially. No publication bias was detected. Conclusion The A allele in COMT G1947A has a modest protective effect for smoking initiation.
