摘要： 目的 提高对睾丸肿瘤病理特征及预后的认识。 方法 回顾性分析2010年6月至2018年6月86 例睾丸肿瘤患者临床资料,统计睾丸肿瘤的病理类型、治疗方式、肿瘤标志物、患者生育及性功能情况,分析各指标间相关性及预后的影响因素。 结果 患者年龄3个月~61岁,平均29.4岁,其中76例生殖细胞瘤(40例精原细胞瘤)、5例淋巴瘤、4例间质细胞瘤、1例腺肉瘤。肿瘤合并隐睾者14例。畸胎瘤及卵黄囊瘤多见于小儿患者,精原细胞瘤、胚胎性癌等生殖细胞肿瘤20~40岁高发,间质细胞瘤、淋巴瘤等非生殖细胞瘤常见于40岁以上患者。随访患者83例,10例死亡,死亡率与病理类型、肿瘤直径(<4 cm 3.33%、4~7 cm 9.52%、>7 cm 35.71%)、分期(Ⅰ期5.17%、Ⅱ期40.00%、Ⅲ期66.67%)、是否合并隐睾相关(χ2=6.42, P=0.003);16.44%患者生活质量受影响,61.11%有生育障碍,38.59%性功能减退。人绒毛膜促性腺激素β亚基(β-HCG)、甲胎蛋白(AFP)在非生殖细胞瘤中均正常,在各类型生殖细胞瘤中表达存在差异。胎盘碱性磷酸酶(PLAP)、CK、CD117等作为免疫组化指标对鉴别肿瘤类型有帮助。 结论 睾丸肿瘤病理类型多样且具有明显的年龄分布及标志物特征,预后的影响因素包括肿瘤类型、分期、大小及是否合并隐睾,患者治疗后生育及性功能均受到影响。
