肠内和肠外营养支持对早期重症脑卒中患者血清ALT、SCr浓度变化及并发症的影响

摘要/Abstract

摘要： 目的探讨肠内营养支持和肠外营养支持对早期重症脑卒中患者血清丙氨酸氨基转移酶(ALT)、血清肌酐(SCr)浓度变化及并发症的影响。方法将住院治疗的60例早期重症脑卒中患者采用随机数字表法分组,按照给予肠外和早期肠内营养支持的不同方式分成两组各30例,对两组患者在治疗前后的血清ALT、SCr浓度变化及并发症发生情况进行分析评价。结果治疗后两组患者的血清ALT浓度随时间延长均有显著性升高,差异有统计学意义(P<0.01),肠内营养组患者血清ALT浓度的升高幅度明显低于肠外营养组,差异有统计学意义(P<0.01);两组患者的血清SCr浓度随时间延长均有升高趋势,且升高幅度相当,差异无统计学意义(P>0.05);肠内营养组患者的并发症发生率为16.7%,显著低于肠外营养组的40.0%,差异有统计学意义(P<0.05)。结论肠内营养支持在早期重症脑卒中患者的治疗中对肝功能的保护作用较大且并发症少,临床效果优于肠外营养支持。

Abstract: Objective To compare the effects of enteral and parenteral nutrition on serum alanine transaminase(ALT), serum creatinine(SCr)and complications in patients with early severe stroke. Methods A total of 60 patients with early severe stroke were randomly divided into two groups:30 patients in the parenteral group were given parenteral nutrition support, while another 30 patients in the enteral group were given early enteral nutrition support. The serum ALT and SCr before and after treatment, and incidence of complications were compared between the two groups. Results The serum ALT concentration significantly increased after treatment in both groups (P<0.01). The increase of ALT in the enteral group was significantly lower than that in the parenteral group (P<0.01). The serum SCr concentration showed comparable increase in both groups after treatment (P>0.05). The incidence of complications in the enteral group was 16.7%, which was significantly lower than 40.0% in the parenteral group (P<0.05). Conclusion Compared with parenteral nutrition support, enteral nutrition support has greater protective effects on liver function, causes fewer complications, and has better clinical effects in the treatment of early severe stroke.
