摘要: 肝癌、胃癌、胰腺癌、结直肠癌等腹部恶性肿瘤的发病率、死亡率均居全身肿瘤前列,且难“完整”切除,易复发、转移,大多数患者被确诊时已是中晚期,使得治疗更加棘手。部分肿瘤化疗效果欠佳,靶向治疗的出现,一定程度改善肿瘤的预后,但不可避免的终会走向耐药。放射性粒子植入组织间低剂量率持续照射治疗肿瘤以其定位准确、高度适形、杀伤持久、安全可靠为晚期恶性肿瘤患者带来一缕曙光,但对部分并发症发生机制及防治措施的研究尚属初步,常见的有消化道反应、感染、出血、溃疡等,部分患者出现危及生命的并发症,如大出血、肺栓塞等,这些症状的正确处理不容忽视,有待临床重视及规范。

Abstract: Abdominal malignant tumors such as liver cancer, gastric cancer, pancreatic cancer and colorectal cancer have high morbidity and mortality due to easy recurrence and metastasis but hard resection. Most of them are already advanced once detected. Whats more, some tumors are resistant to chemotherapy. Targeted medicine therapy can improve the prognosis to a certain extent, but it will inevitably lead to resistance. Radioactive seeds are a new and effective method for patients with advanced malignant tumors because it is highly conformal, durable and safe. Besides, it can accurately locate the tumor. However, the mechanism and prevention of complications still need to be explored, such as common complications including gastrointestinal reaction, infection, bleeding, ulcers, and life-threatening complications including bleeding and pulmonary embolism. Great attention should be paid to the management of these complications.
