OSAHS患者PSG参数与24小时动态血压节律变化的相关性

Association between PSG parameters and 24 hour ambulatory blood pressure rhythm in OSAHS patients

摘要:

目的  探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者多导睡眠图(PSG)参数与夜间及日间血压增高的相关性。方法  将223例受试者分为3组:单纯OSAHS组(无高血压的OSAHS患者)96例,OSAHS合并高血压组66例,对照组(PSG监测和24h动态血压(ABPM)监测均正常的健康人)61例。所有受试者于检测当日上午7:00至次日上午7:00行ABPM监测,当日21:00至次日7:00行PSG监测,比较3组患者PSG参数及ABPM参数结果,并分析OSAHS合并高血压组患者PSG参数与ABPM参数之间的相关性。结果  单纯OSAHS组和OSAHS合并高血压组患者PSG参数中呼吸暂停低通气指数(AHI)、微觉醒指数、90%以下氧减饱和度指数和氧减指数水平与对照组比较,差异均有统计学意义(P<0.01);OSAHS合并高血压组患者AHI、微觉醒指数、90%以下氧减饱和度指数和氧减指数水平与单纯OSAHS组比较,差异均有统计学意义(P<0.01);对OSAHS并高血压组患者日间平均收缩压(DABPS)水平与各PSG参数之间的相关性做多元逐步回归分析显示,仅有微觉醒指数被引入回归方程(偏回归系数=0.55249,P=0.0001);日间平均舒张压(DABPD)水平与各PSG参数之间相关性分析显示,仅有氧减指数被引入回归方程(偏回归系数=0.34201,P=0.0293);夜间平均收缩压(NABPS)水平与各PSG参数之间间相关性分析显示,微觉醒指数、呼吸暂停+低通气总时间和氧减指数被引入回归方程(偏回归系数=0.34216,P=0.0287;偏回归系数=0.46876,P=0.0308;偏回归系数=0.40204,P=0.0249);夜间平均舒张压(NABPD)水平与各PSG参数之间相关性分析显示,呼吸暂停+低通气总时间、90%以下氧减饱和度指数和氧减指数被引入回归方程(偏回归系数=0.39443,P=0.0390;偏回归系数=0.39142,P=0.0399;偏回归系数=0.40360,P=0.0246)。结论  OSAHS患者的血压失去了正常的昼夜节律变化,表现为非“杓”状血压变异;OSAHS患者睡眠呼吸紊乱与高血压可能存在因果关系,睡眠呼吸紊乱程度愈高,愈易形成高血压;OSAHS合并高血压组患者日间收缩压与微觉醒指数呈正相关,日间舒张压与氧减指数呈正相关,夜间收缩压与微觉醒指数、呼吸暂停+低通气总时间和氧减指数呈正相关,夜间舒张压与呼吸暂停+低通气总时间、90%以下氧减饱和度指数和氧减指数呈正相关。

Abstract:

Objective  To investigate the correlation between PSG parameters and high blood pressure in night and day for obstructive sleep apnea-hypopnea syndrome (OSAHS) patients. Method   223 cases were divided into three groups: the simple OSAHS group of 96 cases, the OSAHS  with hypertension of 66 cases and  the normal control group of 61 cases. All subjects were subjected to ABPM monitoring for two periods, from 7:00 to 21:00 and from 21:00 to 7:00  the next day. While these subjects were subjected to PSG monitoring from 21:00 to 7:00 on the next day . Regression analysis was separately performed to find the correlation between blood pressure (ABPM) parameters  and PSG parameters in OSAHS patients with hypertension. Result    PSG parameters of AHI, night-awakening index, below 90 percent by oxygen and oxygen saturation index were different between  the simple OSAHS group, the OSAHShypertension group and the normal control group (P<0.01). The multiple regression analysis on the correlation of DABPS and PSG parameters for OSAHS patients with hypertension showed that only nightawakening number (times/H) was introduced into the regression equation(partial regression coefficient=0.55249, P=0.0001); of DABPS, only the aerobic index was introduced into the regression equation(partial regression coefficient=0.34201, P=0.0293); of NABPS, microawakening index, apnea+low ventilation and oxygen by the time were introduced into the regression equation(partial regression coefficient=0.34216, P=0.0287;  partial regression coefficient=0.46876, P=0.0308; partial regression coefficien=0.40204, P=0.0249); of DABPS, apnea hypopnea+total time, 90 percent oxygen by saturation index (time/h) and oxygen index were introduced into the regression equation(partial regression coefficien=0.39443, P=0.0390;partial regression coefficient=0.39142, P=0.0399; partial regression coefficient=0.40360, P=0.0246).  Conclusions  The blood pressure of OSAHS patients lost normal rhythm variation, representing that the blood pressure increases in day and night.For all OSAHS patients, the higher the AHI, night-awakening index, 90 percent low saturation index and oxygen desaturation index, the more probably the patient sufferes from hypertension.For OSAHS patients with hypertension, daytime shrink pressure positively correlated with night-awakening index; daytime diastolic pressure is positively correlated with oxygen desaturation index; night shrink pressure is positively correlated with night-awakening index, apnea+hypopnea total time and oxygen desaturation index; the night diastolic pressure is positively correlated with apnea+hypopnea total time, 90 percent low saturation index and oxygen desaturation index.
