An analysis of income compensation of public hospitals based on a quasi-experimental design ——a case study of a first-class hospital in Shandong Province

Abstract: Objective To evaluate the effects of the compensation mechanism reform in public hospitals on the income structure and to provide references for policy making. Methods The quasi-experimental study design was a time series study analyzing the monthly income changes of a provincial comprehensive public hospital in Shandong Province. The time from Jan. 2014 to Jun. 2016 was taken as the pre-reform, while the period between Jun. 2016 and Dec. 2017 as the post-reform. Results The monthly trend changes of the percentage of income from drugs, and income from medical services before and after the reform was -0.124% and 0.146%, respectively, and the differences were statistically significant(both P<0.01). After the reform, the median proportion of income from laboratory tests, examinations and health materials increased to 7.96%, 9.91% and 25.57%, respectively(all P<0.001). It is suggested that the prices of surgeries and nursing services should be raised, and a dynamic cost control mechanism should be established to improve the capacity of inner management.
