阿替洛尔对急性冠脉综合征合并心律失常患者的临床疗效及预后影响

摘要： 目的探讨阿替洛尔对急性冠脉综合征合并心律失常患者的临床疗效及预后的影响。方法选取2013年12月至2017年1月住院确诊的急性冠脉综合征合并心律失常患者200例,按照入院时间分为治疗组(在对照组的基础上行阿替洛尔治疗)和对照组(采用常规方法进行治疗)各100例,治疗2周后,观察两组的临床疗效及血脂、高敏C反应蛋白(hsCRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的水平及死亡率。结果治疗组的总有效率(95%)明显高于对照组(79%),差异有统计学意义(P<0.05);两组治疗后的血脂、hsCRP、IL-6、TNF-α水平明显优于治疗前,且治疗组的血脂、hsCRP、IL-6、TNF-α水平明显优于对照组,差异有统计学意义(P<0.05);治疗组的死亡率(5%)明显低于对照组(20%),差异有统计学意义(P<0.05);治疗组的心功能分级中III级和IV级的发生率明显小于对照组,差异有统计学意义(P<0.05)。结论阿替洛尔治疗急性冠脉综合征合并心律失常患者,不仅具有降脂作用,而且可以明显抑制炎症反应,降低死亡率。

Abstract: Objective To explore the efficacy of atenolol in the treatment of acute coronary syndrome complicated with arrhythmia and its effect on the prognosis. Methods According to the admission time, 200 patients with acute coronary syndrome complicated with arrhythmia treated during Dec. 2013 and Jan. 2017 were divided into two groups: control group(n=100, received conventional treatment)and treatment group(n=100, received atenolol besides conventional treatment). After 2-week treatment, the levels of blood lipid, high-sensitivity C-reactive protein(hsCRP), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α)and mortality of the two groups were observed. Results The total effective rate was 95% in the treatment group and 79% in the control group, and the difference was statistically significant(P<0.05). After treatment, the levels of lipid, hsCRP, IL-6 and TNF-α in both groups were significantly higher than those before treatment, and these were superior in the treatment group than in the control group(P<0.05). The mortality was 5% in the treatment group and 20% in the control group(P<0.05). The incidences of grade III and grade IV cardiac function were significantly lower in the treatment group than in the control group(P<0.05). Conclusion Atenolol can not only reduce blood lipid but also inhibit inflammatory reaction and reduce mortality in patients with acute coronary syndrome complicated with arrhythmia.
