HER2和p53在胃腺癌不同衍变阶段中的表达及意义

Expressions and significances of HER2 and p53 in different evolution stages of gastric adenocarcinoma

摘要: 目的 分析HER2和p53在慢性胃炎、癌前病变和胃腺癌中的表达, 探讨其在胃腺癌发生中的作用。方法 采用SP免疫组化法检测HER2和p53蛋白在慢性胃炎(102例)、胃黏膜异型增生(158例, 其中低级别异型增生102例, 高级别异型增生56例)和胃腺癌(152例)中的表达, 分析其相关关系和临床病理意义。结果 免疫组化结果表明, 慢性胃炎组织中HER2和p53均呈阴性表达。HER2在胃黏膜低级别异型增生、高级别异型增生、胃腺癌组织中阳性率分别为3.92%、28.57%、44.09%, p53分别为0%、33.93%、50.66%。HER2和p53在低级别异型增生、高级别异型增生和胃腺癌中的表达呈逐渐增高趋势(P均<0.001)。HER2和p53在高级别异型增生和胃腺癌中的表达均呈正相关(r=0.465和0.505, P<0.001)。结论 HER2和p53在胃腺癌的发生中发挥重要作用。

Abstract: Objective To evaluate the expressions of HER2 and p53 in chronic gastritis, precancerous lesions and gastric adenocarcinomas and to explore their roles in gastric adenocarcinomas. Methods The expressions of HER2 and p53 in chronic gastritis(102 cases), gastric dysplasia (158 cases, including 102 cases of low grade dysplasia and 56 cases of high grade dysplasia) and gastric adenocarcinomas(152 cases) were determined with SP immunohistochemical staining. The relationship and clinicopathological significance were analyzed. Results Immunohistochemical results showed that no positive expressions of HER2 and p53 were found in the tissues of chronic gastritis. The positive expression rates of HER2 in gastric low grade dysplasia, high grade dysplasia and gastric adenocarcinomas were 3.92%,28.57% and 44.09%, and those of p53 were 0%, 33.93% and 50.66% respectively. The expressions of HER2 and p53 gradually increased in low grade dysplasia, high grade dysplasia and adenocarcinoma (P<0.001). Positive correlations between HER2 and p53 were found in both high grade dysplasia and adenocarcinoma(r=0.465 and 0.505 respectively, P<0.001). Conclusion HER2 and p53 play important roles in gastric carcinogenesis.
