良性前列腺增生症急性尿潴留的相关因素分析

Prostate volume, intravesical prostatic protrusion and bladder-urethra angle of  prostate as predictors of acute urinary retention in benign prostate hyperplasia

摘要:

目的  通过测定前列腺体积(PV)、膀胱内前列腺突入程度(IPP)、膀胱尿道角(BUA),探讨其在预测良性前列腺增生症患者(BPH)发生急性尿潴留(AUR)中的作用。方法  回顾分析40例BPH患者,按照有无发生过AUR,分为AUR组(A组)和无AUR组(B组),其中A组22例,B组18例,采用磁共振(MRI)测定PV、IPP、BUA并进行相关性分析。结果  两组患者PV及IPP比较差异无统计学意义(P>0.05)。A组BUA值为(49.3±11.3)°,B组BUA值为(31.3±15.8)°,两组比较差异有统计学意义(P<0.05),当BUA≥45°时,有88.9%的患者可发生AUR。结论  BUA是预测BPH患者发生AUR的重要指标之一, 经MRI测量的BUA是评估BPH患者发生AUR的一种简单、易行的方法

Abstract:

Objective  To evaluate the value of prostate volume(PV), intravesical prostatic protrusion(IPP) and bladder-urethra angle(BUA) as predictors of acute urinary retention (AUR) in men with benign  prostate hyperhlasia(BPH). Methods  40 BPH patients were divided into 2 groups. 22 patients in AUR group had surffered AUR at least one time, while the other18 patients had never underwent AUR, measured PV、IPP 、 BUA by MRI, and make a relative analysis. Results  In the clinical data, there was no significance in the PV and IPP between two groups(P>0.05). There was a statistical significance in the BUA between two groups (P<0.05).When BUA is over 45°, there is a risk of 88.9%  developing AUR. Conclusions  BUA is one of the indicators  to predict whether BPH patients have the probability of developing AUR. BUA measured by MRI is a simple way to assess AUR in BPH patients.
