华中地区唇腭裂人群主观报告的生活质量

A quality of life study based on subjective reports of the cleft lip and palate population in Central China

摘要: 目的 评估8~29岁非综合征型唇腭裂人群生活质量,探讨影响其生活质量的因素。 方法 利用CLEFT-Q量表、生活满意度量表、麦克阿瑟主观家庭社会地位量表对2021年6月至2022年12月就诊于武汉大学口腔医院正畸一科的329例非综合征型唇腭裂患者主观报告的生活质量进行横断面研究,统计描述其生活质量。并根据人口社会学信息(性别、年龄、照顾者人数、兄弟姐妹数量、主观家庭社会地位)及患病信息(单/双侧完全唇腭裂)分为不同组,采用SPSS 26.0对数据进行多元线性回归,分析其生活质量的影响因素。 结果 在相关性分析中,年龄对患者的外貌满意度有显著影响(P<0.001);主观家庭社会地位(SSS)对患者的外貌满意度(P<0.001)、健康相关生活质量(HRQoL)(P=0.011)、面部功能的满意度均有显著影响(P=0.022);不同年龄段患者的生活质量有显著差异(P<0.001)。多元线性回归分析表明,HRQoL、外貌满意度、面部功能满意度、SSS以及年龄均对唇腭裂患者的生活质量有影响。 结论 8~29岁唇腭裂患者其主观视角下的HRQoL、外貌、面部功能以及SSS对生活质量的影响呈正相关性,随着患者年龄增大,其生活质量不断下降。这是针对华中地区唇腭裂人群的,从患者主观角度出发,从单维度转向多维度的生活质量研究。此外,临床医生应当在对客观评价病情和疗效的基础上,积极询问患者主观评价,从而针对性地给出干预方案,全面提升患者生活质量。

Abstract: Objective To assess the quality of life in the non-syndromic cleft lip and palate population aged 8-29 years and to explore its effects from the patients perspective. Methods A cross-sectional study was conducted using the CLEFT-Q scale to statistically describe the subjectively reported quality of life in 329 patients with non-syndromic cleft lip and palate who visited the Department of Orthodontics I of Stomatology Hospital of Wuhan University during Jun. 2021 and Dec. 2022. The patients were divided into subgroups according to gender, age, number of caregivers, number of siblings, subjective family social status(SSS), and unilateral/bilateral complete cleft lip and palate. The data were analyzed with multiple linear regression using SPSS 26.0 to analyze the factors influencing their quality of life. Results Correlation analysis showed that age had a significant effect on patients satisfaction with appearance(P<0.001); SSS had a significant effect on patients satisfaction with appearance(P<0.001), health related quality of life(HRQoL)(P=0.011), and satisfaction with facial function(P<0.022); there were significant differences in the quality of life in patients of different age groups(P<0.001). Multiple linear regression analysis showed that HRQoL, satisfaction with appearance, satisfaction with facial function, SSS and age had an effect on the quality of life. Conclusion The effects of HRQoL, appearance, facial function, and SSS on quality of life from a subjective perspective in patients with cleft lip and palate aged 8 to 29 years were positively correlated, with quality of life decreasing as patients aged. This is a quality of life study from the subjective perspective of patients with cleft lip and palate in Central China, which has shifted from a unidimensional to a multidimensional approach. In addition, clinicians should actively ask patients about their subjective evaluations on the basis of objective assessment of their condition and efficacy, so as to give targeted intervention plans to improve their quality of life comprehensively.
