糖化低密度脂蛋白水平与冠脉粥样硬化程度的关系

Association between glycosylated low density lipoprotein and degree of coronary atherosclerosis

摘要:

目的   探讨糖化低密度脂蛋白(G-LDL)水平与冠脉粥样硬化程度的关系。方法    通过冠脉造影检查,选择糖尿病冠脉造影阴性(DM-NCHD)患者49例、糖尿病冠脉造影阳性(DM-CHD)患者50例及非糖尿病冠脉造影阴性(NC)患者50例。采用氯化硝基四氮唑蓝法检测DM-NCHD、DM-CHD及NC组的血清G-LDL水平。应用受试者工作特征曲线(ROC 曲线)评价血清G-LDL的诊断效能和最佳诊断分界值。结果   DM-NCHD组G-LDL水平低于DM-CHD组(P<0.05),高于NC组(P<0.05)。血清G-LDL水平为50.45时,灵敏度为0.80,特异度为0.84。DM-CHD组G-LDL水平与冠脉粥样硬化程度(用Gensini评分表示)呈正相关(r=0.638,P=0.001)。结论   DM-NCHD患者血清G-LDL水平明显低于DM-CHD患者,且G-LDL水平与冠脉粥样硬化程度呈正相关, G-LDL有望成为评价糖尿病患者冠脉粥样硬化风险的有效指标。

Abstract:

Objective   To investigate the relationship  between  glycosylated low density lipoprotein (G-LDL) degree of  coronary angiography.  MethodsWith reference to the results of coronary angiography examination, we selected 49 diabetic patients with negative coronary angiography results (DM-NCHD), 50 diabetic patients with positive coronary angiography results (DM-CHD), and 50 non-diabetic patients with negative coronary angiography results (NC). The G-LDL levels of the three groups were detected with nitrotetrazolium blue chloride. The diagnostic performance and optimum diagnostic boundary values of G-LDL in serum were evaluated with receiver operating characteristic curve (ROC curve). Results   G-LDL level of the DM-NCHD group was lower than that of the DM-CHD group (P<0.05), and tended to be higher compared to that of the NC group (P<0.05). When G-LDL level was 50.45, the sensitivity and specificity were 0.80 and 0.84, respectively. The G-LDL level in the DM-CHD group was positively correlated with the degree of coronary atherosclerosis (r=0.638, P=0.001). Conclusion    The G-LDL level in the DM-NCHD group is significantly lower than that in the DM-CHD group, and the G-LDL level in the DM-CHD group is positively correlated with the degree of coronary atherosclerosis, which indicates G-LDL may be an effective indicator to evaluate the risk of coronary atherosclerotic in patients with diabetes mellitus.
