Toll样受体4在小儿哮喘致病机制中的作用

Effects of Tolllike receptor 4 in the pathogenesis of asthma in children

摘要:

目的   分析小儿哮喘患者外周血单核细胞(PBMCs)中Toll样受体4(TLR4)的表达及其与PBMCs培养上清中IL-6和TNFα浓度的相关性,探讨TLR4在小儿哮喘发病中的作用。方法   将40例哮喘患儿分为哮喘发作组和哮喘静止组(各20例),以健康查体儿童20例为正常对照组。采用实时荧光定量逆转录PCR方法检测三组PBMCs中TLR4 mRNA的表达水平,采用双抗体夹心酶联免疫吸附法(ELISA)检测PBMCs培养上清IL-6和TNFα的浓度。结果   哮喘发作组PBMCs中TLR4 mRNA的表达水平高于哮喘静止组和正常对照组(P均<0.05),在哮喘发作组PBMCs培养上清中IL-6和TNFα的浓度较哮喘静止组和正常对照组高(P均<0.05),通过TLR4阻断剂抑制其作用后,上清中IL-6和TNFα表达水平降低,而TLR2阻断剂对以上两种细胞因子的表达量无影响。结论   TLR4及其对固有免疫反应的调节在小儿哮喘发病机制中起重要作用,其机制可能与促进IL-6和TNFα等细胞因子的分泌有关。

Abstract:

Objective   To evaluate the expression of TLR4 in peripheral blood mononuclear cells(PBMCs) and its relationship with IL-6 and TNF α concentrations in the culture supernatant of PBMCs in order to study the function of Toll-like receptor 4(TLR4) in the pathogenesis of asthma in children. Methods   Forty children with asthma were averagely divided into asthma group and static asthma group. Twenty physical examination children were taken as normal control group. Quantitative reverse transcription PCR was applied to detect TLR4 mRNA expression level in PBMCs obtained from the three groups. The concentrations of IL-6 and TNFα in the supernatant were detected by double-antibody sandwich ELISA method. Results   TLR4 mRNA expression level in asthma group was significantly higher than those in static asthma group and normal control group(all P<0.05). The concentrations of IL-6 and TNFα in the supernatant of asthma group were also significantly higher than those in the other two groups(all P<0.05). After PBMCs from asthma groups were treated with TLR4 antagonist, supernatant IL-6 and TNFα concentrations declined, whereas TLR2 antagonist had no effect on them. Conclusion   TLR4 and its regulatory functions in innate immune response play an important role in childhood asthma, which might be realized by promoting the secretion of cytokines, such as IL-6 and TNFα.
