c-Met在大肠癌中的表达及在大肠癌发生发展中的作用

Expression of c-Met and its role in development and  progression of colorectal cancer

摘要:

目的     研究肝细胞生长因子(HGF)受体即c-Met在大肠癌组织中的表达及其与微血管密度(MVD)的关系,探讨c-Met在大肠癌发生、发展中的作用。方法     采用RT PCR方法检测大肠癌组(80例)、大肠腺瘤组(60例)及大肠正常黏膜组(40例)3组组织中c-Met mRNA的表达,应用免疫组织化学法检测组织中c-Met蛋白和以CD105 为标记的MVD的表达,评价c-Met和MVD表达与大肠癌临床病理指标的关系。结果    c-Met在71.3%(57/80)的大肠癌组织和31.7%(19/60)的大肠腺瘤组织和17.5%(7/40)大肠正常黏膜组织中呈阳性表达。大肠癌组织中cMet在mRNA水平和蛋白水平均高于大肠腺瘤组织和大肠正常黏膜组织(P<0.01)。c-Met和MVD的表达与大肠癌的淋巴结转移、浸润深度、分化程度和Dukes分期相关(P<0.05),而与患者的性别、年龄、原发部位及肿瘤大小无关(P>0.05)。c-Met表达与微血管密度呈正相关(r=0.0346,P<0.05),c-Met表达阳性的组织中 MVD 值高于cMet 表达阴性者(P<0.01)。大肠癌伴肝转移者c-Met和MVD的表达明显高于无肝转移者(P<0.01)。结论    c-Met在大肠癌的侵袭与转移中可能发挥着重要作用,联合检测c-Met和MVD的表达有助于大肠癌预后的判断。

Abstract:

Objective     To investigate expression of c-Met, the protein tyrosine kinase receptor for hepatocyte growth factor/scatter (HGF) in colorectal cancer, its relationship with micro-vessel density (MVD), and its role in progression and metastasis of colorectal cancer. Methods     C-Met expression was detected at the protein level by immunohistochemistry and at the mRNA level by semi-quantitative reverse transcriptase-polymerase chain reaction in colorectal adenocarcinomas(80 cases), colorectal adenoma(60 cases) and normal mucosa (40 cases). MVD expression marked with CD 105 was determined and calculated by immunohistochemistry . Correlation of C-Met and MVD expressions and their association with clinicopathological features and outcome were analyzed. Results      C-Met-positive rate was 71.3% (57/80) in colorectal cancer, 31.7%(19/60) in colorectal adenoma and 17.5%(7/40) in normal mucosa. C-Met expression in colon cancer was significantly higher than that in colorectal adenoma and normal mucosa (P<0.01). C-Metand MVD expressions were associated with lymph node metastasis, tumor differentiation and Dukes stage (P<0.05). c-Met expression was correlated with MVD(r=0.0346, P<0.05). MVD in the c-Met (+) group was higher than that in the c-Met (-) group(P<0.01). Also, a significant increase in c-Met and MVD was detected in colorectal cancer with liver metastasis compared with primary colorectal cancer (P<0.01). Conclusions     c-Met may play an important role in progression and metastasis of colorectal cancer. Moreover, combined evaluation of cMet and MVD expressions might be a useful indicator for liver metastasis in patients with colorectal cancer.
