医保一体化背景下山东省城乡居民健康的公平性

Health equity of urban and rural residents in Shandong Province under the background of medical insurance integration

摘要： 目的 分析医保一体化背景下山东省城乡居民健康公平性,发现存在问题,为提高城乡居民的生命质量和改善健康公平性问题提供建议。 方法 运用欧洲五维健康量表法(EQ-5D-5L)和集中指数法对山东省城乡居民的健康状况及其公平性进行分析。 结果 不同的婚姻状态、文化程度、收入水平和就业情况下的城乡居民呈现出不同的健康状况和公平性。两周患病、慢性病患病的整体集中指数分别为 -0.076和-0.130,主要集中在收入水平较低的人群中,而低收入人群中多为农村居民。 结论 城乡居民的整体健康状况存在差异,与农村居民相比,城市居民的总体健康水平更高;但城市居民的健康不公平相对于农村居民差异较大。需要进一步完善城乡居民基本医疗保险制度。

Abstract: Objective To analyze the health equity of urban and rural residents in Shandong Province, findout the problems, and provide suggestions for improving the life quality and health equity in the background of medical insurance integration. Methods The health status and equity of urban and rural residents in Shandong Province were analyzed using EQ-5D-5L scale and concentration index method. Results Residents with different marital status, educational level, income level and employment status had different health status and health equity. The overall concentration index of two-week illnesses and chronic diseases was -0.076 and -0.130, respectively, which mainly distributed among people with low income level, especially rural residents. Conclusion There is significant difference in the overall health status between urban and rural residents. Urban residents have higher overall health status than rural residents, but also greater health inequality. The basic medical insurance system remains to be improved.
