非放大窄带成像内镜对结直肠微小息肉病理组织学类型的判定

Non-magnifying narrow band imaging for histologic prediction of diminutive colorectal polyps

摘要: 目的 探讨非放大窄带成像内镜(NM-NBI)对结直肠微小息肉(≤5 mm)病理组织学类型的预测能力.方法 对符合入选条件的293枚息肉,应用NICE分型对病理性质作出预测,然后结合活检或息肉切除术后病理结果,进行对比分析.结果 非放大NBI对结直肠微小息肉病理性质预测的敏感性为92.14%、特异性为81.05%、阳性预测值为81.64%、阴性预测值为91.85%及准确率为86.35%.结论 对于大多数结直肠微小息肉而言,非放大NBI结肠镜或许可代替传统病理组织学检查,进而避免不必要的息肉切除及降低病理检查费用.

Abstract: Objective To evaluate the significance of colonoscopy with non-mangnifying narrow band imaging (NM-NBI) in predicting the pathological histology of diminutive colorectal polyps. Methods A total of 293 lesions detected by white light colonoscopy were enrolled in this study. These lesions were predicted as neoplastic (adenoma or cancer) or non-neoplastic based on narrow-band imaging international colorectal endoscopic (NICE) classification, which were compared with the results of pathology. The sensitivity, specificity, positive and negative predictive values, and accuracy were evaluated. Results In the differentiation between neoplastic and non-neoplastic lesions, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy were 92.14%, 81.05%, 81.64%, 91.85% and 86.35%, respectively. Conclusion For most diminutive colorectal polyps, colonoscopy with NM-NBI has potential to replace pathological diagnosis and reduces costs related to post-polypectomy pathologic examinations and polyp removal.
