两种不同植骨方式治疗骨干骨不连的临床观察

Clinical efficacy comparison of two different bone graft ways on diaphyseal bone nonunion

摘要: 目的 观察指环植骨术治疗骨干骨不连的临床疗效。方法 选取四肢长骨骨不连患者30例,按就诊顺序采用随机数字表法分为观察组和对照组,各15例。对照组患者行髂骨取骨植骨术,观察组患者行指环植骨术。观察2组患者住院时间,并在患者出院半年后,根据上肢功能评分(DASH)评分、美国膝关节协会(AKS)评分[5]对骨愈合及患肢功能进行比较评定。 结果 观察组患者住院时间及DASH评分低于对照组(P<0.05,P<0.01)。观察组AKS评分高于对照组(P<0.01)。结论 指环植骨治疗骨干骨不连的有效性优于传统自体骨移植,是治疗骨不连的理想方式。

Abstract: Objective To observe the clinical effects of ring bone graft therapy of diaphyseal bone ununion. Methods A total of 30 patients with nonunion in long bone were randomly divided into observation (n=15) and control group (n=15) by registration order. Observation group was treated with iliac bone graft. Contral group was treated with ring bone graft. Length of stay was observed. In 6 months after discharge, comparison and evalution of bone healing and limb function between two groups were conducted according to Disability of Arm Shoulder and Hand (DASH) score and American knee society (ASK) knee index. Results Patients in observation group achieved shorter average hospitalization time, lower DASH score (P<0.05,P<0.01), and higher ASK knee index (P<0.01), compared to control group. Conclusion The effectiveness of ring bone graft therapy for diaphyseal bone ununion precedes traditional autogenous bone graft. Ring bone graft is an ideal way of treating bone ununion.
