梅尼埃病患者的垂直半规管功能分析

Vertical semicircular canal function in Meniere′s disease

摘要:

目的   评估梅尼埃病(MD)患者垂直半规管功能的变化并分析梅尼埃病患者垂直半规管功能与水平半规管功能的相关性。方法   应用自主研发的SRMIV全自动化良性阵发性位置性眩晕诊断治疗系统,对45例正常人及66例梅尼埃病患者通过180°非阻尼摆动试验行垂直半规管功能检测,在诱发的同时记录下相应的眼震视频和眼震曲线,并采集垂直眼震的方向及其慢相速度(SPV)参数。计算出右前半规管与左后半规管、左前半规管与右后半规管两对垂直半规管垂直眼震平均慢相速度两侧不对称比(CP)。结果   ① 66例梅尼埃病患者垂直眼震引出率为96.97%(128/132),两对垂直半规管CP值范围在0.13%~89.00%,不论是LARP垂直平面还是RALP垂直平面,MD患者诱发出的垂直眼震平均慢相速度(ASPV)均小于正常人,差异有统计学意义;② 180°非阻尼摆动试验检测出的CP之减弱方向与冷热试验之CP方向一致者为63.64%。结论   梅尼埃病患者不仅有水平半规管功能异常,同时也存在垂直半规管功能异常。运用180°非阻尼摆动试验可成功诱发垂直眼震,联合冷热试验,有助于全面了解梅尼埃病患者半规管功能。

Abstract:

Objective   To assess the function of vertical semicircular canal (VSCC) in patients with Meniere′s disease(MD)  and to analyze the relationship of the VSCC function and HSCC in MD. Methods   Sixty-six patients presenting with MD and 45 normal subjects were examined the VSCC function using the 180-degree vertical plane non-damping swing method with the developed automatic benign paroxysmal positional vertigo (BPPV) diagnosis and therapy system (SRM-Ⅳ). The course including the eye nystagmus orientation and the slow phase velocity(SPV) and other parameters were recorded with computers and videos. The vertical plane swing test was done in all subjects, calculating the average slow phase velocity of the vertical nystagmus and the canal paresis (CP) of the right anterior semicircular canal(SCC) -left posterior SCC plane(RALP) as well as the left anterior SCC-right posterior SCC plane(LARP). PC and SPSS17.0 software were used for statistical analysis. At the same time, all patients were examined the HSCC function with a caloric test. Results   ① Vertical nystagmus could be induced in 128 of 132 (96.97%) pairs in 66 MD patients while the CP range of the two pairs of VSCCs was 0.13%89.00%. 39 patients with MD had at least one pair of VSCCs CP≥20%. The average slow phase velocity(ASPV) of normal subjects was larger than that of patients with MD, and there was statistically significant difference in either LARP or RALP plane. ② CP of 180-degree vertical plane non-damping swing was consistent with caloric test in 63.64%. Conclusion   As supplement of the caloric test,180-degree vertical plane non-damping swing method may prove helpful in assessment of semicircular canal function.
