低分子肝素对重度子痫前期合并胎儿生长受限胎盘病理及HO-1表达的影响

Effect of LMWH on severe preeclampsia complicated with FGR and HO-1 expression

摘要: 目的 探讨低分子肝素干预对重度子痫前期合并胎儿生长受限(FGR)患者胎盘病理及血红素氧合酶-1(HO-1)表达的影响.方法 97例患者随机分为肝素治疗组(44例)及常规治疗组(53例),7 d为一疗程,观察胎盘质量; HE染色观察胎盘绒毛滋养细胞,绒毛间纤维素样渗出、坏死及绒毛血管减少、闭塞及梗死灶情况;免疫组化检测胎盘HO-1表达.结果 肝素治疗组胎盘质量均增加,与常规治疗组比较差异有统计学意义(P<0.05).HE染色结果显示,常规治疗组胎盘形态学观察缺血缺氧较肝素治疗组更明显(P<0.05).免疫组化检测肝素治疗组中HO-1表达更强(P<0.05).结论 低分子肝素治疗可改善重度子痫前期合并FGR胎盘的绒毛形态,改善胎盘的缺血、缺氧表现;可提高HO-1的表达,扩张血管,保护胎盘细胞免受氧化损失,促进胎儿生长发育.

Abstract: Objective To investigate the effect of low molecular weight heparin (LMWH) on severe preeclampsia complicated with fetal growth restriction (FGR) and expression of hemeoxygenase-1(HO-1). Methods A total of 97 cases were randomized into LMWH group (n=44) and control group (n=53). After 7-day treatment, changes of placental weight were observed. Villous stromal cells, fibrinoid effusion and necrosis, decrease, obstruction and infarction of villus blood vessels were observed with HE staining. HO-1 expression was detected with immunohistochemical method. Results The placental weight increased significantly in LMWH group (P<0.05). Hypoxic-ischemia changes of placenta decreased markedly in LMWH group (P<0.05). The HO-1 protein expression increased remarkably in LMWH group (P<0.05). Conclusion LMWH can improve villi morphology and ischemia hypoxia of placenta for severe preeclampsia complicated with FGR. Additionally, it can increase the expression of HO-1, dilate vessels, and protect the placenta from being oxidized, thus promoting the fetal growth.
