肺癌11C-胆碱PET显像机制与胆碱代谢的研究

Mechanism of 11C-choline PET/CT imaging and metabolism of choline in lung cancer

摘要:

目的   探讨肺癌组织11C-胆碱PET的显像及代谢的基本机制。方法   收集肺部结节或团块样病变患者84例,术前均行11C-胆碱PET显像,采用目测法及半定量法分析显像结果,计算标准摄取值(SUV)。将术后取得的病变组织及正常对照组织用RT-PCR法检测胆碱激酶(Chok)及胆碱磷酸胞苷转移酶(PCYT)基因的表达水平,并分析两种关键酶基因表达强度与SUV值的相关性。结果   56例11C-胆碱PET/CT阳性的肺癌组织中有78.57%和71.43%的病例分别存在Chok和PCYT基因的过表达(P均<0.01),两种酶都过表达者占64.29%;9例PET/CT阴性的肺癌组织中Chok、PCYT基因均表达正常;19例良性病变中Chok、PCYT基因分别存在10.53%和15.79%的病例过表达(P=0.07,P=0.08);56例PET/CT阳性的肺癌患者中,Chok、PCYT基因的相对表达强度与SUVmean均存在正相关性(r=0.51,r=0.50,P均<0.01)。结论   肺癌的11C-胆碱显像与肿瘤细胞中磷脂酰胆碱水平的异常升高有关,而磷脂酰胆碱水平的升高是由Chok及PCYT基因的过度表达引起的。

Abstract:

Objective   To investigate the mechanism of 11C-choline PET/CT imaging and choline metabolism in lung cancer. Methods   Eighty-four patients diagnosed with lung cancer or pulmonary nodules were examined by 11C-choline PET/CT before surgery. 11C-choline PET data were analyzed by the visual method and semi-quantitative method. The standardized uptake value(SUV)was measured. RT-PCR was used to detect expressions of the choline kinase(Chok)gene and phosphorylcholine-cytidyl transferase (PCYT)gene in lesions and normal tissues. The correlation between gene expressions and SUV was evaluated. Results   Among 56 lung cancer patients, who were diagnosed with lung cancer by 11C-choline PET, 78. 57% showed over-expression of the Chok gene(P<0.01), 71.43% showed over-expression of the PCYT gene(P<0.01), and 64.29% showed over-expressions of both Chok and PCYT genes. Expressions of Chok and PCYT were at the normal level in 9 lung cancer patients, who were diagnosed with benign tumor by 11C-choline PET. Only 10.53% of 19 benign tumor patients had over-expression of the Chok gene(P=0.07), and 15.79% of them had over-expression of the PCYT gene(P=0.08). Among 56 lung cancer patients diagnosed with lung cancer by 11C-choline PET, expressions of Chok and PCYT showed positive correlations with SUVmean(r=0.51,r=0.50,both P<0.01). Conclusion   The mechanism of 11C-choline PET/CT imaging is related to the increase ofphosphatidylcholine in lung cancer cells, and the increase of phosphatidylcholine results from over-expressions of Chok and PCYT.
