济南地区普通成年女性体成分对肺通气功能的影响

Effects of body composition on pulmonary ventilatory function in  female adults in Jinan area

摘要:

目的      探讨济南地区普通成年女性体成分对肺通气功能的影响。方法       采用整群随机抽样方法,在济南地区抽取20个社区(自然村)657例25~75岁健康成年女性,进行体成分相关指标体脂百分比(BF%)、脂肪体质量指数(FMI)、去脂体质量指数(FFMI)和肺通气功能指标用力肺活量(FVC)、1秒钟用力呼气容积(FEV 1.0)、峰流速(PEF)、一秒率(FEV1.0/FVC)的测定,按年龄、BF%(15%≤BF%<25%为体质量正常、25%≤BF%<30%为超重、BF%≥30%为肥胖)、FFM和FFMI的四分位数(分为Q1、Q2、Q3、Q4 4组)进行分组,采用方差分析、t检验、偏相关分析等统计方法分析肺功能和体成分指标的关系。结果      以BF%分组比较,FEV 1.0、FVC和PEF正常组>超重组>肥胖组(P<0.01),FEV 1.0/FVC正常组>超重组和肥胖组(P均<0.01);以FMI的四分位数分组比较,FEV1.0、FVC和PEF均为Q1组>Q2组>Q3组>Q4组(P<0.05),FEV 1.0/FVC Q1组>Q3组,Q1组>Q4组,Q2组>Q4组(P均<0.05);控制年龄和身高后,BF%、FMI和FVC、FEV1.0、PEF、FEV1.0/FVC呈显著相关(P均<0.05);各肺通气功能指标在FFMI不同水平间的比较及二者间的偏相关系数差异均无统计学意义(P均>0.05)。结论      济南地区成年女性高体脂百分比和体脂含量可导致一定程度的阻塞性通气功能下降;去脂体质量对成年女性的肺通气功能影响不明显。

Abstract:

 Objective    To explore the effects of body composition on pulmonary function in female adults in Jinan area. Methods     Pulmonary functions,  including forced expiratory volume in one second (FEV 1.0), forced vital capacity (FVC), FEV 1.0/FVC and peak expiratory flow (PEF) and body composition, including body fat percent (BF%), fat mass index (FMI) and fat free mass index (FFMI), were examined in 657 normal female adults selected by cluster sampling from 20 communities of Jinan area. Age range was 25~75 years old. The subjects were stratified by the age, BF% (normal, 15≤BF%<25%; overweight, 25%≤BF%<30%; obesity, BF%≥30%) and quartiles of FMI andFFMI (both were represented by Q1,Q2,Q3 and Q4). The differences among the groups with same stratified variable and correlations between body composition and pulmonary function were determined byANOVA, t-test and partial correlation. Results       FEV1.0, FVC, PEF and FEV1.0/FVC decreased with the increase of BF% from normal to obesity (all p<0.05), but, there was no significant difference for FEV1.0/FVC between the overweight and obesity groups (p>0.05); Differences of FVC, FEV1.0 and PEF between each two groups of FMI-quartile were significant, and these parameters decreased with the increase of FMI which were Q1>Q2>Q3>Q4(all p<0.05); The comparisons of FEV1.0/FVC among different FMI-quartile groups were Q4<Q1, Q4<Q2, Q3<Q1 (all p<0.05); After the age and height were adjusted, significant correlations were found between FEV1.0, FVC, PEF, FEV1.0/FVC and BF%, FMI,  respectively (all p<0.05) ; No significant differences between FEV1.0,FVC,FEV1.0/FVC and PEF were found among the FFMI-quartile groups (all p>0.05). Conclusion       Increased BF% and FM may lead to the obstructive impairment of the pulmonary function to a certain degree, while FFM doesn′t show obvious effect on the pulmonary function in female adults in Jinan area.
