冠状动脉旁路移植术后乳内动脉桥竞争血流动物模型的建立

摘要: 目的:建立乳内动脉-冠状动脉旁路移植动物模型,探讨冠状动脉狭窄程度对乳内动脉血管桥血流的影响。方法:15只小型猪在非体外循环心脏跳动下行左侧乳内动脉—冠状动脉左前降支(left anterior descending artery, LAD)旁路移植,利用血流闭塞器造成LAD不同程度狭窄,用超声血流检测仪测量不同狭窄程度时乳内动脉桥即时血流。结果:LAD无狭窄时,乳内动脉桥血流量最小,随LAD狭窄程度增加,桥血流增加,当狭窄>50%时,桥血流均有明显增加。结论:乳内动脉-冠状动脉旁路移植术后存在竞争血流,当LAD狭窄<50%时,竞争血流较大。此动物模型可重复性高,操作简单,是较为理想的研究冠状动脉竞争血流的动物模型。

Abstract: To explore the effect of coronary artery stenosis degree on internal mammary artery blood flow. Methods: Coronary artery bypass grafting was performed in 15 minipigs on beating heart under offpumping and the left internal mammary artery was anastomosed to left anterior descending artery. Different stenosis degrees of LAD were made by an occluder encircling around LAD, and the internal mammary artery flow was measured by transient transonic flowmeter. Results: The internal mammary artery flow was significantly different according to LAD stenosis degree and higher than nonstenosis when the stenosis was more than 50%. Conclusion: There are significant competitive flow between the left anterior descending artery and the internal mammary artery after coronary artery bypass grafting when the stenosis degree of LAD is lower than 50%. This is an ideal animal model to study the competitive flow between the left anterior descending artery and the internal mammary artery after coronary artery bypass grafting.
