阻塞性睡眠呼吸暂停综合征患者体内氧自由基代谢的作用

摘要: 研究持续正压通气后阻塞性睡眠呼吸暂停综合征(OSAS)患者体内超氧化物歧化酶 (SOD)、谷胱甘肽过氧化酶(GSHPX)活力和丙二醛(MDA)含量的变化,探讨OSAS患者多器官功能损害,衰老加速的机理。方法:选择OSAS患者63例行SOD、GSHPX活性和血清MDA含量检测;对OSAS患者行持续正压通气(CPAP)治疗3个月后,重新检测SOD、GSHPX活性和血清MDA含量,并选择60例健康人作为正常对照。结果:SOD、GSHPX活性和血清MDA含量,正常对照组分别为(139.6±36.6)?nmol/L、(78.6±8.0)?U/ml、(1.48±0.59)?mmol/L;OSAS患者含量分别为(104.5±39.3 )?nmol/L、(61.5±6.7)?U/ml、(5.87±1.67)?mmol/L;持续正压通气(CPAP)治疗3个月后,SOD、GSHPX活性和血清MDA含量分别为(134.5±36.1)?nmol/L、(74.6±6.6)?U/ml、(1.62±0.58)?mmol/L,与正常对照组相比,OSAS患者MDA明显升高,SOD、GSHPX活性显著降低,差异均有统计学意义(P<0.05);CPAP治疗3个月后,SOD、GSHPX活性、MDA含量与正常对照组相比差异无统计学意义(P>0.05)。结论:OSAS患者体内存在严重的氧自由基代谢失衡,CPAP治疗3个月后,氧自由基代谢可基本恢复正常,OSAS患者多器官功能损害,衰老加速可能与体内存在严重的氧自由基代谢失衡有关。

Abstract: To explore the mechanism of multiple organ dysfunction and consenescence in patients with obstructive sleep apnea syndrome(OSAS) by observing the levels of SOD, GSHPX and MDA after threemonth treatment of continuous positive airway pressure (CPAP). Methods: The contents of SOD,GSHPX and MDA were detected in 63 patients with OSAS before and after threemonth CPAP treatment and compared the results from 60 healthy controls. Results: The contents of SOD, GSHPX and MDA were 139.6±36.6 nmol/L, 78.6±8.0 U/ml and 1.48±0.59 mmol/L respectively in the healthy people, 104.5±39.3 nmol/L, 61.5±6.7 U/ml and 5.87±1.67 mmol/L respectively in patients with OSAS, and 134.5±36.1 nmol/L, 74.6±6.6 U/ml and 1.62±0.58 mmol/L in patients with OSAS after threemonth CPAP treatment. The level of MDA was higher in patients with OSAS than in the healthy people(P<0.05). The levels of SOD and GSHPX were lower in patients with OSAS than in the healthy people(P<0.01), but three months after CPAP treatment, they were not significantly different with those of the healthy people (P>0.05). Conclusion: The levels of the oxygen free radicals are imbalance in the patients with OSAS, which probably is an important reason for the multiple organ dysfunction,and this can get well after threemonth CPAP treatment.
