颞下颌关节及其周围骨性结构的三维测量

Measurement of temporomandibular joint and surrounding accessory structures

摘要: 目的 测量颞下颌关节及其周围骨性结构的解剖数据,比较左右关节差异性及同侧关节窝和髁突的匹配关系。旨在通过健侧及患侧正常结构解剖数据重建原有关节解剖结构,为个性化人工关节的设计提供数据支持。 方法 收集60例颅颌面CT数据,利用Simplant Pro 11.0 软件对颞下颌关节及其周围附属结构进行定点测量,应用 SPSS 23.0软件进行分析。 结果 在三维方向获得测量了颞下颌关节及其周围骨性结构12项数据,通过对比获得髁突与关节窝前后径比值、髁突与关节窝内外径比值。 结论 左右侧关节对称性好,所有指标差异无统计学意义,同侧关节窝和髁突存在匹配关系,以此重建原有关节解剖结构,为个性化人工关节的设计提供数据支持。

Abstract: Objective To obtain the anatomical data of temporomandibular joint and its surrounding bone structures, evaluate the differences between the left and right joints and the ipsilateral joint fossa and condyle, in order to reconstruct the original joint anatomy and provide data support for the design of personalized artificial joint. Methods The craniofacial CT data of 60 cases were collected. The temporomandibular joint and its surrounding accessory structures were measured with Simplant Pro 11.0 software. The data were then analyzed using SPSS 23.0 software. Results Altogether 12 parameters were collected in three-dimensional direction. The ratios of anterior and posterior, and internal and external diameter of condyle and articular fossa were obtained. Conclusion The left and right temporomandibular joint are symmetrical with no significant differences, and the ipsilateral glenoid fossa and condyle are well matched. Our findings can help to reconstruct the original joint anatomy and provide data support for the design of personalized artificial joint.
