Effect of metformin and hydration therapy on contrast-induced  nephropathy in patients with type 2 diabetes mellitus

Abstract:

 Objective    To evaluate the influence of hydration therapy on renal function in patients with type 2 diabetes mellitus (T2DM) using metformin during angiograph.  Methods    Sixty patients with T2DM were enrolled when they received iodine angiograph in the radiology department. According to using metformin or not, they were divided into the metformin group (n=30), and the non-metformin group (n=30) .With hydration therapy, all patients drank an extra 1000mL water based on the daily water quantity 6 hours after angiograph. Levels of indicators such as creatinine(Cr), total cholesterol(TC), triglyceride(TG), high density lipoprotein(HDL), low density lipoprotein(LDL) and uric acid(UA) were measured 3, 7 and 30 days before and after angiograph, and glomerular filtration rate (GFR) was calculated.  Results     The incidence of contrast-induced nephropathy (CIN) was 3.33% in the metformin group and 6.67% in the non-metformin group. The levels of Cr and GFR had no significant changes after angiograph within each group, and between the two groups (P<0.05). 3 days after angiograph, the levels of TC and LDL in the metformin group and TC in the non-metformin group significantly declined (P<0.05). 7 days after angiograph, the levels of UA and LDL in the metformin group significantly declined (P<0.05).  Conclusion     With hydration therapy and renal function monitoring, using metformin during iodine angiograph is relatively safe for patients with T2DM.
