Intravesical instillation of pirarubicin combined with bacillus calmette-guerin in the prophylaxis of local recurrence after the resection of bladder cancer and the changes of urinary IL-8

Abstract: Objective: To evaluate the intravesical instillation of pirarubicin(THP) plus bacillus calmetteguerin (BCG) for bladder cancer and the changes and predictive value of the urinary cytokine levels of IL-8. Methods: Sixty-two patients were subjected to removal of bladder cancer, and then intravesical instillation of THP plus BCG or only BCG. The urinary IL-8 level was measured by an enzyme-linked immunosorbent assay (ELISA). The results were studied with reference to the clinical outcome of the patients. Results: The recurrence ratio was 6.67% in the THP plus BCG group and 18.75% in the BCG group. The difference was significant(P<0.05). No remarkable side effects were produced in the THP plus BCG group. At 2 hours after BCG instillation, the mean IL-8 levels were significantly higher in responders than in non-responders(P<0.05), but the changes of the urinary IL-8 level was not significantly different between the two groups. Conclusions: THP plus BCG is more responsible than BCG alone against the local recurrence of bladder cancer. After intravesical BCG instillation, the IL-8 secretion strongly correlates with the possibility of future recurrence.
