Abstract:

To investigate the effect of diabetes mellitus(DM) and coronary heart disease(CHD) on heart rate recovery after the treadmill exercise test and the clinical value of heart rate recovery in diagnosing preclinical coronary heart disease in diabetics. Methods88 suspicious CHD patients who underwent the treadmill exercise test were divided into the nondiabetes mellitus (NDM) group(n=45) and DM group(n=43) according to a history of diabetes mellitus and blood glucose levels. Based on a coronary artery angiogram, the NDM group was divided into the noncoronary heart disease (NCHD) group (group A, n=22) and CHD group (group B, n=23), while the DM group was divided into the NCHD group (group C, n=22) and CHD group (group D, n=21). Heart rate recovery among the 4 groups was compared at 1,2,3,4 and 5?minutes after the test. The sensitivity and specificity of abnormal heart rate recovery were compared between the DM group and NDM group for the diagnosis of CHD. ResultsThe order of heart rate recovery after the treadmill exercise test at each time point of the 4 groups was Group A, C, B and D from high to low. The DM group and NDM group had the same specificity and accuracy of abnormal heart rate recovery for the diagnosis of CHD, but the former had higher sensitivity. ConclusionsThe influence of CHD on heart rate recovery was greater than that of DM. Abnormal heart rate recovery after the treadmill exercise test could be used to screen for CHD in diabetic patients.
