摘要：

目的探讨肝细胞癌(HCC)微血管和成熟血管的特征及其关系。方法选取行手术切除并经病理证实的HCC 31例，检测VEGF、Flk1、PCNA的表达情况，同时对微血管和成熟血管的数目、平均面积、总面积、周长、直径、异型指数、血管间距、表达部位，以及动脉数、静脉数、动脉比、静脉比、血管成熟指数和平均灌注分数等进行统计。HE染色计算细胞外间隙及细胞外血管外间隙。分析微血管和成熟血管特征差异及其相互关系。结果HCC微血管的数目、总面积、异型指数均大于成熟血管，而平均面积、直径和血管间距则相反(P＜0.05)。在VEGF阴、阳性表达组之间，微血管的数目、周长及细胞外间隙均存在统计学差异(P＜0.05)，但各成熟血管参数未见统计学差异(P＞0.05)。在Flk1阳性和阴性表达组，微血管的数目、异型指数、周长以及动脉数也存在统计学差异(P＜0.05)。另外，PCNA与微血管异型指数呈正相关(P=0.045, r=0.563)，与微血管间距负相关(P=0.022, r=-0.627)。其余测量参数之间差异未见统计学意义(P＞0.05)。结论HCC微血管和成熟血管特征存在较大差别，VEGF及Flk1主要参与微血管生成的调控，对成熟血管作用不大。

Abstract:

To explore differences in characteristics of microvessels and mature vessels of hepatocellular carcinoma (HCC) and to investigate their relationship. MethodsThirtyone cases of HCC proved by pathology were enrolled in this study. Continuous slices were stained by immunohistochemistry for determination of vascular endothelial growth factor (VEGF) and its receptor (Flk1), proliferating cell nuclear antigen (PCNA), CD34 and αsmooth muscle actin (SMA). Expressions of VEGF， Flk1, and PCNA were determined and vascular parameters such as number, mean area, overall area, circumference, diameter, distance among adjacent vessels (DAV), and variety indexes of microvessels and mature vessels were counted. Arterioles and veinlets, mature vessel indexes, mean perfused fraction (mPF), extracellular space (ES) and extracellular extravascular space (EES) were also calculated. ResultsNumber, overall area and variety indexes of microvessels were more than those of mature vessels, whereas mean area, diameter and DAV were less than those of mature vessels (P＜0.05). Number, circumference, and ES were different in microvessels between positive and negative VEGF expressive groups, whereas they were not different in mature vessels (P＞0.05). Number, circumference and variety indexes of microvessels and arterioles were different among different Flk1 groups (P＜0.05). In addition, PCNA was positively correlated with variety indexes of microvessels (P=0.045, r=0.563), and negatively with DAV ConclusionThere vascular characteristics between microvessels and mature vessels. VEGF and its receptor, Flk1, may regulate the angiogenesis of microvessels, but they have few effects on mature vessels.
