角巩膜铁屑样异物1例

A case report of corneoscleral iron filing-like foreign body

摘要: 目的 对1例隐匿性角巩膜铁屑样异物患者的临床表现、影像学特征、手术方法进行分析,以提高临床医生对于该病的诊疗能力。 方法 回顾我院收治的1例诊断为角巩膜异物患者的临床资料并进行分析,结合相关文献对该疾病进行总结。 结果 患者为31岁男性,左眼被铁屑崩伤1个月余,伤后曾于外院行异物取出治疗,仍有异物感,眼科查体见前房角脓栓,但未见明显异物,经眼眶CT、超声生物显微镜(UBM)检查后确诊为角巩膜异物,随后手术治疗取出异物。 结论 角巩膜异物是常见的眼外伤之一,但是对于病程较长的角巩膜异物,眼科查体往往难以发现,接诊此类患者时,应尽可能详细地采集病史、完善辅助检查,可减少该病的漏诊率。

Abstract: Objective To investigate the clinical manifestations, imaging features and surgical method of a patient diagnosed with corneoscleral foreign body, in order to improve the diagnosis and treatment of this disease. Methods The clinical data of the patient were retrospectively analyzed, and relevant literature was reviewed. Results The patient was a 31-year-old male whose left eye was injured by the crashing of iron filing one month ago. After the injury, the foreign body was removed in a local hospital, but he still had a sensation of foreign body. Ophthalmological examination showed pus plug in the anterior chamber angle, but no obvious foreign body was detected. After orbital CT and ultrasound biomicroscopy(UBM)examination, corneoscleral foreign body was diagnosed and the foreign body was removed with surgery. Conclusion Corneoscleral foreign body is one of the common ophthalmic diseases, but it is hard to diagnose if the duration is long. For such patients, medical history should be collected timely and imaging examinations should be conducted to reduce the rate of misdiagnosis.
