64排螺旋CT在冠心病中的诊断价值

64slice spiral computed tomography angiography in diagnosis of coronary heart disease

摘要: 目的以冠状动脉造影(CAG)为金标准,评价64排螺旋CT冠状动脉成像(CTA)在显示冠状动脉病变时的临床价值。方法搜集70例患者的CTA完整资料,并与两周内实施的CAG结果进行对比,对CTA显示的冠状动脉病变情况进行评估。结果在CTA显示的所有病例中,共计259支冠状动脉可用于评估,107支存在病变,101支得到CAG证实;6支由CTA显示狭窄而CAG确定为正常,另外有8例CAG确诊存在的病变,而在CTA上未能显示。CTA诊断冠状动脉病变的敏感性为92.7%,特异性为96.5%,阳性预测值为94.4%,阴性预测值为95.4%。结论CTA在显示冠状动脉病变时,具有较高的敏感性和特异性,对于病变程度的评估也比较准确,适用于临床怀疑冠心病的患者进行CAG前的筛选检查。

Abstract: To evaluate the clinical values of 64slice computed tomography angiography for coronary heart diseases based on conventional coronary artery angiography serving as the gold standard. MethodsThe results of 64slice CTA were compared with the results of CAG implemented in a recent twoweek period, and the different degrees of the coronary stenosis and pathological changes of different branches of coronary artery shown by 64slice CTA were assessed. ResultsIn 70 patients, 259 coronary arteries were evaluated with 107 lesions found by 64slice CTA. Among the lesions, 101 were diagnosed with CAG; 6 lesions were falsely positive and 8 other lesions were confirmed by CAG, which were not picked up by 64slice CTA. The sensitivity, specificity, positive predictive values and negative predictive values were 92.7%,96.5%,94.4% and 95.4% respectively for detection of the diseases. Conclusion64slice CTA has good sensitivity and specificity in detecting CAD,and it can also give a fairly accurate assessment for stenosis severity in most patients compared with CAG. Therefore,it is a useful noninvasive method for detecting coronary artery diseases.
