多层螺旋CT对肾癌的灌注研究

摘要: 目的:探讨多层螺旋CT对肾癌的灌注特点。方法: 对25例经手术病理证实为肾癌患者(肿瘤组)和22例健康志愿者(正常对照组)进行多层螺旋CT灌注扫描,所得数据送入Advanced Workstation 4.2工作站,应用perfusion3软件分别测量肿瘤组与对照组肾皮质、髓质的灌注参数:血流量(BF)、血容量(BV)、对比剂平均通过时间(MTT)、毛细血管表面通透系数(PS)、强化峰值(PV)和峰值时间(TTP)。结果:肿瘤组BF、BV、MTT和PS与对照组肾皮质、髓质的相应灌注参数间差异有统计学意义(P<0.05)。肿瘤组TTP与对照组肾皮质TTP差异无显著性,而PV差异有显著性。肿瘤组BF低于对照组肾皮质、高于肾髓质;BV高于对照组肾皮质及肾髓质;MTT与PS高于对照组肾皮质、低于肾髓质;PV低于对照组肾皮质;TTP高于对照组肾皮质。结论:肾癌CT灌注参数BF、BV、MTT、PS、PV和TTP不同于正常肾皮、髓质,有其特点。

Abstract: To study the characteristics of multislice spiral computed tomography perfusion of the renal cancer. Methods: Multislice spiral computed tomography perfusion was performed on 25 patients with renal cancer and 22 volunteers as control. All data were sent to advanced workstation 4.2, and the perfusion parameters, such as blood flow(BF), blood volume(BV), mean transit time(MTT), capillary permeability surface(CPS), peak value(PV) and time to peak(TTP) of the tumor and the renal cortex or the medulla were obtained by Perfusion 3 software. Results: BF, BV, MTT, PS and PV was statistically different between the cancer and the renal cortex or medulla of the control group(P<0.05), whereas TTP was not. BF was lower in cancer group than that in normal renal cortex, but was higher than that of medulla; BV was higher in cancer group than that in normal renal cortex or medulla; Both MTT and PS were longer in tumor group than those in normal renal cortex, but were shorter than those in medulla; PV was lower in tumor group than that in normal renal cortex; TTP was longer than that of normal renal cortex. Conclusion: The characteristics of CT perfusion imaging of the renal cancer are different from the normal kidney cortex or medulla in BV, BF, MTT and PS.
