急性期脑梗死后远隔部位损害的临床相关因素

摘要/Abstract

摘要： 目的探讨导致急性期脑梗死后远隔损害的影像及临床表现相关预测因素。方法分析2014年6月至12月在山东大学第二医院住院治疗的连续纳入的单侧大脑中动脉供血区梗死患者106例临床资料。按梗死后14 d是否出现远隔损害影像学表现分为阳性组(39例)与阴性组(67例),进行单因素相关分析,并继续追踪其梗死后3个月临床结局。结果影像学阳性患者39例,阳性率36%。阳性组其3个月临床结局差于阴性组(mRS评分,U=1 446.500,P=0.022);单因素分析显示,阳性组入院时美国国立卫生研究院卒中量表(NIHSS)评分高于阴性组(U=960.000,P<0.01),阳性组早期症状改善率低于阴性组(24 h NIHSS评分降低≥4分或降至0分;χ2=19.758,P<0.01)。结论入院时NIHSS评分和早期症状改善是急性期脑梗死后远隔损害的相关因素。

Abstract: Objective To explore the clinical and image factors associated with remote lesions in patients with acute ischemic stroke. Methods Clinical data of 106 patients with acute middle cerebral artery occlusion treated during June and Dec., 2014 were retrospectively reviewed. The patients were divided into the positive group and negative group according to imaging findings. The baseline characteristics were compared with univariate analysis. The outcome after 3 months were followed up and evaluated with mRS score. Results Of all 106 patients, 39 patients(36%)had positive imaging findings. The 3-month outcome was worse than the negative group(mRS score, U=1 446.500, P=0.022). Univariate analysis showed that positive group had higher admission NIHSS score than the negative group(U=960.000, P<0.01), and the positive group had lower rate of improvement than the negative group(24-hour NIHSS score decreased≥4 points, or down to 0; χ2=19.758, P<0.01). Conclusion Admission NIHSS score and early symptom improvement were the factors associated with remote lesions.
