女性不孕症患者心理健康状况及其影响因素分析

摘要： 目的研究女性不孕症患者的心理健康状况,寻找并分析影响女性不孕症患者心理健康状况的因素。方法采用单纯随机抽样选取在不孕不育门诊就诊的320名女性不孕症患者作为研究对象,通过自制基本情况调查表和SCL-90量表对患者进行调查。运用Logistic回归进行多因素的影响因素分析。结果本研究回收有效问卷273份,有效率为85.31%。两独立样本t检验结果显示,患者与全国常模的阳性项目数、人际关系敏感因子分、焦虑因子分、敌对因子分、恐怖因子分、偏执因子分、精神病性因子分的差异有统计学意义(P<0.05)。单因素分析结果显示,本人受教育程度、实际治疗费用、首次就诊年份、门诊治疗次数、治疗年限、进行试管婴儿治疗情况、不孕类型、夫妻关系、婆媳关系是心理健康问题的影响因素(P<0.05)。二分类Logistic回归分析结果显示,门诊治疗次数多(OR=1.63)、不孕类型的原发性不孕(OR=2.50)是心理健康问题的危险因素,夫妻关系和谐(OR=0.30)是心理健康问题的保护因素。结论女性不孕症患者与全国常模的心理健康状况相比较有差别,整体心理健康水平较低。影响患者心理健康问题的主要因素为门诊治疗次数、不孕类型及夫妻关系。

Abstract: Objective To investigate the mental health status and influencing factors of female infertile patients. Methods A total of 320 females were selected with simple random sampling from infertile outpatients and surveyed with Symptom Checklist 90(SCL-90)and a self-made questionnaire. The influencing factors of mental health status were analyzed with multivariate Logistic regression. Results Altogether 273 valid questionnaires were collected and the effective rate was 85.31%. Two independent sample t-test showed statistical differences in the number of positive items, interpersonal sensitivity factor score, anxiety factor score, hostility factor score, terror factor score, paranoid factor score and psychiatric factor score between patients and norms(P<0.05). Single factor analysis indicated that education level, actual treatment cost, first year of consultation, number of outpatient treatment, duration of treatment, treatment of in vitro fertilization(IVF), type of infertility, husband-wife relationship, and mother-in-law and daughter-in-law relationship could influence mental health(P<0.05). Conclusion The mental health status of female infertile patients is lower than that of normal adults. The main influencing factors of mental health include number of outpatient treatment, type of infertility and husband-wife relationship.
