食管癌患者血清中DKK-1的表达

Expression of serum DKK-1 in esophageal cancer

摘要:

目的 探讨DKK-1在食管癌患者血清中表达的临床意义。方法 应用酶联免疫(Elisa)法检测80例食管鳞癌患者和35例正常人血清中的DKK-1的表达水平,并与常规肿瘤标记物进行比较,探讨其在食管癌中的诊断价值。结果 食管鳞癌患者血清中DKK-1浓度为(90.44±57.41)ng/mL,而正常人血清浓度为(11.29±9.45)ng/mL,两者之间差异有统计学意义(P<0.01)。若以正常人血清DKK-1浓度95%可信区间上限为界值(14.54ng/mL),则食管癌鳞患者血清中DKK-1的敏感性为66.25%(53/80),特异性为82.86%;血清中的DKK-1的阳性率仅为17.14%(6/35),两者之间差异有统计学意义(P<0.01)。经过统计学分析显示,食管鳞癌患者血清中的DKK-1水平与肿瘤大小、分期及淋巴结转移等因素相关(P<0.05),而与性别、年龄及肿瘤的分化程度无关(P>0.05)。通过与其他标记物比较发现DKK-1的检测对食管鳞癌诊断的敏感度明显高于常规肿瘤标记物(CEA 39.44%,CYFRA21-1 25.35%,P<0.01),三者联合检测能明显提高诊断食管癌的敏感性。结论 DKK-1可作为检测食管癌的一种新的肿瘤标记物,对食管癌的诊断和病情评估有一定的辅助参考价值。

Abstract:

Objective To explore expression of DKK-1 in esophageal cancer and its clinical significance. Methods Serum DKK-1 was determined by sandwich enzyme linked immunosorbent assay(Elisa) in 80 patients with esophageal squamous cell cancer and in 35 healthy subjects.Sensitivities of serum DKK-1, CEA and CYFRA21-1 in esophageal squamous cell cancer were analyzed. Results Serum DKK-1 level in  patients with esophageal squamous cell cancer was significantly higher than that of healthy subjects(P<0.001).Based on the highest value of medicine reference of DKK-1(14.54 ng/ml),its sensitivity and specificity were 66.25% and 82.86%. There was a significant difference in sensitivity between esophageal cancers and healthy subjects(P<0.01).Serum DKK-1 in esophageal squamous cell cancers was associated  with tumor size, TNM stage and lymph node metastasis(P<0.05), but not associated with gender, age and differentiation grade(P>0.05). Sensitivity of DKK-1(66.25%) was significantly higher than that of CEA and CYFRA21-1 (39.44%,25.35%,P<0.01). Conclusion  DKK-1 plays an important role in the growth and metastasis of esophageal cancer and may be a new tumor marker or an important index in diagnosis of esophageal cancer.
