Objective: To evaluate the dynamic changes of the right atrial structure and function in atrial fibrillation using the acoustic quantification (AQ) technique. Methods: Dog models with chronic atrial fibrillation were established by rapid atrial pacing. Right atrial AQ waveforms were recorded before rapid atrial pacing and after pacing for 1, 4 and 8 weeks. Results: Compared with the control group, ESV, EDV and EREV of the atrial fibrillation group were increased, and EF and CV were decreased (P<0.05) at 4 weeks after pacing. And 8 weeks after pacing, ESV, EDV and EREV were significantly increased (P<0.001), while EF and PAER significantly decreased (P<0.001) and CV was also significantly decreased (P<0.05), and the other parameters had no significant changes in the pacing progress. In the atrial fibrillation group, compared with those before pacing, right atrial AQ parameters had no significant changes at 1 week after pacing (P>0.05); ESV, EREV and EDV were significantly increased (P<0.05) and EF was significantly decreased (P<0.05) at 4 weeks after pacing, while ESV, EREV and EDV were significantly increased (P<0.001) and EF and PAER were significantly decreased (P<0.001) at 8 weeks after pacing. Compared with the 1week group, ESV, EREV and EDV were significantly increased (P<0.01) and EF and PAER were significantly decreased (P<0.01) at 8 weeks after pacing, and other parameters had no statistical significance. Conclusions: Atrial fibrillation may result in right atrial booster pump and conduit function impairment but no changes of reservoir function. AQ technique may be used as a valuable noninvasive method for evaluating right atrial structure and function in atrial fibrillation.

Objective: To develop an algorithm of signal quality assessment of ECG in the intensive care unit. Methods: The signal quality index (SQI) was obtained by analyzing the morphological and statistical characteristics of each waveform and their relationships. The heart rate was robustly estimated based on SQI. The algorithm was evaluated using more than 6?000 hours of simultaneously acquired ECG from a 437patient subset of the MultiParameter Intelligent Monitoring for Intensive Care II database and adding real ECG noises. Results: The SQI was decreased along with a decrease of the signal noise ratio. The SQI had high-positive correlation with the QRS detective sensitivity and positive predictivity. This method provided an accurate HR estimation even in the presence of high levels of persistent noises and artifacts. Conclusion: The signal quality assessment algorithm could provide an objective estimation of ECG quality.

Objective: To investigate the therapeutic effect of lipid-coated microbubbles carrying Taxol on rat brain C6 glioma in vitro and vivo. Methods: For the in vitro study, C6 glioma cell lines were treated with Taxol-LCM or LCM. The distribution of LCM and changes of cell morphology and ultra-structure were observed by immunoflurescent microscopy and confocal microscopy. The process of LCMs entering the glioma cells was dynamically observed. For the in vivo study, 12 rats with intra-cranial C6 glioma were divided into 4 groups: groups with LCM-Taxol injection, LCM injection, Taxol injection and the control group. For the in vivo study, each rat was sacrificed to harvest the brain. Oil red O staining was used to determine the therapeutic effect and distributional characters in intracranial C6 glioma. Life spans were also compared. Results: For the in vitro study, Taxol was connected with lipidcoated microbubbles and the compound was endocyted by C6 glioma and had a killing effect. The whole endocyte process was that lipid- coated micro-bubbles first connected with the surface of glioma cells, second were redistributed in the cells, and finally degraded in the cells associated with the acidic component of cytoplasm. Oil red O staining revealed that LCM had aggregated and there was obvious necrosis in the tumor region in the LCM-Taxol injection group. In the LCM injection groups, there was LCM in tumor regions without obvious necrosis. In the Taxol injection group and the control group, there was no nectrosis or LCM. Life span comparison revealed that survival of the LCM-Taxol injection group was much longer than that of the other three groups. Conclusion: Lipid-coated microbubbles may carry Taxol to C6 glioma cells, and the compound may have a killing effect in vitro and in vivo.

Objective: The wild type FHIT gene was transfected into the human gastric cancer cell line MGC-803 in which the FHIT gene had been totally lost so as to investigate its effect on Adriamycin (ADM). Methods: The FHIT gene was transfected into the gastric cancer cell line MGC-803 by liposome. Western bolt assay was employed to determine the expression of FHIT. Cancer cells which were transfected with empty vector pRcCMV and the nontransfected gastric cancer cell line MGC-803 were viewed as a control and a blank respectively. After treatment with Adriamycin, an MTT colormetric assay was applied to determine the inhibition ratio. The apoptosis and cell cycle were analyzed by flow cytometry (FCM). Results: A stable expression of FHIT protein was obtained in transfected MGC803 cells. After treatment with Adriamycin, apoptosis cells were significantly more in FHITtransfected MGC-803 cells than in control cells and blank cells (40.66% VS 13.94%, 15.81%, P<0.01). Adriamycin-induced apoptosis can be enhanced by wild FHIT expression (P<0.05). Before and after treatment of Adriamycin, the rates of G0/G1 phase were statistically higher in FHIT-transfected cells than in control cells (74.43% VS 56.30%, 99.27% VS 95.10%, respectively). Conclusion: The FHIT gene might be involved in apoptosis and blocks the cell cycle of gastric cancer cells. Expression of the wild FHIT gene can increase the sensitivity of gastric cancer cell line MGC-803 to Adriamycin.

Objective: To explore the mRNA expression of lunx in regional lymph nodes of non-small cell lung cancer (NSCLC) and its correlation with prognosis. Methods SYBR greenⅠreal-time RT-PCR was used to determine the lunx mRNA level in 153 regional lymph nodes obtained from 75 patients with NSCLC. All patients had been followed up for 3 years. Six factors that may influence the survival rate were analyzed by a Cox regression model. Results: Expressions of lunx in regional lymph nodes had a statistical correlation with age, nutritional index, relapse within 3 years and metastasis(P<0.01, P<0.01, and P<0.05) but had no correlation with gender, tumor types and pathological grades(P>0.05). There were statistical significances between the survival of NSCLC patients and post-operative adjuvant therapy and expression of lunx (P<0.05 and P<0.01). Conclusions: The expression of Lunx in regional lymph nodes closely correlates with the survival of NSCLC patients and it may be a target gene for detection of micrometastasis from NSCLC and a marker for detection in 3 years prognosis. Quantitative real-time RT-PCR is helpful in early diagnosis of micrometastasis of the lymph node in patients with NSCLC.

Objective: To explore the diagnostic value and standard of quantitative PCR(qPCR) in pulmonary candidiasis albicance in rabbits referring to pathology. Methods: New Zealand white rabbit models of pulmonary candidiasis were developed. Thirty-six rabbits were randomly divided into two groups: the experimental group and the control group. 18 rabbits were enrolled in each group and they were randomly ranked. Rabbits in the experimental group were formed by per cutem intratracheal injection of 0.2ml bacterium fluid containing 1×108 candida albicans(C.albicans). The same volume of normal saline was given in the control group. Rabbits were killed based on a sequence day by day and the pathology of the lungs was observed. Bronchi lavage fluid (BLF) was collected to determine C.albicans by qPCR. Result: If 500 C.albicans/ml was a critical value, the qPCR and pathological results had a significant consistence: sensitivity, specificity, accuracy, positive likelihood ratio and negative likelihood ratio respectively were 71.9%, 92.5%, 83.3%, 9.59 and 0.3(P<0.001). Conclusion: The qPCR is quite consistent with pathology in diagnosis of the disease. The critical value is 500/ml.

Objective: To investigate the effect of matrine on the invasiveness and expression of heparanas-mRNA in the human colon cancer cell line HT-29. Methods: HT-29 cells were subjected to matrine of different concentrations. Fortyeight hours later, the expression of heparanase-mRNA was determined by semi-quantitative RTPCR, and the level of HPSE protein by western blot. The effect of matrine on HT29 cells was tested by cell Matrigel adhesion assay. The chemotactic migration and the invasive ability of HT-29 cells were determined by Matrigel invasion assay. Results: The expression of hepanase-mRNA and hepanase protein, adhesion, chemotactic migrations and invasiveness of matrine treated HT-29 cells significantly decreased compared with those of the controls(P<0.01). The inhibitory effect was dose-dependent(P<0.01). Conclusion: By down-regulating the expression of heparanase-mRNA and hepanase protein, matrine has a significant inhibitory effect on the adhesion and invasiveness of the human colon cancer cell line HT-29 in a dose-dependent manner within a certain range.

Objective: To study the effect of Simvastatin on the proliferation and mineralization of cultured rat calvaria osteoblasts (RCOBs). Methods: RCOBs were cultured in vitro by an enzyme-digestion method and treated with Simvastatin and rhBMP-2 in series gradient of concentration for 24 hours. Then the viability and proliferation of RCOBs were determined by MTT assay. After implantation in a 12well microtiter plate for 7 days, the mineralization nodes were stained by the von Kossa method. Photos were transferred into a black-white style and the mean mineralization area ratio was measured. Results: Simvastatin could not only stimulate the proliferation of the RCOBs in a dosedependent manner, but also increased the mineralization ratio compared with the control group, which mimicked rhBMP-2. Conclusion: Simvastatin can promote the proliferation and mineralization of RCOBs in vitro, which may partially be attributed to the osteogenesis mechanism of statins.

Objective: To investigate the protective effect of monosialoganglioside GM-1 on rat retinal tissues and the influence of GM-1 on NF-κB after ischemia-reperfusion injury. Methods: Seventy-five healthy adult Wistar rats were randomly divided into 3 groups: the control group(n=5), the NS group(n=35) and the GM-1 group(n=35). The control group received no treatment. Rats in the other 2 groups received complete retinal ischemia by increasing the intraocular pressure for 60 minutes and they were intraperitonially injected to 3 ml·kg-1 NS or 3 ml·kg-1 GM-1 at 12 and 1 hour before and immediately after ischemia. According to the survival time after reperfusion, the experimental 2 groups were subdivided into 7 time points: 0 (only ischemia), 1, 6, 12, 24, 72 and 168 hours after ischemia. Morphological changes and mean thickness of the inner retinal layers (MTIRL) of the retina were determined under light microscopy. Expression of NFκB was determined by the immunohistochemistry method. Results: Rats inner retinas had the lesion process of edema and atrophy after ischemiareperfusion injury.gradually decreased. Pathological changes of retina and activation of NFκB were obviously relieved by GM-1. Conclusion: GM-1 definitely protects against retinal ischemia-reperfusion injury and it is a potential agent for retinal ischemia-reperfusion injury.

Objective: To evaluate the relationship between WWOX expressions at the protein level and the clinic-pathological characteristics of human breast cancer. Method: Expression of WWOX was determined by immunohistochemical staining in a total of 155 tissue micro-arrays including 56 normal breast tissues, 12 ductal carcinoma in situ cases and 87 primary invasive breast carcinoma specimens. Results: A decreased or lost of WWOX expression was observed more frequently in invasive tumors (62.1%) when compared with normal breast tissues (28.6%) (P<0.01). 82.1% of ER(-)cases and 52.5% of ER(+)cases showed a decrease or lost of WWOX expression(P<0.05). 22.4% of premenopausal cases were completely negative for WWOX expression versus 51.8% for postmenopausal breast carcinomas (P<0.05). Furthermore, 23.1% of stage Ⅰ (6/26), 28.6% of stage Ⅱ (10/35), and 46.2% of stage Ⅲ (12/26) tumors showed a negative WWOX protein expression (P<0.01). Conclusions: Loss of WWOX is a common event in breast cancer. Associations of WWOX expression with ER status, menopausal status and clinical stage of the disease strengthen the hypothesis that WWOX plays a role in carcinogenesis and development of breast cancer in a pathway or pathways associated with sex steroid metabolism.

Objective: To investigate the expression of E-cadherin and VEGF in esophageal squamous cell carcinoma (ESCC) and the correlation of them with the invasion, metastasis and prognosis of ESCC. Methods: Expressions of E-cadherin and VEGF in 40 specimens of ESCC were determined by immunohistochemistry. Results: The positive rate of E-cadherin in ESCC was 27.5%, lower than that in the control group (P<0.01). The low expression of Ecadherin correlated with the invasive degree and lymph node metastasis. The positive rate of VEGF in ESCC was 78.0%, higher than that in the control group. The high expression of VEGF correlated with the invasive degree and lymph node metastasis. The expression of E-cadherin was negatively correlated with the expression of VEGF(rs=-0.4460,P<0.01). Conclusions: Expressions of Ecadherin and VEGF correlate with the biological behavior of ESCC, and so it is valuable in evaluating the intensity of metastasis and prognosis of the lymph node.

Objective: To investigate the value of alcohol flushing to predict the efficacy of nitroglycerin (GTN) on angina pectoris attacks. Methods: Two hundred and thirty-one patients with coronary heart disease, who were confirmed by coronary angiography, were enrolled in this study and they were divided into two groups according to alcohol flushing: the no-flushing group (n=111) and the flushing group (n=120). The genotype of ALDH2 was determined by polymerase chain reaction and direct sequencing. Risk factors of coronary heart disease and blood routine analysis were determined. The efficacy of nitroglycerin on angina pectoris was gained by a questionnaire. Base clinical characteristics and nitroglycerin efficacy between the two groups were compared. Results: The rate of efficacious response to GTN and the ALDH2*1 genotype in the noflushing group was significantly higher than that in the flushing group (88.3% VS 39.2%, P<0.01; 85.6% VS 41.7%, P<0.01). The rate of efficacious response to GTN was significantly higher in patients with the ALDH2*1 genotype than in patients with the ALDH2*2 genotype (80.7% VS 32.6%, P<0.01). Conclusion: Efficacy of GTN can be predicted by alcohol flushing, which may relate to the effect of gene polymorphisms.

To investigate the cytoimmunologic status of patients with breast cancer and its relationship with prognostic risk factors. Methods: One hundred and ten patients with breast cancer in Shandong Provincial Hospital between 1997 and 2002 were retrospectively analyzed. Clinical features, as well as the alternation of CD+3T, CD+4T, CD+8T, CD+4/CD+8 and LTR and their relationship with mass size, metastasis of lymph nodes and CEA were also analyzed. Results: ①There were statistical differences in the percentage of CD+4T and CD+8T and the ratio of CD+4/CD+8 between groups of different mass sizes (P<0.05 or P<0.01). The percentage of CD+4T was decreased and of CD+8T was increased when mass size was small. The ratio of CD+4/CD+8 went down with an increase of mass size. ②Percentages of CD+3T and CD+8T in patients with metastasis of axillary lymph nodes[LN(+)] were significant higher than those without metastasis of axillary lymph nodes [LN(-)] (P<0.05 or P<0.01). ratioin patients with metastasis of LN(+) than those without metastasis of LN(-) (P<0.05 or P<0.01). The percentage of CD+4T was higher in patients with metastasis of LN(+)than in patients without metastasis of LN(-), but no significant difference was found(P>0.05). ③The CEA level was significant higher in patients with metastasis of LN(+) than in patients without metastasis of LN(-) (P<0.05), and no such difference was shown between different mass-size groups(P>0.05). The increase of CD+4T percentage and the decrease of CD+4/CD+8 ratio were obvious in the CEA(+) group compared with the CEA(-) group(P<0.01), and CEA was significantly correlated with the CD+4T percentage and the CD+4/CD+8 ratio (P<0.01). Conclusion: Disorder of the cytoimmunologic situation exists in patients with breast cancer, and it is closely correlated with the highly prognostic factors of mass size and the metastasis situation of axillary lymph nodes.

Objective: To investigate the durability of the effects after cessation of lamivudine for HBeAg-negative chronic hepatitis B(CHB) patients. Methods: Forty-seven patients subjecting to lamivudine for no less than 24 months and their HBVDNA-negative duration more than 18 months were enrolled in this study. Hepatic biochemical parameters, HBV-DNA determination, and follow-up were determined after cessation of lamivudine. Results: The cumulative rate of relapse after 3, 6, 12, 24 and 36 months after cessation of lamivudine was 25.5%, 25.5%, 41.8%, 48.3% and 54.8%, respectively. Twenty-three patients relapsed and the latest happened on the 39th month. The cumulative relapse ratio at month 4, 9 and 12 was 52.2%. 65.2% and 82.6%, respectively. The durability of the effects after cessation of lamivudine was related to age at the end of treatment. Conclusion: Durability of the effects after cessation of lamivudine of HBeAg-negative patients is lower than that of HBeAg-positive ones. The younger the patient is, the lower the relapse rate.

Objective: To explore the association of genetic polymorphism of superoxide dismutase(SOD) and chronic obstructive pulmonary diseases(COPD). Methods: 70 patients with COPD (the COPD group)and 70 healthy volunteers (the control group ) were enrolled in this study. Limosis peripheral blood was taken and whole blood corpuscle genomic DNA was extracted. The genetic polymorphism of SOD1-3 genes was determined by polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP). Result: There were no logistic differences between the COPD and the control groups in the genetic polymorphism of SOD1(CuZn-SOD) and SOD2(Mn-SOD), however, there were logistic differences in the genetic polymorphism of SOD3(EC-SOD)(P<0.05). Conclusion: SOD3(EC-SOD)genetic polymorphism (C+760G,Arg213Gly) is related to the development of COPD and it may be the COPDrelated gene.

Objective: To investigate the mechanism of neurological protective effect of the Naoningkang granula on brain tissues of rats with intracerebral hemorrhage (ICH). Methods: Rats were randomly divided into five groups: the sham operation group, the ICH model group and the experimental groups (1.2, 2.4 and 4.8?mg·kg-1 Naoningkang, respectively). CollagenaseⅦ was injected into the caudate nucleus to establish a rat model of ICH. After 2 hours, the Naoningkang granula was given to the experimental groups by gastrogavage, and saline was given to the other 2 groups by gastrogavage. At 3 and 7 days after treatments, brain tissues were taken. HE staining was used to observe the pathological changes. An immunohistochemical method was used to determine the PAR-1 content in brain tissues surrounding hematoma and the dry-wet weight method to assay the water content of brains. Result: The Naoningkang granula significantly improved the pathological changes, inhibited PAR-1 expression and reduced brain edema. Conclusion: PAR-1 expression is up-regulated after ICH in rats, which might play an important role in inducing nerve cell damage and brain edema. The heat-clearing and detoxication method has a significant neurological protective effect on ICH rats probably by inhibiting PAR-1 expression and reducing brain edema.

Objective: To evaluate the clinical efficiency of uvulopalatopharyngoplasty(UPPP) combined with sub-mucous resection of the nasal septum or inferior turbinate for patients with obstructive sleep apnea-hypopnea syndrome(OSAHS). Methods: All the patients were diagnosed by polysomonography and treated by UPPP. At the same time, sub-mucous resection of the nasal septum was performed on 18 patients, submucous resection of the bilateral turbinate on 20 patients, and adenoidectomy on 2 patients. All of the nasal surgeries were performed by an endoscope. Prophylactic tracheotomy was performed on 5 severe cases. In another 2 cases tracheotomy were performed about 6-7 days before. All the patients were followed up for 1 to 2 years. Results: Symptoms of all the patients were alleviated to some degree: disappeared in 5 patients, reduced in 14 patients, alleviated in 6 patients. No complications occurred. Patients on whom tracheotomies were performed were decannulated in 6-13 days postoperatively. Conclusion: For OSAHS patients, UPPP combined with sub-mucous resection of nasal septum or inferior turbinate could give a better result. For severe OSAHS patients, prophylactic tracheotomy could avoid surgical complications.

Objective: To provide anatomical evidence for frontal sinus surgery through the agger nasi cell approach and the uncinate process approach by using the lamellar sectional anatomy image. Methods: The computerized cryomilling technique was used to obtain the continuous sectional data of the nasal parts. The frontal sinus′s anatomical boundary and the drainage path were observed. The relationship among the agger nasi cells, the uncinate process and the frontal recess was found. The distance of the center of the agger nasi cell′s anterior wall to the anterior extremity of the middle nasal concha′s root, the thickness of the agger nasi cell′s anterior wall, of the uncinate process′s root, and of the nasolacrimal canal′s medial wall connecting with the uncinate process′s root were determined. Results: The distance of the center of the agger nasi cell′s anterior wall to the anterior extremity of the middle nasal concha′s root was (7.12±0.74) mm. The thickness of the agger nasi cell′s anterior wall was (0.47±0.09)mm, of the uncinate process′s root was (1.27±0.22)mm, and of the nasolacrimal canal′s medial wall connecting with the uncinate process′s root was (1.71±0.08) mm. Conclusion: A precise data of the agger nasi cell′s anterior wall and the uncinate process can be found by the lamellar sectional anatomy image of the frontal sinus.
